EXPORT CONTROL REGULATIONS

U.S. ENHANCED PROLIFERATION CONTROL INITIATIVE (EPCI)

The worldwide proliferation of nuclear, chemical and biological weapons, and the missiles to deliver them, is a major threat to
world peace. As a result of this threat, the United States and its allies have enhanced controls on the transfer of the products and
technology that could directly or indirectly support the development of these unconventional weapons. Olympus Scientific
Solutions Americas needs to ensure that all transactions concerning its products or services are in full compliance with the
established EPCI regulations. Therefore, we will require the end-user information and end-use information for all orders being
exported out of the United States. If you are a reseller and have any reason to believe that the end-user or end-use will directly of
indirectly support the design, development, production, stockpiling or use of nuclear, chemical or biological weapons or missiles,
you are obligated to notify us immediately for further approval before proceeding.

The focus of these guidelines is to know our customer and the customer’s end-use of our products and technology. In order for us
to comply with these regulations, we need to receive a signed copy of this form before we can release your order. By signing this
form, you are assuming all responsibilities for making sure that the use of our goods are exported and re-exported in accordance
to U.S. Export Administration Regulations. Specifically, your end user is not part of the denied person list maintained by the U.S.
