Good day ,

Our company has been with business to you for almost five (5) years. And thru those years, we
have availed different plans that you offered and switched to different DSL bundles. The current
services that we have right now are:

1. two (2) 10 mbps DSL bundles , both with Land Line ( Telephone Number: 02 2853477 and 02
2825916) which will terminate in the year 2018;
2. Three (3) Iphone 6 Plus postpaid line which will terminate on 2019. Phone Numbers:
09987940854,09987940852, and 09987940850.

However, due to permanent bankruptcy of our company, Triple Star Force Security Services Inc., we
have decided to request for the termination of our DSL line provided to us by your respective
company. The said termination shall only include the DSL Line and excludes the two (2) Land Line
and three (3) Iphone 6 Plus postpaid Line, because the latter services can still be used for our
personal use.

Respectively, we pray that you waive the pre-termination fee that will be charged for our action
considering , sad to say, that our Company has no more dispensable funds left. However, we are
still willing to settle the outstanding balance of our current bill.
