2017008152 March 18, 2017 11:26 am 303 N KERR AVE/BIRCHWOOD DR On Saturday 03/18/2017 at 1126 hrs, I Officer Nagel responded to the listed location in response to a traffic crash involving a bicycle. Upon my arrival FIRE
was already on scene. Both parties refused medical attention.
Driver 1 stated that she was making a right turn onto N Kerr Ave and never saw the bicyclist. She then tapped him with her vehicle.
The bicyclist stated he was traveling south bound on N Kerr Ave when driver 1 hit him as he crossed Birchwood Ave.
An independent witness had stopped and blocked traffic with his tow truck until LEO was arrival. He stated that vehicle 1 turned off of Birchwood Ave and
struck the bicyclist and he crossed the intersection.
Driver 1 was issued a citation for failure to reduce speed. The bicyclist left on his bicycle and refused a ride.
No further to report.
2017010581 April 8, 2017 4:19 am 3899 WILSHIRE BLV/BONHAM AVE On April 8, 2017 Corporal Merritt and I, D. Johnson responded to a car collision that involved a pedestrian on Wilshire Blvd. Upon arrival EMS and fire was
already on the scene.

Driver One stated that she was driving West on Wilshire Blvd. Then all of a sudden a Pedestrian tried to run across the street directly in front of her
vehicle. Driver One did not have any time to stop and hit the Pedestrian. Due to the collision, Driver`s One right front bumper and hood of the car was
damaged along with the right side windshield.

Witness One who works for Earl`s Towing, he stated that he was checking parking lots for cars to tow. As he was leaving he noticed the Pedestrian with
dark clothing on the side of the street. Witness One stated that he almost hit her himself and was about to turn around and tell her to get away from the road.
Then Witness one said he heard a boom as Driver One hit the Pedestrian in the road.

Witness Two stated that he was driving east on Wilshire Blvd. Then all of a sudden a Pedestrian ran across the street trying to beat the traffic. The
Pedestrian made it past his car, but he seen her get hit by Driver One heading west bound whom she did not see.

Witness Three stated that the Pedestrian was getting dropped off from an Uber driver across the street from his house. The Pedestrian was originally
coming to visit him. As he was standing outside she tried to cross the street. The Pedestrian made it past incoming traffic but did not make it past oncoming
traffic as Driver One hit her. The Uber driver stated he did not see anything just heard a boom, then turned around to see what had happened.

The Pedestrian stated that she was dropped off by an Uber Driver on the south side of Wilshire Blvd. She stated that she had to cross the street to get to
Witness Three`s house. She said she made it past incoming traffic, then all of a sudden was struck by a car in oncoming traffic. She stated that she did not
see the car but just blanked out and felt pain. From the collision the Pedestrian fractured her right humerus and her right leg.The Pedestrian was transported
to NHRMC.

Corporal Merritt and I drove to NHRMC to ensure that the Pedestrian did not have life threatening injuries. Once I saw the Pedestrian, I saw that her face
was covered in blood, but she was conscious of her surroundings. Once I got her side of the story, I could smell a strong odor of alcohol coming from her
breath. The Pedestrian went on to yell out that she was not drunk, as I did not even mention to her I smelled the odor.

Investigation revealed that the Pedestrian did not see oncoming traffic as she ran across the street.
2017011255 April 14, 2017 8:34 pm 1298 S 3RD ST/KIDDER ST On the listed date and time I, Corporal B. P. McInerney responded to the area of S 3rd St and Kidder St in reference to a hit and run involving a pedestrian
pushing a bicycle. I spoke to the victim Ms. Jazmine Canty who stated she attempting to cross the street with a bicycle holding two walkie talkies when a black
in color mini van stopped and was letting her cross. As she began to cross the van began to move and struck her elbow and maybe the bike as well. Ms.
Canty went on to say she thinks the white male driving the van backed up to get around her after striking her or he ran over the bike. There were no
independent witnesses to the hit and run and the bicycle involved was not damaged in any way. EMS responded to placed a band aide on Ms. Canty`s right
elbow and no other injuries were reported. I was unable to determine if the driver of the van had knowledge of him striking Ms. Canty or not. Ms. Canty did say
he appeared to be letting her cross in front of his van. I spoke with Ms. Canty`s mother Mrs. Ladonna Singleton and informed her that without a license plate
number it may be difficult to figure out what vehicle struck her daughter.

2017011121 April 13, 2017 5:20 pm 1599 EAST PLZ/WILLARD ST Driver of Vehicle #1 stated she was backing up when she struck Pedestrian #1. Driver of Vehicle #1 stated she was only using her rear view mirrors, and did
not turn to look behind to ensure no pedestrians were in the roadway. Driver of Vehicle #1 stated she was going approximately 10 mph.

Pedestrian #1 stated she had been struck by Vehicle #1. Pedestrian #1 was transported to NHRMC due to visible abrasions on both of her knees. Pedestrian
#1 left NHRMC before contact could be made. Card containing reporting officers information and case number was left at her residence 1514 East Plaza.

Witness #1 stated Vehicle #1 was reversing at a high rate of speed. Witness #1 stated Vehicle #1 was going 15 mph.

No further.
2017011407 April 15, 2017 9:21 am 99 LULLWATER DR/KINSTON AVE On 04/16/2017, I, Officer Griffith responded to 4818 Franklin Ave. in reference to a past occured hit and run. Upon arrival, I was met by the victim.

Mr. Munto advised that he was skateboarding back from the handee hugo`s at the intersection of Market St. and Lullwater Dr. after buying a pack of
cigarettes. Munto advised that he was skateboarding down the wrong side of the road. As Munto was skateboarding back on Lullwater Dr. a silver in color
volkswagon sedan with tinted windows struck Munto. I observed abrasion injuries to Munto`s arms and legs, as well as a small cut to Munto`s foot. Munto
advised the vehicle did not stop and kept going south on Lullwater Dr. Munto advised he did not get a plate number from the vehicle. Munto advised there
were a few witnesses, however, Munto did not ask for there information. Munto did not call 911 because he did not want to go to the hospital, and have police
called at the time of the accident.

Munto advised he might know the driver of the volkswagen that hit him as Chris Pasko. Munto advised he is familiar with the Pasko subject from previous
threats. Munto advised he did want EMS to come check his injuries.

In this incident, I did a canvass and I was unable to locate witnesses or additional suspect information. I gave Mr. Munto additional information if he needed
LEO in this incident or in the future.

There`s nothing further.


2017013432 May 5, 2017 10:33 pm 4499 WILSHIRE BLV/S KERR AVE Pedestrian 1 stated he was leaving the Scotchman and trying to cross the Wilshire Blvd approximately 60 ft. from the intersection when Driver 2 was
eastbound on Wilshire attempting to make a left turn onto S Kerr Ave. Pedestrian 1 believes that Driver 2 was going too fast and looked like he forgot to make
his turn and just steered into him. Pedestrian did not cross at a crosswalk.

Driver 2 stated he was eastbound on Wilshire Blvd and going to make a left turn onto S Kerr Ave. when Pedestrian 1 darted out from the side of the road.
Driver 2 stated he did not have enough time to swerve to avoid, nor stop in time to avoid colliding with Pedestrian 1.

Driver 2 was uninjured. Driver 2`s vehicle had a small dent on the front passenger side of the hood.

Pedestrian 1 sustained minor injuries and was transported to the New Hanover Regional Medical Center.

Driver 2 was cited for NOL

I left a copy of the case number with both parties.

2017014189 May 12, 2017 3:09 am 1299 GREENFIELD ST/S 13TH ST At 1601, 5/12/17, I was dispatched to a traffic collision. Upon arrival I spoke with driver-1 who told me he was driving straight ahead through the intersection
when he heard something strike the left side of his vehicle. After stopping he realized pedestrian-2 had run into the side of his vehicle. No witnesses
presented themselves at the scene. My investigation revealed that pedestrian-2 is a 5 year old child that while in the crosswalk, failed to obey a red signal that
would have forbidden him to enter the intersection. As a result, pedestrian-2 ran North across the intersection and collided with the left side of vehicle-1. The
left rear tire drove across the right leg of pedestrian-2.
2017014592 May 16, 2017 7:41 pm 1700 DRYSDALE DR/MILITARY Driver #1 (Mr Woodburn) stated that he was traveling south down the bike / walking path that runs parallel with Military Cutoff Rd. As he was approaching
CUTOFF RD Drysdale Dr, he slowed down and thought traffic was stopping so he proceeded to cross the intersection when Driver #2 collided with his bicycle and knocking
him to the ground. He sustained injuries from the collision and was transported by EMS to Cape Fear Hospital. Driver #2 volunteered (and he agreed), for
her to take possession of his bicycle to transport it to his home and drop it off on the front porch. Driver #1 had given Driver #2 his information prior to my
arrival.

Driver #2 (Ms Hildreth) stated that she was approaching the intersection traveling west on Drysdale Dr in the outside travel lane preparing to turn right. As she
approached the intersection, she was in the process of slowing to stop when she collided with Driver #1. She did not see Driver #1 until the collision.

Witness #1 stated that she was traveling North on Military Cutoff approaching the intersection with a green light in her travel direction. As she approached the
intersection she saw Driver #2 "pull out" and collide with the bicycle.

Witness #2 stated that she was in the left hand turn lane on Military Cutoff Rd, stopped at a red light waiting to turn onto Drysdale Dr. She stated that as Mr
Woodburn was crossing the intersection across Drysdale Dr, when Driver #2 began to enter the intersection and collided with Driver #1 knocking him onto the
hood of her vehicle and then he fell to the ground.

My investigation revealed that there are contributing factors from both parties and from the landscape of the area. Ms Hildreth did not stop at the stop line
marked for her travel lane. However, the stop line for her travel lane has a placement that if you stop at that line, a drivers view in the direction where Mr
Woodburn was traveling from, is obstructed by a row of hedges. It appeared to be quite normal during the time of my investigation that other vehicle also pull
forward of the stop line on a regular basis. From my questioning of Ms Hildreth, that while she was perhaps in the process of stopping, her attention was
diverted to observation of traffic coming from her left in order to determine whether or not is was clear for her to turn "right on red".

Mr Woodburns contributing factor is the fact that he did not stop and yield to traffic on the road prior to entering the intersection. Cyclist are required to stop,
yield and walk their bicycles across intersections and not ride across the intersection without stopping to verify the path is clear and safe. He also would not
have had a clear line of vision to approaching traffic until he was right at the intersection because of the row of hedges obstructing his line of vision.

Since both parties negligence played key roles in contributing to the crash, neither operator was cited.

2017016266 May 31, 2017 1:22 am 1699 QUEEN ST/S 17TH ST Driver 2 advised that he was traveling west on Queen St. when he came to a stop at the intersection of Queen St. and S. 17th St. He advised that he was
looking left at oncoming traffic. He advised that he was at the stop sign for approximately 15 seconds and when he saw an opening he proceeded forward
across S. 17th St. When doing so he felt a collision with something in the road as if he had run over something. He proceeded across S. 17th St. out of the
way of oncoming traffic and parked his vehicle in the 1600 block of Queen St. When he exited his vehicle to see what happened he saw that he had collided
with a cyclist.

Witness 1 advised that she walking trash out to the dumpster behind Charlie Grangiers and as she was walking back she a white male in the road as if he had
toppled over on his bike. She said that a white in color truck attempted to cross S. 17th St. on Queen St. and ran over the man.

Witness 2 advised she was traveling north on S. 17th St. when she saw a man on the ground with beer all around him. She advised there was a guy pulled up
to the stop sign and he was on the phone at the time and did not see the man on the ground. She advised that as she passed by, the truck drove over the
man in the road. She then pulled over immediately to assist.

Charlie Grainger CCTV footage shows Pedalcyclist 1 riding west in the 1700 block of Queen St. and coming to a stop at the northern corner of Queen St.
Vehicle 2 is then seen pulling up to the intersection stopping just past the stop line. Pedalcyclist 1 then attempts to cross S. 17th St. and wobbles left, right,
and then left again falling to the ground in front of Vehicle 2 approximately 4 - 5 feet. Ten seconds later Vehicle 2 begins to cross S. 17th St. colliding with
Pedalcyclist 1 who was lying on his back in the roadway in front of Vehicle 2.

Investigation revealed that Pedalcyclist 1 failed to maneuver safely across S. 17th St. falling in front of Vehicle 2. Vehicle 2, unable to see Pedalcyclist 1,
proceeded across S. 17th St. colliding with Pedalcyclist 1.

No charges filed.

2017015481 May 24, 2017 1:27 am Driver of Veh 1 left the scene prior to my arrival.

Pedestrian stated he was behind his car at the gas pump when Vehicle 1 backed up and hit him in the leg, before putting it into drive and leaving the scene.
He stated it was a black car with a CA license plate.

Pedestrian refused EMS and I was unable to view the video footage as the person working did not have the password to the camera system. I did check the
driveways in the surrounding area for a black car with CA plates, as I was told that the suspect may live behind the store, but did not locate same.

2017016915 June 6, 2017 10:11 pm 999 HANOVER ST/N 10TH ST On 06/07/2017 at 18:52 hours I Ofc Cummings was dispatched to a car wreck with injuries in the 1000 block of Hanover St. Upon arrival, I observed a small
child approximately two years old on the sidewalk of Hanover St. The two year old was being held by his father, who was attempting to calm the child down.
Ofc Lee, Ofc Walker, WFD and EMS were all arrival about the same time.

EMS and FIRE administered first aid to the child who had visible blood coming from about his left shoulder, mouth and nose. The child was crying loudly and
did not appear to lose consciousness during first aid. The child was wearing a white and yellow striped shirt with blue jean shorts and a diaper. The child
was identified as Jadence Alexander Strayhorn dob 03/03/2015. The father of the child was identified as Christopher Dante Strayhorn dob 08/07/1991.

Chris stated that he lives at 1010 Hanover St and was walking to his car when he heard a "thud" sound. Chris turned around and saw a late model Mercury.
He then noticed his son laying on the ground. Jadence did not cry at first, but as soon as he heard his father`s voice, he started crying. Chris ran over to
Jadence, picked him up and carried him over to the sidewalk. A bystander called 911 and flagged down officers upon arrival.

The vehicle was identified as a 2002 Mercury Grand Marquis belonging to Willie Clark at 7320 Ann St Wilmington, NC 28401. The Mercury had light damage
to the passenger side headlamp lens. A piece of the vehicle`s headlamp was located 71.2 feet west in the roadway from the front of the vehicle.

The driver of the Mercury was identified as Marcus Alonzo Clark Jr of 1205 Walnut St. Marcus stated that he had just left the stop sign at N 10th and
Hanover. He was traveling East on Hanover St. when a small child ran out in front of his car. Marcus immediately stopped the car, got out and attempted to
render aid to the child. Marcus also assisted in flagging down the first responders.

Further investigation revealed that the Mercury was traveling East down Hanover St. when a small child ran out in front of the vehicle and the vehicle did not
have time to avoid collision with the child. The child was following his father to a parked car across the street from their residence. The father did not know
that the child was behind him, and did not see the child get hit. The child was transported by EMS to NHRMC. The child sustained a concussion, a fractured
left collar bone, a questionable fracture to his nasal plates along with small abrasion to his left shoulder and left hip. The child was admitted into the pediatric
ICU for treatment.

LT Boucher assisted with photographing the scene of the accident along with the external injuries of the child.

Adelay with DSS was notified about the incident.

Due to not being able to determine any contributing factors by the driver of the vehicle, no citation was issued at this time.

2017015707 May 26, 2017 8:43 pm 998 S COLLEGE RD/CEDAR AVE Driver 2 advised that he was traveling north on S. College Rd in the outer lane of travel. He advised that he had just left the stop light at S. College Rd. and
Cedar Ave. when he observed the vehicle to his left (Witn 1) suddenly brake. As this happened he saw Pedestrian 1 run into his lane of travel. He stated he
"slammed on the brakes" and Pedestrian 1 collided with the left front corner of his vehicle. He stated that she appeared to push herself off of the vehicle and
fall to the ground.

Witness 1 advised that he was traveling north on S. College Rd. when he observed Pedestrian 1 run across the south bound lanes of travel and onto the
center median. He advised that Pedestrian 1 then continued to run across the north bound lane and that he had to slam on the brakes to avoid colliding with
her.

Witness 2 advised that he was traveling south on S. College Rd. when he witnessed Pedestrian 1 run across the south bound lanes. He stated to his
passenger "She is gonna get hit" and not soon after she did. He advised that Driver 1 did everything he could to stop.

Witness 3 reiterated Witness 2`s statement.

Witness 4 advised that she was traveling south on S. College Rd. when she witnessed Pedestrian 1 run across the south bound lanes. She advised that traffic
was heavy and Pedestrian 1 attempted to cross the northbound lane but collided with Vehicle 2.

Pedestrian 1 advised that she crossed over S. College Rd. and as she crossed the median Vehicle 2 sped up and hit her.

Investigation revealed that Pedestrian 1 failed to yield the right of way to the vehicles traveling on S. College Rd. and crossed in a non-designated crossing
section.

Pedestrian 1 was cited for Failure to Yield to All Vehicles.

No Further.
2017016102 May 30, 2017 11:45 am 2392 CAROLINA BEACH RD Driver one was in the parking lot of 2392 Carolina Beach Road waiting to turn right. Driver one stated he looked to his right and to his left and then began
making his turn. Driver one stated he then saw driver two on her bicycle out of the corner of his eye but could not stop before striking the bicycle.

Driver two stated she was riding her bicycle on the sidewalk. Driver two stated she felt as if she was in driver one`s blind spot and he didn`t see her as he was
pulling out. Driver two stated she was struck by vehicle one as it pulled out.

2017019314 June 28, 2017 9:44 am 3699 BETHEL RD/WALTMOOR RD Investigation revealed that vehicle #1 was east on Waltmoore Rd behind heavy traffic. Vehicle #1 then proceeded with a left turn onto Bethel Rd, and collided
into a pedestrian and the pedestrians dog, who were both west bound in the intersection. Vehicle #1 failed to yield to pedestrian; no citation issued.

Driver #1 stated that he was in slow and heavy traffic when he made a left turn onto Bethel Rd. Driver #1 also advised that that the sun was in his eyes and
had trouble seeing the roadway. Driver #1 said that he then collided into the pedestrian and her dog. Driver #1 had no further to report.

Unit #2 (pedestrian), stated that she was walking west bound across Bethel Rd with her dog. Unit #2 then stated that she and her dog were then struck by
vehicle #1. Unit #2 said that she had gone to MEDAC prior to meeting with officers, and said that she had a sprained left wrist, and had pain in her left knee
and right hip area. Unit #2 advised that she had also taken her dog to the veterinarian clinic, prior to meeting with officer, due to injury to the dog`s left front
leg and left ear. Unit #2 said that her dog will be placed in an intensive care unit through out the night. Unit #2 had no further to report.

2017023435 July 31, 2017 7:16 am 822 S COLLEGE RD Vehicle # 1 (blue nissian sentra, AHJ-7083)
Pedestrian #1 (W/F)

Driver #1 stated that she was going east through the PVA of McDonalds, traveling at low rate of speed. Driver #1 stated that the sun was in her eyes, when
she hit pedestrian #1. Driver #1 stated that pedestrian #1 was coming out of McDonald`s, driver #1 did not see the pedestrian in the middle of the parking lot
when she hit her with the car.

Pedestrian #1 was coming out of the South exit of McDonald`s when she stopped in the middle of the parking lot to light a cigraette. At this time she was hit in
the back by vehicle #1.

New Hanover EMS, looked over pedestrian #1, no injuries to the pedestrian.

The investigation relieved, driver #1 was traveling east in the PVA. Pedestrain #1 was heading South out of the building, pedestrain #1 stopped in the parking
lot to light a cigarette. Driver #1 stated the sun was in her eyes, when she hit pedestrain #1. No damage to vehicle #1.
2017023535 July 31, 2017 7:43 pm 3698 S COLLEGE RD/S 17TH ST On 07/31/2017, I, Officer Savage was dispatched to S 17th and S College Rd in reference to a traffic crash.

Driver 2 stated he was riding his bicycle on S 17th St crossing the intersection of S College Rd heading east. He stated while crossing S College Rd on the
cross walk he had a green light to cross. He stated while he crossed over S College Rd, vehicle 1 struck him on his bicycle. Driver 2 stated vehicle 1 was
making a right turn from Waltmoore Dr heading north on S College Rd at the time of impact.

Driver 1 stated he was operating vehicle 1 making a right turn from Waltmoore Dr onto S College Rd heading north. He stated he had a red light t the
intersection of S College Rd. He stated he was stopped at red light and was looking to his left to make the right turn onto S College Rd. He stated when he
made the right turn onto S College Rd he crashed into driver 2.

Witness 1 and 2 stated they were both riding there bikes and saw the traffic crash happen. They stated driver 2 had a green light to cross over S College Rd.
They stated vehicle 1 crashed into driver 2.

EMS rendered first aid to driver 1 and transported him to NHRMC.

Driver 2 sustained injuries to most of his body (scratches/ bruises) driver 1 also sustained a broken finger and broken right leg.

Driver 1 was cited for safe movement violation GS 20-154)


2017025136 August 13, 2017 12:47 am 5020 MARKET ST Pedestrian stated he was struck at 1700 hrs on 08/13/2017 in the PVA of 4903 Market St. Pedestrian reported this incident at 0047 hrs on 08/14/2017.
Pedestrian is blind and was unable to advise the make or model of the vehicle or direction of travel of vehicle, but stated it was a white vehicle. Pedestrian
was transported to Cape Fear by EMS. No further information.
2017026188 August 22, 2017 1:13 pm 1929 OLEANDER DR Driver 1 left prior to my arrival.

Mr. Huynh advised that he was walking to his vehicle in the parking lot and when he crossed behind vehicle 1, the driver backed into him. He advised that the
driver then got out of the vehicle as well as the passenger. He stated that the driver told the passenger that she did not see him and then the passenger got
into the driver seat and left the area.

EMS responded prior to my arrival and check out Mr. Huymh. He advised that his back and left arm was hurting.

I was not able to follow up on this because of call volume in the city and county.

2017028692 September 12, 2017 7:05 am 1429 COMMONWEALTH DR/PRIVATE On September 12, 2017 at 0734 hours, I (Officer Kuzian) was dispatched to 1415 Commonwealth Dr, the PVA of McDonald`s. Upon arrival I spoke to the
driver of vehicle #1, Lois Liborio. Ms. Liborio stated she entered the PVA of McDonald`s from Military Cutoff and began traveling north. Ms. Liborio stated she
saw a man dart in front of her. Ms. Liborio stated she rolled down the window and saw the man on the ground. Ms. Liborio stated she asked if he was ok and
the man stated yes. Ms. Liborio stated she saw the man was bleeding from his elbow.

The pedestrian was Richard Nelson. Mr. Nelson stated he parked his vehicle facing east and exited the vehicle. Mr. Nelson stated he did not see any
vehicles approaching and began to cross. Mr. Nelson stated he was struck by vehicle #1 in the left hip. Mr. Nelson stated his upper body hit the hood of
vehicle #1, and then struck the pavement causing the scrape on his arm.

Mr. Nelson was checked out on scene by WFD and EMS. Mr. Nelson refused to be transported to the hospital. Minor damage was done to vehicle #1.
At this time I am unable to determine who is at fault. I believe the weather conditions played a big part in this incident. Visibility was extremely low at this time.

No further information at this time.


2017032115 October 8, 2017 9:32 pm 399 S KERR AVE/PATRICK AVE Driver 1 stated that he was pedalling his bycicle northbound on Kerr Avenue and when crossing over Patrick Avenue, he did not see Driver 2 making a left
turn onto Patrick Avenue therefore causing Driver 2 to strike Driver 1.

Driver 2 stated she was making a left turn from South Kerr Avenue onto Patrick Avenue when all of a sudden Driver 1 came out of nowhere causing the
accident.

My investigation concluded that both drivers where not paying attention which caused the accident. Neither driver had injuries. Nothing further.

2017032264 October 9, 2017 10:21 pm 3799 PRINCESS PLACE DR/MOSLEY On 10/9/17 at 2233 hrs I (Officer Borton), currently assigned to the Traffic Unit, responded to the area of Princess Place Dr and Mosely Ave in reference to a
ST traffic collision involving a 7 month old in a stroller.
Upon arrival I spoke with Antoninetta Mallette, Pedestrian 1, who advised that she was walking back to her mother`s house from the Food Lion on S Kerr
Ave. She was pushing her 7 month old nephew, Chosen Oliver, (Pedestrian 2) in the stroller as they were walking west on Princess Place Drive on the south
side of the street. They got to the 3800 block of Princess Place Dr between Mosley St and Manley St when they decided to cross the street. Antoninetta
advised that she went first and made to the turn lane with Chosen but her mother, Sharon Vaught 9/14/64 (Witness 1), stayed behind to wait for cars to pass.
Antoninetta got to the turn lane and waiting for cars to pass when she heard her mother scream and turned around to see a black car swerve in to the turn
lane and clip her and the stroller. She advised that the stroller was knocked back toward the curb she had come from and her nephew was thrown from the
stroller and rolled on the pavement. She advised that the vehicle was a black 4-door car with tinted windows, the driver stopped in the turn lane and was
shouting something but they were rushing to Chosen, then the driver drove off East bound with no headlights. She described the driver a black male, tall and
slender with a brush cut. She advised that she knew the driver, and that his name was possibly Dequah Drakeford. She filled out a witness statement form
and went to hospital.
Karen Eldridge 1/28/80 (Witness 3) advised that she was driving from McDonalds and when she got to Mosley Academy she saw a black 4-door car hit a
baby. She advised that the stroller flew in the air and fell to the ground. She pulled over and saw a male getting out of the black car as if he was going to help
the people with the baby but he sped off. She filled out a Witness Statement form.
Sara Wilson 8/18/66 (Witness 2) advised that she was on Princess Place Dr when she heard a scream. She looked up to see a black 4-door car speeding
east on Princess Place Dr. She filled out a Witness Statement Form.
Sara Bailey 12/19/55 (Witess 4) advised that she was traveling on Princess Place when she saw a dark car hit a stroller and she saw a baby fly across the
street. She saw the driver get out of the car and described him as tall and slender before he got back into the car and left. She attempted to turn around and
get the tag off the car and provided DMD3760 NC when came back to a Lexus. We were able to make contact with the owner of the car and he contacted the
current driver of the car, Kourtney Moore 1/1/91. Moore advised that she had not been in any collision recently and advised that she was at Elizabeth`s Pizza
at the time of the collision. She did not match any of the descriptions given by the witnesses and they did not think that her car was the suspect car. Ms Bailey
filled out a Witness Statement Form.
ID was called to the scene to take overall photos and to collect the stroller for evidence. There were no tire marks or blood in the roadway and the stroller
had been moved by emergency personnel prior to my arrival.
I went to NHRMC and met with grandmother of Chosen, Sharon Vaught. Sharon advised that she, Chosen and Antoninetta were walking back from Food
Lion. They were crossing Princess Place from South to North. She stated that Antoninetta went across first with Chosen but they made to the turn lane and
had to stop. She said that she waited for two cars to pass then started to cross, she took three steps then heard a loud noise and saw the car swerve to avoid
her and hit the stroller. The stoller went up in the air toward the curb and Chosen was thrown from the stroller and rolled on the ground. Sharon went on to say
that the driver stopped in the turn lane and got out, he was approaching them saying "Im so sorry, I didn`t see you!" and "I`ll pay you" but Sharon advised that
she said she was going to call 911 and then the male went back to his car and left. She described him as young, lighter skinned, possibly 5`8" with a brush
cut. Ms Vaught filled out a Witness Statement form.
I made contact with Antoinetta Mallette again at the hospital and asked her if she remember the driver`s name. She advised that she was sure he was a
Drakeford and gave the name Dequah Drakeford, I walked her out to my car and showed her a picture of Dequah Drakeford. Upon seeing the picture she
advised that it wasn`t him but she knew Dequah as "Bart" and advised that the suspect is Bart`s cousin. She knows the Drakeford`s because their mother
Traci works at the Food Lion on Dawson St. I advised Antoinetta that I would be calling her or Sharon soon to schedule a photo lineup to try and identify the
driver.
Chosen had minor road rash on his forehead, right knee and right hand. He suffered a bump on his head but no serious injury. ID took photos of the
injuries.

2017033058 October 17, 2017 2:41 pm 798 S COLLEGE RD/FOUNTAIN DR Driver 1 stated that he was traveling south on S College Rd when he noticed a bicycle ahead of him. He then attempted to move from the right most lane into
the center lane, however traffic was backed up in the center lane and he could not do so. As he tried to get as much room as possible, he stated that a
vehicle in the center lane drifted towards him causing him to drift a little more to the right. At that moment he stated he did not have time to slow down and
thought there was enough room for the bicycle. He then struck Unit 2 with his right rear view mirror. Driver 1 stated that after his vehicle struck Unit 2, he did
not feel any more bumps from his vehicle.

Unit 2 stated that he was pedaling south on S College Rd. He stated that he was in the bicycle lane and was struck by Driver 1. Unit 2 stated that he had pain
in his left leg, back and neck. Unit 2 stated that he was in the space between the white line and the grass.

Upon my arrival, I observed Unit 2 lying in the middle of the road. There was little to no shoulder for a bicycle to travel. There were red scrape marks from the
bike in the road. Unit 2 was not wearing a helmet. I did not observe any noticeable injuries on Unit 2. Driver 1 seemed more worried about the Unit 2 being
okay than anything else. I instructed Driver 1 to remain in his vehicle and to pull into the nearest parking lot. I observed the broken right mirror and slight scuff
marks just under the mirror. There was no other damage to the truck. The bicycle was slightly damaged as well.

Witness 1 stated that he watched Unit 2 get struck just hard enough to push him off of his bike. Witness 1 added that no other vehicle made contact and Unit
2 was not run over by any vehicle.

Unit 2 was transported by EMS to New Hanover Regional Medical Center for possible injuries.

Driver 1 reported no injuries.


2017036156 November 13, 2017 7:28 pm 4199 MARKET ST/CINEMA DR Driver 1 advised that she was traveling west-bound on Market St, in the left lane, when she advised that suddenly, she felt an impact on the right side of
Vehicle 1. Driver 1 advised that shortly afterward, she realized that her vehicle struck a person, she stopped and pulled into the nearest parking lot at 4195
Market St (Wells Fargo Bank).

Pedestrian 1 advised that he was crossing Market St. Pedestrian 1 advised that there was a vehicle he was waiting on traveling west-bound on Market St in
the right lane, when he saw Vehicle 1 change lanes, going into the left lane, while he was crossing being struck by same.

Pedestrian 1 suffered a broken right, tibula, fibula, and foot, along with a broken nose. Pedestrian 1 also suffered abrasions on his right elbow and left hand.
EMS Paramedics and Fire Department were dispatched in reference to the injuries reported.

Vehicle 1 had a broken right side mirror. Vehicle 1 was not towed.

Investigation reveals that Pedestrian 1 did not yield the right of way while crossing Market St in an area with no designated cross-walk.

Pedestrian 1 was transported to New Hanover Regional Medical Center.

2017038599 December 6, 2017 7:30 am 899 DAWSON ST/S 9TH ST Investigation revealed pedestrian #1 failed to yield to on coming vehicles and failed to cross the roadway at a designated crossing area.

Driver #2 stated she was traveling east on Dawson St in the far right lane. Driver #2 advised as she crossed the intersection at S 9th St an unknown black
male ran north across Dawson St in front of her vehicle. Driver #2 attempted to avoid striking pedestrian #1 however she was unable to stop, and was unable
to swerve into another lane due to the high traffic volume. Vehicle #2 then struck pedestrian #1 and came to a stop. There was no apparent damage done to
vehicle #2.

I then spoke with Witness #1 Deputy Tiller with the New Hanover County Sheriff`s Office. Deputy Tiller advised he was traveling east on Dawson St behind
vehicle #2. As they crossed the intersection at S 9th St he observed pedestrian #1 enter the roadway and run in front of vehicle #2. Deputy Tiller stated
vehicle #2 was unable to change lanes due to vehicles being in the roadway next to her. Driver #2 was also unable to stop due to the distance in which the
male ran in front of her vehicle.

I attempted to speak with Pedestrian #1 however the individual was unable to speak with me due to an unknown impairing substance. Pedestrian #1 was
unable to speak, unsteady on his feet, and was attempting to remove clothing which is common with narcotics use.

2017038201 December 3, 2017 3:52 pm On 12-03-2017 at approximately 1513 hours, I Officer Collver doubled up with Officer Timken responded to a crash at Airle Road and Eastwood involving a
suv and a cyclist.

When I arrived on scene I spoke with the Driver of the bicycle who stated that he and his friends were coming off the bridge out of Wrightsville beach on the
wrong side of the road and went through the intersection of Airle road and Eastwood. The driver said he was hit by vehicle 2 when he went through the
intersection.

I spoke with the Driver of vehicle 2 who stated he was at the stop sign and was starting to turn right onto Eastwood and suddenly saw the cyclist appear in his
view and was unable to stop in time and hit the side of the bicycle.

I spoke with an independent witness who stated that she saw the kids coming off the bridge and that they were riding against traffic, when the driver of vehicle
pulled out onto Eastwood from Airle road and hit the one cyclist.

The cyclist received minor injuries and was treated by Cape Fear EMS on the scene. I was able to get a hold of the cyclist mother since he was a juvenile.
The cyclist mother arrived on scene and transported her son to Cape Fear Memorial Hospital to be further evaluated.

I didn`t cite the cyclist for failing to obey traffic laws, but advised him that when he and his friends are on the roadway on bicycle`s that they must obey all
posted traffic signs and signals, move in the same direction of travel, and act as a motorized vehicle.

2017038366 December 4, 2017 2:48 am 3298 OLEANDER DR/INDEPENDENCE The driver of vehicle 2 stated that he was stopped at a red light at the intersection of Independence Blv and Oleander Dr. The driver stated that he was facing
BLV westbound on Oleander Dr waiting to turn left, which was southbound on Independence Blv. The driver stated that when the light turned green, he made a left
turn. The driver stated that as he was going on to Independence Blv, he witnessed a subject walking in the middle of the road. The driver stated he slowed
down to possibly five miles per hour, but could not avoid hitting the subject. The driver stated he then saw the subject fall to the ground, but did not feel his
vehicle hit the subject.

A witness that was waiting at the red light was facing southbound on Independence Blv. The witness stated she saw the light then turn green, motioning for
vehicles to make a left turn and travel southbound on Indepence Blv. The witness stated she then saw the subject enter the intersection. The witness stated
that the driver was traveling extremely slow when she saw the vehicle hit the subject in the road.

The pedestrian stated that he did not remember which direction he was walking when he entered the intersection of Indpendence Blv and Oleander Dr. The
pedestrian stated that he also does not usually pay attention to crosswalk signs and that he did not know if he was allowed to cross the intersection or not at
the time of the crash. The pedestrian also stated that he was wearing all black clothing at the time and that the driver of the vehicle would not have been able
to see him.

There was no damage on the vehicle and the driver stated he did not know where on the vehicle he struck the pedestrian. He stated that it could possibly
have been the front right section. The pedestrian had no visual signs of injury, but was transported by EMS due to complaints about right leg pain. No citations
were issued. No further information is available at this time.
2018000466 January 8, 2018 3:54 pm 899 DAWSON ST/S 9TH ST Driver 1 advised that she was traveling east on Dawson St. when she observed a traffic stop on the roadside. She continued without changing lanes because
she thought that there was a vehicle on the other side of her.

Pedestrian 2 advised that he was completing a traffic stop in the 900 block of Dawson St. As he completed the stop Vehicle 1 passed him closely causing the
right side mirror to strike his hand. He advised that he believed Vehicle 1 was traveling approximately 35 mph.

Investigation revealed that Vehicle 1 failed to move vehicle into a lane that is not the lane nearest to the emergency vehicle colliding with Pedestrian 2.

Driver 1 cited for failure to move over for emergency vehicle and driver`s license restriction violation (Rest 1).

2018004272 February 9, 2018 6:24 pm 4300 FAIRVIEW DR/GEORGE My investigation revealed that Driver 2 turned East onto George Anderson Drive off of Carolina Beach Road to then turn right (heading South) on Fairview
ANDERSON DR Drive. Driver 2 was about to turn onto Fairview Drive when Pedestrian 1 was walking across the intersection. Driver 2 stated she didn`t even see the
pedestrian and she struck the pedestrian.

Pedestrian 1 was transported by New Hanover EMS to New Hanover Regional Medical Center with minor injuries to her arm. There was no damage to
Vehicle 2.

Driver 2 was cited with Failure to Yield Right of Way of a Pedestrian.

2018004922 February 15, 2018 11:06 am 5999 MARKET ST/JUDGES RD Vehicle 1 was traveling east on Judges Road and was waiting to turn right onto Market Street. The operator of the bicycle was riding north on Market Street
on the left side of the roadway against traffic.

Driver 1 was looking left at approaching vehicles waiting for an opening in traffic so he could make his right turn into the travel lane. Driver 1 did not see the
bicycle coming up the side of the road and start to cross in front of his vehicle. Vehicle collided with the cyclist and she was knocked to the ground.

There was minor damage to the bicycle and vehicle 1. The bicycle operator was not wearing any safety equipment. She was evaluated by EMS at the
scene but refused transport.
2018004695 February 13, 2018 1:58 pm 399 HILLSDALE DR/WESLEY AVE Driver #1 and Vehicle #1 is unknown at this time.

Pedestrian #2 stated he was at 300 block of Hillsdale Dr. Pedestrian #2 stated driver #1 was going to give him some money. Pedestrian #2 stated he
reached in the passenger side window of vehicle #1 so driver #1 could hand him some money. Pedestrian #2 stated driver #1 accelerated vehicle #1 causing
vehicle #1 to drag pedestrian #2 down the road. Pedestrian #2 stated driver #1 left the scene prior to Police arrival.

2018006290 February 27, 2018 6:43 pm 3999 PRINCESS PLACE DR/BARCLAY On February 27, 2018 I, Ofc. L. Knipp responded to the 4000 block of Princess Place in response to a female that had been struck by a vehicle.
HILLS DR
Upon my arrival I met with a witness a Mr. Williams who stated that he was walking with his friend a Miss Alajah Graham. He stated that they were crossing
Barclay Hills drive when a gray car stuck his friend and kept driving toward Market St on Barclay Hills Dr.

I then spoke with Jason another witness who observed Mr. Williams and Ms. Graham walk up to the vehicle and stop it, the male talked to the driver while the
female punched the rear passenger side, fell to the ground and screamed she got hit and had a broken leg.

The female Miss Graham got transported to Cape fear Hospital. She stated that she was crossing the street and thought the vehicle making a left turn was
going to stop for her but then got hit dead center with the front of the vehicle and that she never fell to the ground and that she just walked to the side of the
street. She stated her friend Mr. Williams tried to stop the vehicle but it kept driving away on Barclay Hills Dr toward Market St.

The driver of the vehicle called into dispatch stating she was standing by at another location because she did not feel safe. I met with Kathy at Forest Hills Dr.
She stated that she was making a left turn onto Barclay Hills Dr from Princess Pl. When a black male with braids stopped her and simultaneously she heard a
banging on her rear passenger side and then a female voice saying her leg is broken. Kathy stated that the male said "you need to pay us" You need to give
us money" Kathy stated that she had cash on her because she was headed to the bank. She said that she had a visible $20 and Mr. Williams said he needed
a lot more then $20.

I observed no damage to the vehicle in both the front nor the back. I spoke with the X-ray tech at the hospital and with Miss Grahams permission got her X-
ray results. There were not broken bones, swelling or marks. Just pain that Miss Graham stated she had.

Because of the conflicting information I am unable to determine if a vehicle crash occurred or if anyone was struck by a vehicle due to no damage done to the
vehicle and not visible or internal injuries to the alleged victim.

2018006460 March 1, 2018 3:43 pm 799 MEARES ST/S 8TH ST Witness 1 advised that he and Pedestrian 2 were attempting to stop and detain Driver 1. He advised that Vehicle 1 turned from Jervay onto Meares St
heading west. As it approached S. 8th St. it stopped and met with another unknown vehicle facing east on Meares St. Driver 1 opened the door to
converse/exchange with the unknown vehicle. He advised Pedestrian 2 to attempt contact with Driver 1. He witnessed Pedestrian 2 pull his truck into the
intersection of Meares St. and S. 8th St. As Pedestrian 2 exited his vehicle to make contact with Driver 1 he witnessed Driver 1 accelerate striking Pedestrian
2 causing him to roll onto the hood and over the vehicle to the right. Vehicle 1 then fled the scene.

Pedestrian 2 advised that he was attempting to stop and detain Driver 1. When advised by Witness 1 to go ahead with the stop he positioned his vehicle in the
intersection and exited his vehicle. As he exited, his NHCS badge was displayed and he identified himself as a deputy. He stated that Vehicle 1 then
accelerated while he was in front of it. With no where to escape to he was struck by Vehicle 1 causing him to roll over the hood, onto the roof, and off to the
side. He stated that Vehicle 1 then fled the scene heading north on S. 8th St. He advised that he then got back up and returned to his vehicle to give chase.

Witness 2 advised that she saw a white male in the roadway get hit by a vehicle but was unable to describe the vehicle.

Investigation revealed that Vehicle 1 accelerated while Pedestrian 2 was in front of the vehicle. Vehicle 1 struck Pedestrian 2 causing him to roll over the
vehicle suffering injuries. Vehicle 1 then fled the scene after striking Pedestrian 2 knowing that she did or could have caused serious injury to Pedestrian 2.

Pedestrian 2 suffered injuries to his leg, back, and head.

Driver 1 charged with Felony Hit and Run.

2018009971 April 2, 2018 3:30 pm Driver of vehicle #1 stated he was backing up and he then realized he had run over his wife.
Pedestrian advised she bent over to pick up a cat in her driveway because she knew her husband could not see the cat. The next thing she knew she was
getting hit by her husbands truck.
I/O`s investigation revealed driver of vehicle #1 failed to see prior to backing up and collided with his wife in the driveway.
2018013042 April 30, 2018 12:37 pm 1099 GREENFIELD ST/S 11TH ST On 4/30/18, I, Officer Genda responded to the area of S. 11th St and Greenfield St reference a pedestrian struck by a vehicle.

Upon arrival, I found Mr. George Brookshire seated on the ground complaining of ankle pain in the pva of the Village of Greenfield. He stated that he was
walking down the hill when vehicle #1 backed into him.

I then spoke with Ms. Dorothy Garner, the driver of vehicle #1, and she stated she had been backing from the parking area of the laundry building with the
Village of Greenfield. She did not see Mr. Brookshire walking and struck him.

Mr. Brookshire was able to get up and walk and refused EMS treatment on scene.

Drivers information was exchanged.

2017004013 February 8, 2017 10:52 am 7049 NEW CENTER (BRUNSWICK) Driver #1 stated that he was parked next to the curb in the PVA of Applebee`s finishing a delivery. Driver #1 stated he was leaving from his parking area and
WAY/TOWN CENTER ( making a left turn to leave the PVA. Driver #1 stated that he heard a "bump" and when he looked in the right side mirror saw a subject laying across the curb.
Driver #1 stated that Vehicle #1 turns very wide and that he did not see Pedestrian #1 when leaving parked position.

Pedestrian #1 stated that he was walking with his handcart on the left of the truck, between the truck and the curb. Pedestrian #1 stated that as the truck pull
away and began turning to the left the back of the truck struck him, causing him to fall to the ground.

The investigation revealed that Pedestrian #1 was walking in between the truck and the curb. Vehicle #1 was leaving from a parked position and turning to the
left. The rear right corner of the truck struck Pedestrian #1.

2017003244 January 31, 2017 4:31 pm 1299 GREENFIELD ST/S 13TH ST At 1631, 1/31/17, I was dispatched to a traffic collision. Upon my arrival I spoke with driver-2 who told me she had been stopped at the red light signal. When
the traffic light turned green she entered the intersection and was struck on her passenger side door. At the hospital I spoke with the bicyclist. She told me
the brakes in her bicycle were poor and she mistakenly thought she still had a green light signal as she entered the intersection. My investigation revealed
that bicyclist-1 was operating a bicycle with poor brakes, was on the wrong side of the roadway and unlawfully entered the intersection from the side walk. As
a result, the front of the bicycle collided with the right door of vehicle-2.
2017004880 February 15, 2017 5:36 pm 399 N 23RD ST/PRINCESS PLACE DR Driver #1 stated that he was driving south on N 23rd st and was turning right (west) onto Princess Place Dr. Driver #1 stated that he had a green light and did
not see the pedestrian until he was colliding with him.

Pedestrian #2 stated that he was walking south on N 23rd St and was crossing Princess Place Dr in the marked crosswalk when vehicle #1 struck him.
Pedestrian #2 confirmed driver #1s statement about the green light. A witness confirmed that Pedestrian #2 was in the marked crosswalk and the light for
southbound traffic was green at the time of the collision.

Vehicle #1 suffered damage to the hood of the vehicle. Pedestrian #1 suffered several visible abrasions to his face and arms from the collision, and was
transported to the hospital for further examination. As of 02/16/2017 no further injuries have been reported. Investigation revealed that driver #1 failed to yield
right of way to the pedestrian at the intersection. Driver #1 was cited for failure to yield to a pedestrian and driving with an expired license. No further
information was available at this time.
2017005191 February 18, 2017 6:55 pm 199 S 16TH ST/ORANGE ST DRIVER OF VEHICLE #1 STATED SHE WAS SOUTH BOUND ON S 16TH ST AND CHANGING THE STATION ON THE CAR RADIO. DRIVER OF
VEHICLE #1 STATED SHE NEVER SAW THE VICTIM ON HIS BICYCLE AND RAN INTO THE BACK OF HIM KNOCKING HIM OFF HIS BIKE. THE RIDER
OF VEHICLE #2 WAS TRANSPORTED TO N.H.R.M.C. DUE TO THE RIDERS INJURIES AND BEING UNCONSCIOUS I HAVE NOT BEEN ABLE TO
SPEAK TO HIM.

and he fell back and hit his head. He stated that none of the drivers in that area look to the right before turning right, that they are always looking left at
oncoming traffic before they pull out onto Military Cutoff Rd.

Driver of vehicle 2 stated that he had stopped and pulled up to leave the pva to turn right onto Military Cutoff Rd. His vehicle was stopped on top of the
crosswalk with his front end extending past it. He looked right and did not see anything or anyone there. He then was looking left waiting for traffic to clear and
when it did he began to proceed forward when he noticed pedestrian 1 strike his right front quarter panel by his right tire and fall back. He backed up and
parked his vehicle to get out and check the pedestrian 1.

My investigation revealed based on the statements of both parties involved, the position of the pedestrian, and vehicle upon my arrival at the scene that the
pedestrian saw the vehicle was stopped in the cross walk, saw that the driver was not looking and began walking out of the crosswalk to go around in front of
vehicle 2 and was struck when he did so when driver of vehicle 2 began moving forward. Vehicle 2 was parked in the crosswalk when I arrived and pedestrian
was laying 12 feet in front of it.

EMS was on scene and transported Pedestrian 1 to NHRMC.


2017006153 February 27, 2017 12:29 pm 500 CAROLINA BAY DR/EASTWOOD On Monday 02/27/2017 at 1229 hrs, I Officer Nagel responded to the listed location when I was flagged down about a pedestrian struck by a vehicle. Upon
RD my arrival on scene I observed a female laying in the roadway and a SUV parked beyond the cross walk. There was an off duty EMT working with the female.
Driver 1 stated that she came to a stop at the cross walk. She looked right and saw the pedestrian jogging while looking at her cell phone and thought she
had plenty of time to go through her turn. She looked left top look for traffic but could not see so she started inching forward. She passed the crosswalk
completely and could finally see on coming traffic. The pedestrian then ran in front of her vehicle as she started to go. She stated " I wasn`t even doing 5 mph,
all I did was let off the break and start rolling forward." The vehicle came in contact with the pedestrian and she fell to the floor.
I spoke with the pedestrian at Cape Fear Hospital where she was transported for back pain. She stated that she was jogging and entered the cross walk
when she was hit by vehicle 1.
No citation was issued due to the parties involved having opposing stories. There was no damage to the vehicle involved. The pedestrian was treated for
lower back pain at the ER.

2017005463 February 21, 2017 3:56 pm 5099 RANDALL PKY/S COLLEGE RD Bicyclist 2 stated she was traveling west on Randall Pky. in the eastbound bicycle lane when the driver of Veh 1 traveled from a PVA north and struck her.

Driver of Veh 1 stated he was attempting to travel north from a PVA on Randall Pky. when the rider of Bicycle 2 was traveling the wrong way on Randall Pky
causing her to strike the front right of Veh 1 with the front of the bike.

I/O observed damage to the front right headlight and hood.

The bicyclist was transported to Cape Fear Memorial Hospital with a laceration to her head.

2017006409 March 1, 2017 6:55 pm 598 S 3RD ST/CASTLE ST At 1855, 3/1/17 I was dispatched to a traffic collision. Upon my arrival I spoke with driver-1 and witnesses at the scene. Driver-1 said that he was driving in
the left lane when pedestrian-2 walked in front of his vehicle. He told me he was unable to avoid a collision. Driver-1 said that after the collision, he stopped
his vehicle then backed it a short ways and moved the vehicle to the right side of the roadway. Witness (Edmonds) said he was on a motorcycle and saw a
van in the left turn lane which would impede the view driver-1 would have of the full intersection. He saw vehicle-1 collide with pedestrian-2. I later spoke by
telephone with witnesses (Nettleman and McDermott). Witness (Nettleman) said she was traveling North and saw pedestrian-2 walk into the roadway in front
of her. She said that she was forced to slow suddenly to avoid striking pedestrian-1. She called 911 after hearing on the news that there had been a
pedestrian involved collision. Witness (McDermott) said she had been following witness (Nettleman) who is her mother and was in a separate car. She gave
essentially the same information as witness (Nettleman) except that she actually saw the collision. My investigation revealed that driver-1 was traveling South
on S 3rd St. Pedestrian-2 unlawfully entered the intersection and darted, unseen into the path of vehicle-1. As a result the front right of vehicle-1 struck
pedestrian-2. Pedestrian-2 later died at the hospital as a result of his injuries.
2017006947 March 7, 2017 7:22 am 0 5501 MARKET ST Investigation revealed that a pedestrian was briefly pinned between two vehicles at the Capital Nissan Service Bay located at 5501 Market St.

The pedestrian stated that he had begun walking across the service bay, cutting between Vehicles 1 and 3, which were parked in the service bay.

Vehicle 1 was parked directly behind Vehicle 3.

As the pedestrian crossed between the vehicles, Driver 1 entered Vehicle 1. Driver 1 stated he was unaware the vehicle was in gear with parking brake up.
Driver 1 said he depressed the clutch and released the parking brake and vehicle 1 jerked forward resulting in the vehicle pinning the pedestrian between
Vehicles 1 and 3.

The pedestrian stated that Driver 1 panicked, pulled forward and back up approximately 3 times, striking the pedestrian`s legs each time.

Although Driver 1 stated that he believed he had only struck the pedestrian`s right leg, the pedestrian stated that both of his legs had been struck.

The predestrian was seen by EMS and transported to NHRMC for treatment.
2017006409 March 1, 2017 6:45 pm 599 S 3RD ST/CASTLE ST On 3/7/17 I was notified by WPD records the original report wrongly contained code injury code 2 which had been initially correct. Pedestrian-2 later died at
the hospital and the correct code for block #32 is now injury code 1. In reviewing the original report I also found that I omitted block number #64 for both units.
This supplement report corrects these errors.
2017007252 March 9, 2017 2:27 pm 2300 S COLLEGE RD/SHIPYARD BLV On March 9, 2017 at 1427 hours, I (Officer Kuzian) was dispatched to the 2800 block of S College Rd in reference to a vehicle vs a bicycle. Upon arrival I
spoke to the driver of the vehicle (Betty Sylivant). Ms. Sylivant stated she was looking left for a safe opening in oncoming traffic to make a right turn onto S
College Rd. Ms. Sylivant stated she never saw the bicyclist approach. Ms. Sylivant stated when she began the turn, and her right front bumper made impact
with the bicycle.

The male subject riding the bicycle was Anthony Still. Mr. Still was riding north bound on S College Rd. Mr. Still was on the left side of the road riding
against traffic. Mr. Still stated he was trying to make a left into the PVA of Harris Teeter.

The witness (Kerry Fitzgerald) statement coincided with Ms. Sylivant`s statement. Ms. Fitzgerald stated Ms. Sylivant pulled forward to see oncoming
traffic. Ms. Sylivant began to turn and the bicycle collided with the vehicle.

Mr. Still was transported to Cape Fear Hospital. Mr. Still currently suffers from a broken ankle, but waiting on results from further tests. Mr. Still was cited
for riding a bicycle on the wrong side of the road. Ms. Sylivant stated she did not have any injuries. Driver exchange form was given to both parties.

DR/EASTWOOD RD the above intersection. Barlow added that when he saw a clearing in the traffic, he proceeded to make a right turn onto Eastwood Rd. Barlow added that he
then struck a bicyclist that was traveling west on Eastwood Rd.

Bicyclist, Philip Blethen, advised he was traveling westbound on the bike path on Eastwood Rd. Blethen added that as he was about to cross over
Commonwealth Dr, he noticed vehicle #1 stopped at the stop sign. Blethen added that as he was crossing the intersection, vehicle #1 began to accelerate and
he struck the vehicle and fell to the ground.

Blethen complained of shoulder and neck pain, and was transported to the hospital by EMS.

Barlow was cited for safe movement violation.


2017008661 March 22, 2017 8:37 pm 799 N 3RD ST/BRUNSWICK ST Pedestrian stated he was crossing the street and never saw the vehicles headlights.
Driver of vehicle #2 stated she was driving straight ahead when she saw the the pedestrian run out in front of her vehicle. She hit the brakes but was
unable to stop in time and hit the pedestrian wiith the front corner of her vehicle.
Witness stated she was driving toward Brunswick St when she saw a man get hit by the the truck. The pedestrian crossed the intersection but the traffic on
N 3rd St had a green light.
2017009431 March 29, 2017 5:56 pm 399 N 31ST ST/PRINCESS PLACE DR On 03/29/2017 I, Officer Noah Garay responded to a hit and run call at Cape Fear Memorial Hospital.

Upon arrival I spoke with Earla, she stated that while walking home from the Prince Mini-Mart on Princess place, a Black Jeep backed into her, knocking her
over. She said that she yelled for the vehicle to stop, and when it did she got up and told the driver he just hit her. Earla then got into the suspect vehicle
where there were two Black males and one Black female. The victim also told me that there was a "strong odor of weed". She told the driver to take her to her
house so she could get the driver`s information. When they got to Earla`s house she said that she got out of the car and the car sped away heading west on
Princess Place.

Investigation finds the driver was a Black male, 35-36 years old, with dreadlocked hair. The victim stated that the passenger might have been someone
named Mike that lives in a Brown house on 31st. street. The victim sustained bruising from the fall, a scrape on her left elbow and her knees.

OCA was given.

The victim was transported to Cape Fear Memorial Hospital for minor injuries.
2017009245 March 28, 2017 2:53 pm 1099 S COLLEGE RD/MAPLE AVE Vehicle #1 is a black colored bicycle, and was being operated by its owner. This Vehicle was attempting to cross S. College Road at or near Maple Ave.

Witnesses state that the driver of the bicycle was "snaking" in between stopped cars for heavy traffic congestion. The bicycle continued east and was about in
the middle of the road when he failed to see Vehicle #2 approaching.

Vehicle #2 is a burgundy mini van, and was traveling south on South College Road in the middle turn lane. Vehicle #2 was attempting to turn left onto Maple
Ave when Vehicle #1 ( bicycle) ran into the front right quarter panel, knocking him to the ground.

The operator of the bicycle failed to cross in a cross walk and didn`t have any protective equipment on.

down by the listed witness, Alexander Laut, he advised his girlfriend was walking through the crosswalk when a black in color Nissan ran her over. He advised
that he saw his girlfriend`s legs fly into the air and he hit the passenger side vehicle window. He advised the vehicle went eastbound on Princess St. and
made a right onto N. 4th St. Witnesses were unable to provide any further information on the suspect vehicle other than a black male was driving. An
unrelated witness advised the vehicle had the green light and the victim walked out right in front of the car. I then spoke with the listed victim who was
conscious and advised she did not want EMS. I called for them routine anyway based on witness information. She advised that she was arguing with her
boyfriend and was walking away from him in the crosswalk when she was struck by the vehicle. She could not advise what happened after the impact but said
her head and knees were hurting. EMS arrived on scene and the victim declined any treatment. She advised that she just wanted to go home to the 300 block
of Orange St. I offered to give her a ride after conferring with my Corporal but she declined. The victim`s boyfriend became Irate that EMS did nothing despite
her declining treatment. Approximately 2 hours later he called back into 911 requesting EMS for his girlfriend. She again declined any treatment. The victim
was given an OCA number for her records.

2017012625 April 28, 2017 3:52 am 1 S KERR AVE/MARKET ST Driver 2 states that she was turning right onto Market St. from S. Kerr Ave. Driver 2 said that she had a green light. Driver 2 stated that Driver 1 was riding
down the side of the road and crashed into the side of Driver 2s car.

Driver 1 was not there to give his side of the story.


2017012829 April 30, 2017 8:18 pm 100 DOGWOOD DR/ROGERSVILLE RD On 04/30/2017 at approximately 2018hrs, I, Officer Salem, was dispatched to 133 Dogwood Dr in reference to a traffic incident with injuries.

Upon arrival, EMS was on scene and tending to a struck pedestrian identified as Jennifer Wightman. Jennifer was laying on her back in the road. It was very
apparent that her left leg was broken.

The driver, Scott Franks, was standing outside of his truck. I detected a slight alcohol presence, however Franks was speaking very clearly, standing on his
own and did not appear to be at all impaired. I also noticed there were slight tire impressions in the roadway.

Franks stated that he was on his boat all day and had gotten stuck out on the water making him in a hurry to get home. He added that the reason he was on
Dogwood Dr was that he was hoping it would be a faster cut through to Eastwood Rd. He advised that he was still new to the area. Franks then stated that
he was traveling west on Dogwood Dr when he noticed slight movement ahead of him. He added that he thought it may have been a dog. He advised that
his vision was obstructed by an oncoming car. Therefore, he slammed on the breaks and thought he hit something. At first he thought he had hit a dog. After
turning his truck and boat trailer around at the end of the street, he then pulled up and noticed Jennifer lying still in the road.

There was minimal damage on the front right corner of Franks`s truck.

On the scene, Jennifer stated that she had consumed five glasses of wine. She stated that she was walking her dog west on Dogwood Dr. She stated that
she was in the road but not directly in the center. She advised that she normally walks her dog off the leash and when a car comes she squats down and
calls her dog over to hook the leash back on. She then added that the next thing she knew, she was rolling in the street. Jennifer was transported to Cape
Fear Memorial Hospital.

Witness 1, Kevin Simpson, a neighbor and family friend of Jennifer, stated he saw the truck moving at at least 45 mph. Simpson also advised that he didn`t
see the collision occur, only heard a female scream.

At the hospital, Jennifer`s leg was completely wrapped up. She added that she remembered getting struck while squatting to hook her dog up.

Witness 2, Jaimie Wightman, Jennifer`s husband, arrived at the hospital and stated that his wife was not in the center of the road although was in the road,
although he did not actually see the collision occur. He estimated the truck`s speed at at least 60 mph. He added that he did not see an oncoming car like
Franks had said. Jaimie also advised that people speeding down their street is an ongoing issue.

2017011788 April 20, 2017 10:40 am 299 N 3RD ST/GRACE ST On 04/20/2017 I was dispatched to the intersection of N 3rd St. and Grace St. in reference to a traffic crash involving a car and a bicycle. When I arrived I
spoke to the driver of the car Bruce Finn Jr.
Mr. Finn stated that he had the red light and was trying to turn right on N 3rd St. Mr. Finn said he was looking left and did not see him crossing. Mr. Finn
was driving a Dodge Intrepid with a Maryland license plate of 3CM7857. The car had minor damage where the front license plate came off during the crash.
I also spoke with multiple witnesses who stated confirmed Mr. Finn`s statements. Along with those statements all the witnesses stated that the other party
involved in this crash was riding a bicycle heading North while on the side walk. The person riding the bike was hit while he was entering the crosswalk.
EMS was already on scene when I arrived and then transported the second party involved to Cape Fear hospital. By the time I finished up at the crash
scene and went to Cape Fear hospital the other party had left. The hospital gave me a name of Josh Patterson with a date of birth of 1-21-1997. I attempted
several times to call the phone number I found in RMS for him but was uable to reach him.
Mr. Patteson`s bike was left at the scene and I secured the bike by taking it to WPD property and evidence for safekeeping. No citations were given due to
both parties having contributing factors to the crash.

2017018177 June 17, 2017 3:35 pm 899 RANKIN ST/ANDERSON ST My investigation concluded that driver 1 operated his motorcycle in a reckless manner, losing control and falling off of same, which resulted in the motorcycle
colliding with a pedestrian.

A large crowd had gathered and while many people were able to recount the events of the incident, none of the passerby`s would provide their identity to be
recorded as a witness.

On scene, driver one was dazed and confused , unable to make a coherent statement , and the pedestrian was being treated by EMS.

I spoke with the witness who stated that she was driving her vehicle west on Rankin Street attempting to make a left turn south onto Anderson St. She noticed
the motorcycle behind her coming up at a high rate of speed and while she was making her left turn, heard a collision, and then parked her car on Anderson
Street after stopping. The witness and I inspected her vehicle but could not find any damage. The witness was unsure as to which side the motorcycle or it`s
rider went to. On the rear right side of the witness`s vehicle, I found a streak of transferred rubber that the witness did not remember being there before.

I then returned to Driver 1. I asked him to recount the events of the incident but he was unsure and seemed confused. I then asked him to tell me which
direction he was coming from and he pointed to the west toward the river. This was opposite of what the crowd and the witness communicated. Driver one
was then transported to Cape Fear hospital.

I then went to New Hanover regional medical center to speak with the pedestrian. The pedestrian stated that he had just left Ruth`s corner store and crossed
Anderson Street. He was then walking across Rankin Street heading north when he noticed the motorcycle "dip behind the car" and then collide with him. He
was not able to advise any details about what happened to the rider at that time.

I then responded to Cape Fear Hospital to speak with driver one again. Driver one stated that he turned left from 10th St. onto Rankin St. and when he
approached the intersection at Rankin and Anderson St. , the vehicle in front of him slowed promptly and failed to signal intent to turn. He then stated that he
attempted to stop and then avoided hitting the rear of the car in front of him by directing the motorcycle to the right of the vehicle. He then fell off the
motorcycle.

Driver one was cited for careless and reckless, NCGS 20-140(A).

Pedestrian 2 had a slight abrasion to her right knee. Pedestrian 2 refused photographs to be taken.

Investigation revealed Driver of Vehicle 1 operated the vehicle eratically and struck pedestrian 2. Pedestrian 2 advised Driver of Vehicle 1 did not intentionally
steer in her direction or change direction to intentionally strike her.

2017025783 August 19, 2017 10:58 pm 99 MARKET ST/N FRONT ST Pedestrian stated he was crossing south across Market St near Ibiza nightclub. Pedestrian began to cross the road and saw a vehicle coming towards him
heading east on Market St. Pedestrian thought the vehicle would stop as he crossed the road, but the vehicle struck him. Pedestrian was not able to give
description of vehicle that struck him.

Witness 1 was standing on the southern sidewalk on Market St. near Ibiza nightclub. Witness stated he saw vehicle 1 hit the pedestrian as he crossed the
road. Witness 1 stated it was an older beige colored car.

Witness 2 is a bouncer for Ibiza nightclub and was outside the front door and observed vehicle 1 hit pedestrian with the front right corner of the vehicle.
Witness stated the pedestrian went airborne and hit the windshield of the vehicle and landing on the pavement. Witness 2 described vehicle 1 as a beige late
model Toyota Camry.

Vehicle 1 fled the scene after colliding with pedestrian. Pedestrian walked from behind a parked vehicle to cross the road. The location where the pedestrian
crossed the roadway was not a designated crosswalk.
2017030560 September 26, 2017 3:54 pm 4933 WRIGHTSVILLE AVE/DIXIE AVE At 1554, 9/26/2017, I was dispatched to a traffic collision. Upon my arrival I spoke with the witnesses. Witness (Holder) and (Yarbrough) told me they had
been traveling behind driver-1 and had noted her erratic driving. The witnesses told me driver-1 would speed up and then slow down as though "she was
looking at houses." Witness (Powell) said he saw the collision. (Powell) told me bicyclist-2 was clearly traveling to the right side of the white fog line when he
was hit by driver-1. Driver-1 told me she was unsure how the collision occurred and later at the police department told me she had not been driving the
vehicle. Upon speaking with driver-1 it became clear that she was impaired by alcohol. At the police department driver-1 submitted a breath sample which
read .18ac. On 9/28/2017 I spoke by telephone with bicyclist-2. He told me that he was riding his bike in the "bike lane{ when he was struck by vehicle-1. I
noted there is no bike lane but rather just a fog line that was visible on the roadway. My investigation of this collision revealed that driver-1 was impaired by
alcohol and failed to drive the vehicle entirely within a single lane of travel. As a result the right side of vehicle-1 struck unit-2. At the scene I could see the
broken mirror from vehicle-1. The computer program dwould not allow me to enter the proper code of 28 for point of contact for the bicycle.
2017034676 November 1, 2017 2:30 am 5382 NEW CENTRE DR Unit one while backing, backed into pedestrian one who was walking through the PVA.

Driver one stated he was pulling out of a parking spot when he noticed Pedestrian one was walking towards his vehicle.

Pedestrian one stated driver one backed up without looking striking her in the knee with his vehicle.

Pedestrian one refused to be transported by EMS.

avoid the collision. Driver #2 stated that Lee was crossing the street against traffic and was not in the designated cross walk at the time of the collision.

2. Driver #2 struck Lee knocking her to the ground. Lee sustained minor bumps and bruising to her legs and was transported to NHRMC by EMS.

3. I then spoke to Taylor McEnaney who stated that she witnessed this incident. McEnaney stated similar details as driver #2 and that she witnessed Jenny
Lee crossing S 3Rd St from the east side of the roadway towards the west side of the roadway. McEnaney stated that Lee was crossing outside to the
designated crosswalk at the corner of S 3Rd and Market St and that Lee suddenly darted out in between vehicles that were traveling north bound on S 3Rd St
towards N 3Rd St. McEnaney stated that driver #2 was unable to slow/stop her vehicle in time to avoid the collision.

4. No other injuries or property damage was reported at this time.

2017034651 October 31, 2017 7:39 pm 699 ROBERT E LEE DR/BRAGG DR On 10/31/17 at 1939 hours I was dispatched to Robert E. Lee Dr. and Bragg Dr. regarding a single motorcycle traffic collision. All times, speeds and
measurements are approximate.

D-1, said he was traveling north on Robert E. Lee Dr. when a child in a Halloween costume darted across the street in front of him causing him to swerve
and dump his motorcycle onto the grass. He said that he did not hit the child and he did not have any description.

D-1, sustained minor injury to his arm and he was transported by EMS to Cape Fear Hospital.

V-1, sustained minor damage to the right side and was picked up by the registered owner.

Pedestrian #1, is unknown and fled scene. There were no witnesses.

2017035047 November 4, 2017 9:21 am 2499 OLEANDER DR/COUNTRY CLUB Vehicle #1 was traveling south in an alley and approaching Oleander Dr. Vehicle #2, a bicycle, was traveling west on the sidewalk of Oleander Dr. As vehicle
RD #1 approached Oleander the drivers view of the sidewalk was blocked by a large hedge and he stated he was pulling forward at only 2 or 3 mph when the
bicycle came across in front of him and struck the front left corner of the truck.
Driver #2 was thrown from the bike. He complained of pain but had no visible injuries and refused EMS treatment. Vehicle #1 suffered minor cosmetic
damage. No further information was available at this time.
2017037266 November 24, 2017 7:02 pm 200 DOGWOOD LN/OLEANDER DR The driver of V/1 (bicycle) advised he was going southbound on Dogwood Ln while texting on his cellphone when he collided into the front of V/2.

The driver of V/2 advised she was driving northbound on Dogwood Ln when the driver of V/1 suddenly entered her lane of travel and collided into the front of
her vehicle.

Investigation revealed that the driver of V/1 (bicycle) was distracted by an electronic device and was cited for said violation.

2017037743 November 29, 2017 5:54 pm 310 S COLLEGE RD/NEW CENTRE DR Driver 1 advised that she was driving in the PVA of 412 S. College Rd. coming from Bed Bath and Beyond towards Sams Club. Driver 1 advised she was
stopped at a stop sign at an intersection in the PVA. Driver 1 stated that after she stopped and made sure no one was coming, she then proceeded to go
straight through the stop sign passing Sams Club. Driver 1 advised that as she was driving straight the bicycle came out of no where and caused Driver 1 to
hit the bicycle. Driver 1 stated she has no idea where the bicycle came from.

I then spoke to the driver of the bicycle who advised that she was heading north in the PVA of 412. S. College Rd. She advised that she was beside Sams
Club and stopped at that stop sign facing towards Bed Bath and Beyond. The bicyclist advised that she stopped at the stop sign and waited for a couple cars
to go through the intersection. The bicyclist then advised that after she made sure the intersection was clear, she attempted to turn left to go towards Lowes
Home Improvement. The bicyclist stated that after she turned left, Driver 1 did a rolling stop and went through the stop sign without even looking and hit her on
her bicycle.

As of this time, investigation has not revealed who or what was the cause of the accident and I am unable to determine who is at fault.

2017037103 November 22, 2017 10:34 am 2799 CAROLINA BEACH Driver of vehicle one stated that he was leaving a parking lot and was making a left turn and did not see the pedestrian standing in the middle turning lane in
RD/HOLBROOKE AVE the middle of the road.
Driver one then stated that he tapped him with his front right quarter panel.
The pedestrian stated that he was standing in the road waiting for a break in traffic when he was hit by vehicle one.
Witness stated that the pedestrian was standing in the middle turning lane when driver one came out into the road making a left turn and struck the
pedestrian.
Investigation revealed that driver of vehicle one failed to see before starting.
2017040409 December 22, 2017 1:45 pm 3799 OLEANDER DR/FLORAL PKY Driver 1 advised that she was exiting the main parking area from the traffic aisle onto the main traffic way which runs in front of the shopping center itself. She
advised prior to doing so, she stopped and looked in both directions of travel and did not see anyone or any vehicles coming. She then began making a right
turn. She advised as she was making a right turn, she heard a noise and came to a stop when she realized the sideswiped the pedestrian. She advised she
stopped to check on the pedestrian and then moved her vehicle out of the traffic-way since there were other people assisting the pedestrian.

Pedestrian 1, Mrs. Linda Carroll, advised that she had just parked her vehicle in the handicap parking space. She advised she then got out of her car and
locked same. She then turned around to try to cross the traffic way. She advised she waited for a van to go by and then she began to cross the traffic-way.
She advised as she began to cross, vehicle 1 turned in front of her and knocked her down. She advised the side of vehicle one knocked her to the ground.
She advised driver 1 then stopped to check on her and stated to her " I`m sorry, I didn`t see you." She advised her right hip was the main area hurting on her
but she also hit her head as she fell to the ground.

Upon my arrival, vehicle 1 had already been moved into a parking space. There were a few people standing by the pedestrian, Mrs. Carroll. She was laying on
her back, with her feet at the edge of the marked crosswalk. She was laying at the edge of the traffic-way where the crosswalk also begins. She was wearing
a bright, blue in color, jacket with red sweater underneath. EMS arrived on scene. I spoke to the few people which were standing by her, but they all advised
they did not see the collision occur. The weather conditions was cloudy with misting style rain. I made contact with staff at Books-A-Million to look at their
video cameras. After looking at their camera that points out the front door where the incident occurred, it revealed that the incident was not caught on camera.
Due to there being no witnesses, the vehicle being moved prior to arrival, and the area the pedestrian was located upon arrival, I did not issue a citation.
Driver exchange form completed and give to both. Mrs. Carroll was transported to Cape Fear Hospital. I observed the right side view mirror, on vehicle 1,
folded slightly back and a small rub mark on the front passenger door. The rub mark was just a small area where the water / dirt was wiped off of same. There
did not appear to be any damage to vehicle 1. No further.

2017040176 December 20, 2017 5:19 pm 2499 CANTERWOOD DR/S 17TH ST Driver #2 stated that she was approaching the intersection from Canterwood. She stated that she was looking for traffic as she approached the interection;
however, she never saw the bicyclist prior to the impact with the right side of her car.

Passenger #1 (in vehicle #2) stated that as his mother was approaching the intersection, Driver #2, who was riding his bicycle north bound on the western side
of the street, approached the intersection at the same time his mother did. He stated he was not going really fast, but he did not stop and drove straight into
the intersection, colliding with his mothers car.

Driver #1 stated that he had the right of way as he approached the intersection, and proceeded into the intersection as driver #2 ran the stop sign and into his
path, causing him to collide with her vehicle.

My investigation revealed that Driver #2 did not stop behind the stop line on Canterwood Dr., nor did Driver #1 stop as he approached the intersection, in order
to verify that the intersection was clear, prior to entering it. Furthermore, Driver #1 was in all dark clothing, and it was dusk outside, as well as raining, There
was a mini light on the handlbar of the bicycle, but it is doubtful it would have been easily seen, if it was on at the time of the impact. It was not on at the time
of my arrival on scene. In checking the brakes on the bicycle, one of them did not work, and pushing the bicycle while squeezing the brake that did work, did
not produce enough friction to stop the bicycle. It felt like the brakes were wet and not offering any friction.

Both operators commited violations which led to the crash. No citations were issued to either party.

2018002447 January 24, 2018 2:05 pm 5034 MARKET ST/COBBLESTONE DR Driver of vehicle 1 left the scene without stopping.

Pedestrian 1 stated that she was crossing the street and when she was in the center turn lane vehicle 1 made a left turn onto Market St from Cobblestone Dr
and struck her, then left without stopping.

Pedestrian 1 refused EMS.


2018002616 January 25, 2018 7:30 pm 700 MEDICAL CENTER DR/CAROLINA Driver of vehicle 1 stated that he was turning left onto Medical Center Drive from Carolina Beach Road when he suddenly hit a pedestrian running across the
BEACH RD middle of the street. The driver stated that all he could see was the pedestrian`s leg right before the collision.

The pedestrian stated that he was at the Speedway gas station and saw someone he knew at the Bojangles across the street. The pedestrian ran in the
street to go make contact with his friend but was struck by vehicle one. The pedestrian said that he looked both ways before running across the street.

The witness stated that she saw the pedestrian run across the street and must not have seen the car turning onto Medical Center Drive.

The pedestrian was transported to the hospital.

2018002153 January 22, 2018 11:29 am 899 S CAROLINA BEACH Driver #2 stated he was in the Hardees PVA waiting to turn right onto Carolina Beach Rd. When the traffic was clear on his right Driver #2 started to pull out
AVE/SPARTANBURG AVE from the PVA and make his turn. At this time Pedestrian #1 was crossing the Driveway while Driver #2 was turning. Unit #2 collided with Unit #1.

Pedestrian #1 stated that he was riding his bike in the grass on the side of the road traveling south along Carolina Beach Rd. As he approched the Hardees
PVA he went straight across the driveway without stopping. Unit #2 collided with Unit #1.

Pedestrian #1 stated his bike did not have any breaks.

Driver #2 stated he did not see Pedestrian #1 at all.

Pedestrian #1 was transported to Cape Fear Hospital with a knee injury.

2018003633 February 3, 2018 11:09 am 1499 CHURCH ST/S 15TH ST The investigation revealed that vehicle 1 was starting forward from the stop sign going straight on S 15th St. Vehicle 2 a bicyclist was riding his bicycle east
on Church St. Vehicle 1 started forward and struck the bicyclist in the intersection.

Driver 1 stated that he stopped at the stop sign and looked both ways and then proceeded , he stated that he did not see the bicycle.

Driver 2 stated that he was riding his bicycle east on Church St when vehicle 1 ran the stop and struck his bicycle. He stated that he fell and hit the pavement.
Ems responded and checked Driver 2 for minor injuries
Driver 1 was cited for safe movement viol.
2018004695 February 13, 2018 2:56 pm I/O C W Stansbury conducted a follow up investigation and obtained probable cause to charge the drive of vehicle #1 with felony Hit and Run, Reckless
driving and Assault with a deadly weapon inflicting serious injury. Warrants for arrest were obtained and were served Monday March 5th 2018 by US
Marshall task force.
2018007683 March 12, 2018 6:35 pm 199 S 17TH ST/ORANGE ST At 0241, 3/12/2018, I was dispatched to a traffic collision. Upon my arrival I spoke with driver-2 who told me he was driving behind an unidentified vehicle and
both vehicles were in the far right travel lane. Driver-2 told me that he saw the unidentified vehicle swerve suddenly to the left lane. As that movement
happened driver-2 saw pedestrian-1 standing in the roadway. Driver-2 told me he had no chance to react and struck pedestrian-1. The driver of the
unidentified vehicle did not stop at the scene and may not have known the collision between pedestrian-1 and vehicle-2 had occurred. At the scene driver-2
submitted twice to an alcohol screening test which read .05ac. This result is below the state perse` level and I do not suspect impairment related to driver-2.
A short while before this collision, patrol officers had made contact with pedestrian-1 who had been pan handling neat=r the intersection of N 16th St and
Market St. The officers told me they believed pedestrian-1 had been highly intoxicated at the time of their encounter. My investigation revealed that
pedestrian-1 was intoxicated and standing in the street when he was struck by vehicle-2. Due to ongoing medical care I have not been able to speak with
pedestrian-1.
2018009703 April 1, 2018 9:11 am 1799 CAROLINA BEACH RD/W LAKE Vehicle 1 was starting from the stop sign turning right onto Carolina Beach Rd. Vehicle 2 a bicyclist was riding south on the side walk and was just crossing
SHORE DR the intersection of W. lake Shore and Carolina Beach Rd. Vehicle 1 then struck the bicyclist , the bicyclist then fell onto the roadway.
The bicyclist received minor injuries and complained of pain.

on Wooster St. Driver of Unit 2 stated when Wooster St traffic was stopped due to the red he began to ride across Wooster St. Driver of Unit 2 stated he
crossed between stopped vehicles. Driver of Unit 2 stated the far left lane had no vehicles in that lane. Driver of Unit 2 stated the far left lane was clear when
he attempted to cross on the bicycle. Driver of Unit 2 stated a white in color Ford F-150 or F-250 (Unit 1) collided with Unit 2. Driver of Unit 2 stated the impact
forced Driver 2 off Unit 2. Driver of Unit 2 stated Unit 1 made contact with Driver 2`s left side. Driver of Unit 2 stated he was thrown to the asphalt from the
impact. Driver of Unit 2 that Unit 1 did not drive over him but was able to come to a stop after the initial impact. Driver of Unit 2 stated Unit 1 stopped but
Driver 1 never exited vehicle before Unit 1 left scene prior to law enforcement arrival. Driver of Unit 2 stated Driver 1 asked if he was okay but he was catching
his breath and Unit 1 left before he answered.

Witness #1 stated he called 911 to report the motor vehicle collision. Witness #1 stated he was stopped in his SUV in the left straight lane on Wooster St at
the red light. Witness #1 stated there was just 1 or 2 unknown vehicles in front of him. Witness #1 stated Driver of Unit 2 (Bicyclist) crossed Wooster St
between vehicles in the middle of the roadway. Witness #1 stated Driver of Unit 2 crossed right in front of Witness #1`s vehicle going south across Wooster
St. Witness #1 stated the collision occurred to the lane to his left. Witness #1 stated a white in color Ford F-150 (Unit 1) driven by an older looking white male
slammed on brakes and collided with Unit 2 in the far left lane. Witness #1 stated that Driver 1 and Driver 2 spoke before Unit 1 left the scene heading west
over the Cape Fear Memorial Bridge.

Witness #2 stated the bicyclist (Unit 2) was weaving across the street and did not seem like the truck (Unit 1) saw him. Witness #2 stated Unit 1 initially
stopped and Driver 1 and Driver 2 exchanged words but then Unit 1 drove away. Witness #2 stated Unit 1 was a white Ford pickup truck that left heading west
over the bridge.

No statement obtained by Driver of Unit 1. Unit 1 failed to stay at the scene before law enforcement arrived.

Upon law enforcement arrival Unit 2 was out of the roadway on the southside sidewalk of Wooster St near the Wooster St entrance to Burger King. Driver of
Unit 2 was being checked by EMS.

Driver of Unit 2 was wearing a black do-rag on his head, no helmet was worn, dark in color coat with a light color undershirt, and had on blue denim jeans.

Roadway condition was wet from rain that passed through the area earlier in the morning.

EMS stated Driver of Unit 2 possibly has a broken right wrist.

EMS transported Driver of Unit 2 who had visible injuries to the right side of his body.

Based on statements obtained Driver of Unit 2 did not cross at a marked crosswalk. Driver of Unit 2 crossed in the middle of the roadway between vehicles.

2018010924 April 10, 2018 6:23 pm On 4-10-18, I, Officer Reyes, responded to listed location after Officer C. Morales advised over the radio of a crash between a vehicle and pedestrian.

EMS was called to the scene.

Upon my arrival, the pedestrian was by the entrance to the Chic-Fil-A restaurant and was sitting outside on a chair provided by a concerned citizen. The injury
appeared to be non life-threatening. Pedestrian complained of pain on his right leg and appeared to have some difficulty moving it. There was some dirt on his
pants from the collision.

Mr. Sloan stated that he had just walked out of the restaurant and was crossing the parking lot to go to his vehicle. He stated that Vehicle #1 came suddenly
and hit him on his right leg causing him to fall.

Driver stated that she was driving in the parking lot and when she was coming around a corner, she did not see him crossing.

Mr. Sloan was transported to the hospital.

Vehicle insurance and registration were both expired on the vehicle. The vehicle was not registered to the driver. Driver was given a citation for traffic violation.

2018013740 May 5, 2018 11:37 pm 2298 S 17TH ST/GLEN MEADE RD On 05-05-2018, I Officer Hayes doubled up with Cpl. Schwarz for training, responded to a call at 2131 South 17th Street. Call notes stated a pedestrian had
been struck in front of the hospital.

Upon arriving on the scene, I spoke with Driver 2 who advised she was driving down the street when she saw Pedestrian 1 on the sidewalk. Driver 2 stated
Pedestrian 1 looked at her vehicle and began to walk into the roadway. Driver 2 advised she attempted to stop her vehicle but did not have enough time as
she did not think Pedestrian 1 was going to walk out in the street. Driver 2 stated, Pedestrian 1 jumped on top of her hood and then rolled to the ground.

Shantelle Messick stated she saw Pedestrian 1 when he walked out in front of another car prior to the traffic crash occurring with Driver 2. Ms. Messick
advised, she saw Pedestrian 1 waiting for another vehicle to approach. Ms. Messick advised, as Driver 2 approached Pedestrian 1 stepped out in front of her
vehicle.

The investigation of the crash revealed Pedestrian 1 crossed into oncoming traffic without using a crosswalk. Pedestrian 1 was cited for this violation.

2017000062 January 1, 2017 8:29 pm 898 S COLLEGE RD/WILSHIRE BLV Driver of Vehicle 1 stated he was backing out of a parking space, heading North in the PVA of Harris Teeter on S. College Rd. and he was looking behind him
and he had seen Pedestrian going to the cart return and when he looked again, she was not behind him so he backed up. He stated he heard banging on his
car and moved forward and Pedestrian told him that he had run over her right foot.

Pedestrian stated she was walking back from returning her cart in the Harris Teeter PVA on S. College Rd when Veh 1 backed up and stopped with the right
rear tire on top of her foot. She stated it stopped there for a few seconds before he pulled forward.

Upon viewing the location of the accident, It appears odd how her right foot could of been run over as he was completely backed into the driving lane and she
was about ten feet from her vehicle. The witness stated that he yelled to her to look out and that is when she stopped and turned and that was when the
vehicle ran over her foot.

Pedestrian was transported to Cape Fear Hospital.

Driver of Vehicle 1 was cited with improper backing.

2017000351 January 4, 2017 7:16 pm 699 N KERR AVE/NEW CENTRE DR Driver of Veh 1 stated he was traveling north on N. Kerr Ave. when he observed the pedacyclist operating his bike in the middle turn lane and as he continued
to travel the pedacyclist attempted to merge to the shoulder in front of him forcing the front of Veh 1 to strike the pedacyclist.

The pedacyclist advised he was traveling north on N. Kerr Ave. and attempted to move over to the shoulder and was struck by Veh 1.

I/O observed damage to the front of Veh 1 and the bicycle.

The pedacyclist was intoxicated and had a strong odor of alcohol coming from his breath and person.

The pedacyclist was cited for a safe movement violation.

2017001272 January 13, 2017 2:49 pm 4499 MARKET ST/S KERR AVE The bicyclist advised he was about to cross the intersection at S. Kerr Ave. and Market St. He advised that as he started to cross, driver 2 was looking to his
left on S. Kerr Ave. and about to make a right turn onto Market St. He stated that he started crossing the intersection, and driver 2 started moving forward and
collided with him. He advised that his bicycle went under the vehicle and he was also under the car.

Driver 2 advised he was about to make a right turn onto Market St. from S. Kerr Ave. HE stated that he saw bicyclist and stopped for him to cross. He stated
that when the bicyclist started crossing the intersection, he turned hi bicycle into the front of his vehicle and fell over. Driver 2 advised he never moved his
vehicle. On the front of vehicle 2 were bicycle tire marks that looked to be coming in at an angle.

The witness advised that she was behind driver 2 and that driver 2 never moved his vehicle. She stated that the bicyclist crossed the intersection and turned
into driver 2 and fell off his bicycle. She advised she does not know driver 2 or the bicyclist.

Per EMS 22 who transported the bicyclist to Cape Fear Hospital, stated that while they were in route, he made statements that this was going to be a big pay
day and that driver 2 would pay out.

The bicycle at the scene had no damage on it and there were no visible injuries to the bicyclist. Fire personal that responded did several test on him while he
was laying on the ground, and they also advised that there were no injuries at the scene.

Because of the witness statement and other factors, the bicyclist was cited for failing to see before starting.

The bicycle was left in the care of two females who knew the bicyclist. The two females advised they did not see the crash, but saw him after the crash and
wanted to check on him.

No further info.
2017001404 January 14, 2017 6:05 pm 399 N 3RD ST/WALNUT ST 1) Unit one (PEDESTRIAN) stated that he was on the southeast corner of the intersection at N 3rd and Walnut St. There was a vehicle traveling south on N
3rd and made a right turn on Walnut St. When the vehicle turned, he did not see the vehicle behind it and "booked it across the street" to make it to the
southwest corner of the intersection on his skateboard while there was a "no walk" symbol for his travel direction. The vehicle was not able to slow down
enough in time to avoid the collision. The impact caused damage to the vehicle`s left headlight, light damage to the hood and the hood`s windscreen. The
pedestrian sustained minor injuries.

2) Driver two stated that he was traveling southbound on N 3rd St. When he came up to the intersection of Walnut St, the vehicle ahead of him made a right
turn. Driver two then accelerated when the vehicle ahead completed their turn. Driver two did not see the pedestrian and collided into him causing minor
damage and minor injuries to the pedestrian. Driver 2 had a green light for his travel direction.

3) My investigation concluded that the collision was caused by the pedestrian`s decision to cross traffic while there was a "no crossing" signal. The pedestrian
was cited for "pedestrian crossing traffic".

4) EMS responded to the scene to transport the pedestrian to Cape Fear Hospital to treat for minor injuries.

2017001820 January 18, 2017 11:40 pm 115 HINTON AVE/OLEANDER DR Driver #1 stated that he did not remember the accident at all or his operation of the vehicle leading to the crash.

Pedestrian #1 stated that she had left the gas station and was walking down Hinton Ave when she felt this tremendous pressure in the back of her head. She
heard the loudest sound she has ever heard and she flew into the air and landed on the ground. She looked around and saw the moped driver laying there
unconscious and she thought he was dead. She began to call for help when the Witness and her friend came to her aid and called 911.

Witness #1 stated that she saw Pedestrian #1 walking down Hinton Ave near the side of the road when Driver #1 struck here from behind causing her to fly up
into the air and land on the asphalt. Driver #1 was unconscious for a few minutes as they called 911.

Driver #1 was cited for DWI as well as Failure to reduce speed. It should be noted that Pedestrian #1 was wearing dark clothing, walking on a dark road and
in the roadway which would make her difficult to see at this time of day.

2017001098 January 12, 2017 1:25 am 498 S COLLEGE RD/RANDALL PKY I, Officer J. Knott, responded a traffic accident involving a pedestrian at S. College Rd and Randall Pky on 01/12/2017 at 0116 hours.

Upon arrival Officer Raper was gathering information from the witness, Ashley Graham, whom stated that vehicle 1, a gold sedan, had a green light and was
turning left to go south onto S. College Rd. The witness stated she did not see the crash she only saw the light cycle at the time of the crash. Ms. Graham
stated the driver of vehicle 1 left the scene of the crash. Ms. Graham did not have a description of the driver of vehicle 1. She described it as a gold 4 door
sedan. Direction of travel was south on S. College Rd.

The pedestrian, John Mcbryde, was extremely intoxicated. Mr. Mcbryde stated he was riding a skateboard across the crosswalk at S. College Dr. on Randall
Pky. There was no skateboard on scene.

Vehicle 1 was not on scene.

Mr. Mcbryde suffered minor injuries and was transported by EMS.

No further at this time.


2017001982 January 20, 2017 2:09 am 2198 SHIPYARD BLV/ASHTON DR VEHICLE 1 WAS TRAVELING WEST ON SHIPYARD BLVD. VEHICLE 2 A BICYCLE WAS TRAVELING WEST ON SHIPYARD IN THE LEFT LANE.
VEHICLE 1 FAILED TO REDUCE SPEED AND COLLIDED WITH VEHICLE 2. THE RIDER OF THE BICYCLE WAS KNOCKED OFF THE BIKE AND
LANDED IN THE RIGHT LANE. VEHICLE 1 CONTINUED WEST DRAGGING THE BICYCLE. THE BICYCLE CAME TO REST IN THE LEFT LANE ,
VEHICLE 1 LEFT THE SCENE AND CONTINUED TRAVELING WEST.
THE VICTIM WAS TRANSPORTED TO THE NEW HANOVER MEDICAL CENTER FOR TREATMENT BUT LATER DIED FROM INJURIES FROM THE
CRASH.
OFFICER MOORE TOOK MEASUREMENTS AND LOCATED CAR PARTS FROM A MERCEDES BENZ. THE PARTS APPEARED TO BE FROM THE
FRONT BUMPER. CSI TOOK PHOTOS, COLLECTED THE VEHICLE PARTS AND THE VICTIM`S CLOTHING.
A PATROL OFFICER WAS SEARCHING THE AREA FOR THE SUSPECT VEHICLE WHEN HE LOCATED A BLACK MERCEDES BENZ AT THE COBBLE
STONE APARTMENTS AT 2700 S 17TH ST. THE VEHICLE HAD EXTENSIVE RIGHT FRONT DAMAGE, THE LOWER BUMPER WAS BROKEN AND
MISSING A SECTION. THE PART OF THE BUMPER THAT WAS COLLECTED FROM THE SCENE MATCHED THE ABANDON VEHICLES FRONT
BUMPER.

2017003535 February 2, 2017 10:26 pm 5114 WRIGHTSVILLE AVE/ROSE AVE At 2107, 2/2/17 I was dispatched to a traffic collision. At the hospital I spoke with pedestrian-2 who appeared to be intoxicated. He told me he was walking
from the Scotchman Store #182 which is located at 5204 Wrightsville Ave to a friends residence located near the intersection of Rose Ave and Wrightsville
Ave. He told me that as he was crossing the street he was struck by a black Honda sedan that was traveling West on Wrightsville Ave without illuminated
headlights. Vehicle-1 continued Westbound without stopping. Pedestrian-2 continued to his destination and later was driven by friends to Cape Fear Hospital
for treatment.

2017003411 February 1, 2017 9:03 pm 4799 COLLEGE ACRES DR/RACINE DR Pedestrian stated she was crossing over College Acres Dr. heading North on Racine Dr., when she saw Vehicle 1 come to what she thought was a stop, and
then realized it was more of a rolling stop. She saw the driver look both ways, but did not see her and as she realized the vehicle was going to hit her, she tried
to pedal faster and get out of the way,but was unable and was struck by the vehicle. She stated she fell off the bike and that she hit the right side of her face
on the concrete. Driver had a small scrape on the right side of her forehead and the right side of her face was swollen. The wire basket on the front of her bike
was bent from the impact. She stated the driver got out and stated "i`m sorry, I`m sorry" and then she asked the driver her name, which she gave and then the
driver was getting back into her vehicle and she took a picture of the license plate as the driver was leaving the scene.

The witness I spoke too stated the same, he stated he was coming South on Racine Dr. and he saw pedestrian crossing over College Acres and then saw
Vehicle 1 appear to be at a stop, but then accelerate forward and strike the pedestrian.

Upon locating the driver of Vehicle 1, she stated that she stopped to make sure pedestrian was ok, and that she thought she was fine, and she appeared to be
walking toward the sidewalk and maybe leaving. Driver stated she gave her name when asked and that she saw pedestrian taking a picture of her license
plate.

Driver of Vehicle 1 was cited with failure to reduce speed and hit and run failure to leave information/give assistance.

2017026455 August 24, 2017 9:06 pm 199 N 11TH ST/CHESTNUT ST Driver 2, a sworn law enforcement officer, was traveling West on Market St when she observed Witness 1 chasing Pedestrian 1 from the Family Dollar at
1013 Market St. Driver 2 activated her emergency equipment and gave verbal commands to Pedestrian 1 to stop but he continued North on N 11th St. Driver
1 eventually pulled her vehicle over in front of Pedestrian 1 in the 200 block N 11th St but Pedestrian 1 kicked the front left quarter of her vehicle and
continued on.
Witness 1, a manager at the Family Dollar, advised that Pedestrian 1 took items from the store without paying and he went to confront him. Pedestrian 1
assaulted Witness 1 and took off North on N 11th St. Witness 1 advised that Driver 2 pulled up, turned on her lights and shout at the male to stop but he
continued North on N 11th St. Witness 1 stated that Driver 2 pulled her vehicle in front of Pedestrian 1 in an attempt to get him to stop but he kicked her car
and kept running. Witness 1 advised that Driver 2 did not hit Pedestrian 1 but pulled in front of him to stop him from running.
Pedestrian 1 was uncooperative but advised that he was struck by Vehicle 2.

2017026593 August 25, 2017 2:28 pm 1499 S 8TH ST/GREENFIELD ST Driver of vehicle #1 advised he stopped for the stop sign and saw the pedestrian standing on the corner. He looked to his left and didn`t look back forward
and hit the pedestrian.
Pedestrian advised she was standing on the corner and saw that vehicle #1 was stopped so she crossed in front of his vehicle when she was hit.
I/O`s investigation revealed driver of vehicle #1 failed to see prior to starting that such a movement could be made in safety and collided with the
pedestrian.

2017026535 August 25, 2017 9:58 am 499 WOOSTER ST/S 5TH AVE Investigation revealed pedestrian #1 did walk across a street when such traffic light indicated a red light for that direction of travel.

Driver #2 stated she was traveling west on Wooster St approaching the intersection at S 5th Ave. As she approached the intersection driver #2 stated she
had a green light in her direction of travel. Despite the green light she noticed two vehicles had stopped in the roadway in the lanes to her right. However due
to the green light she began to proceed into the intersection when pedestrian #1 stepped in front of her vehicle. Driver #2 stated she attempted to swerve to
avoid colliding with pedestrian #1 but was unable to do so before striking him. Driver #2 stated she was traveling at approximately 25 mph at the time of the
collision.

I then spoke with witness #1 who stated he was traveling west on Wooster St in the far right lane. As witness #1 traveled on Wooster St he advised he
observed the light was green in their direction of travel. Witness #1 stated he recalled the vehicles in the middle two lanes appeared to have been stopped in
the roadway as vehicle #2 and he entered the intersection. As he continued into the intersection he observed pedestrian #1 fly into the air before landing in
the roadway. Witness #1 stated he never saw the male until the collision occurred.

The investigation completed at the scene indicated the impact was caused by driver #2 striking pedestrian #1 with the right front corner of her vehicle. The
force of the impact caused pedestrian #1 to also strike vehicle #2`s windshield. Pedestrian #1 and vehicle #2 then traveled approximately 25 ft prior to coming
to rest.

I responded to the hospital to attempt to speak with pedestrian #1. Upon arrival I began speaking with him however he stated he didn`t remember the
collision.

Ofc. Genda took photos of the collision scene.

No further.
2017028539 September 10, 2017 3:10 pm 1395 MILITARY CUTOFF Driver #1 was traveling north in the travel lane next to Haverty`s Furniture. She stated that she was turning into the parking lot area and did not see the
RD/EASTWOOD RD bicycle that was traveling south in the opposite lane. She turned left and collided with the cyclist.

Driver #2 stated that he was riding his bicycle south in the PVA approaching Driver #1. As he was getting ready to pass her, she suddenly turned in front of
him causing him to collide with the front of her vehicle.

Driver #1 was cited for a safe movement violation.

2017029690 September 20, 2017 7:38 am 1539 S 12TH ST/CROCUS CT On 9-20-17, I, Officer Reyes, responded to listed area in reference a 2 vehicle crash.

Driver 1 stated that she was moving her vehicle from a parked position. As she was backing up, she collided with an unoccupied parked vehicle behind her.

Ms. Cobbs stated that as she was crossing in between vehicles, which belongs to her uncle, vehicle 1 made contact with her leg before it hit her parked
vehicle. No injury was reported and EMS was not requested.

2017028001 September 5, 2017 8:53 pm 4710 OLEANDER DR/PARKWAY DR Driver 2 stated that he was driving his bike on oleander towards pinegrove crossing over a PVA driveway when Driver 1 collided with driver 2.
Driver 1 stated that he was stopped at the PVA driveway waiting to turn onto oleander when driver 2 collided into driver 1 and landed in roadway.
EMS responded Driver 2 had minor injuries and refused transport.

2017031121 September 30, 2017 7:28 pm 5299 MARKET ST/LENNON DR On 09/30/2017, I officer Wilson responded to the intersection of Market Street/Lennon Drive in reference to a pedestrian being struck by a motor vehicle.
When I arrived EMS was rendering aid to the subject in the middle of the road. I identified that subject as Mr. Tony Oxendine.

I responded to the hospital to speak with Mr. Oxendine. Mr. Oxendine stated that he was trying to cross the road at Market/Lennon to go to the Murphy gas
station. Mr. Oxendine suffered a laceration to the back of his head, an abrasion on his forehead, right buttox, left elbow, and his left eye was swollen.

Driver # 1 stated that she was driving home going 40 mph and the man walked out in front of her car. She slammed on the brakes but struck him and he hit
her windshield.

A witness by the name of Eden Simpson stated that she had just pulled into Jason`s Deli when she saw a pedestrian try and cross the road. At first she
thought a tire blew out because of how loud the noise was. The subject was unresponsive for the first 5-7 minutes. He was in the median before she heard the
noise.

After speaking with Mr. Oxendine and the driver my conclusion is that Mr. Oxendine attempted to illegally cross the roadway at Market/Lennon and was struck
by vehicle # 2 in the process.

2017036437 November 16, 2017 5:49 am 1 N KERR AVE/MARKET ST Operator one of his bicycle stated that he was traveling East on Market Street crossing North Kerr Avenue when he was struck by the driver of vehicle two.

Driver two stated that she was stopped at the intersection about to conduct a right hand turn West onto Market Street when she stated a black male on a
bicycle came out into traffic as she had a green light hitting and scratching her bumper.

Investigation found that the operator of the bicycle to be at fault due to damage to driver two`s vehicle. The pedestrian did not abide to the flashing traffic
controls in the intersection traveling in the roadway. The pedestrian was cited for operating a bicycle on a roadway permitted for vehicular traffic traveling on
the same side of the roadway.

The pedestrian on the bicycle signed off and refused treatment from EMS.

Both parties were provided OCAs.

2017035854 November 10, 2017 12:32 am 698 S COLLEGE RD/HOGGARD DR Pedestrian Cameron Thomas Stratton stated that he and his roommate were crossing west from UNCW to the other side of Hoggard Drive when he was
struck by vehicle two. He and his roommate Cedric Boyd stated that they attempted to cross the street with only three seconds left on the street crossing sign.
Boyd was on a bicycle and Stratton was on a skate board. Boyd passed through the intersection first approximately two to three seconds before his
roommate. When Stratton followed he was struck

Driver two stated that she was traveling in the right side center lane traveling south through the intersection when she saw a man from a short distance on a
bike pass in front of her. She stated that she did not see anyone else. Driver two stated that a man on a skate board came out from behind another vehicle
that was attempting to make a left hand turn into UNCW heading in the same direction. The pedestrian came in front of her vehicle and was struck. The
pedestrian went onto the hood hitting the windshield damaging it. The left front head light was damaged as well.

Stratton stated to Officer Poff and I that he should not have just cut out into the roadway like he had done. Stratton was seen by EMS and refused to be
transported for treatment. Stratton had no significant injuries and walked away from the scene.

Stratton was found to be at fault by his own admission.

2017036523 November 16, 2017 10:46 pm 199 HANOVER ST/N 2ND ST Investigation revealed that the traffic collision occurred on Hanover St. at the stop sign near N 3rd St.

The driver of vehicle 1 stated she approached the stop sign on Hanover St/ N 3rd St., and made a complete stop. She stated she looked left then proceeded
forward through the stop sign. The driver stated when she accelerated she collided into the pedestrian that was crossing the street North bound on her Blue
Ragal Hoveround.

The pedestrian stated she was traveling across the street when the driver of vehicle 1 struck her.

The pedestrian did not thrown from her hoveround. However she was transported to the hospital for observation by EMS. The driver of vehicle 1 was cited for
failing to yield right of way to a
pedestrian.

2017036290 November 14, 2017 11:06 pm 99 N 3RD ST/PRINCESS ST Driver #1 advised he was driving North on N. 3rd St. and was about to cross Princess St. when he heard a sound. He crossed princess St. and noticed his
driver side mirror had been struck. He stopped the vehicle and observed Mr. Jeffrey Stoltz crossing the road. He advised that he did not see Mr. Stoltz
crossing the road.

Mr. Stoltz complained of no injury and advised he was crossing the street at the intersection of Princess St. and N. 3rd St. when he was struck by the driver
side mirror of vehicle #1. He did not have any injuries. Mr. Stoltz did not want EMS. The damage to the vehicle was minor.

2017036676 November 18, 2017 9:29 pm 1 N FRONT ST/MARKET ST Driver #1 advised he was driving south on N front St. and his window fogged up on the inside. As he attempted to wipe off the fog he struck the pedestrian at
the cross walk.

Pedestrian #1 advised he was crossing the street at the cross walk between Market St. and Princess St. and was struck by vehicle #1. There was no visible
injuries on pedestrian #1. EMS was refused by the pedestrian.

Pedestrian #1 commented "His insurance is going to pay." The investigating officer checked the vehicle for damage but there was no new damage. Dew from
the cool weather was on the vehicle but there were no spots wiped off from the impact. The only areas the dew was missing; was where the pedestrian laid
against the car while waiting for EMS.

The pedestrian was able to walk away and the driver was able to drive home.

2017039118 December 11, 2017 2:34 pm 3819 CAROLINA BEACH Driver #1 stated that she was decelerating in the right hand turn lane, preparing to turn right (east) onto Independence Blvd. As she approached the
RD/INDEPENDENCE BLV intersection, she stated that she had a green arrow. She looked to her left, but the sun was in such a position that it was blinding coming from that direction.
She did not see the bicyclist until just before she collided with the left side of her vehicle. She jammed on her brakes and stopped, kept her foot on the brake
and stayed inside of her vehicle until other bystanders were able to verify that the operator of the bicycle was not pinned underneath, nor would she further
injure the cyclist.

Driver #2 stated that she had waited at the crosswalk for the green light. She stated that once the traffic light turned green then she was able to cross. She
also stated that she watched the crosswalk sign as well as the traffic light. Once she was crossing in the crosswalk, she said the driver didn`t see me, and I
didn`t see here until it was too late. I tried to brake and swerve left but collided with the vehicle.

Driver #2`s daughter (Jessica Pearce), who arrived on scene after the crash had been riding with her mother several minutes prior to the crash. She herself
had crossed ahead of her mother against the crosswalk sign to get to the teller machine at the nearby shopping center. Her mother wanted to wait for the
proper crossing signal prior to crossing.

My personal observation of the traffic light / crosswalk light is that it was working normally, and there are no witnesses to corroberate either parties story. The
sun was in a position to cause a glare that could have hindered Driver #1 from clearly seeing Driver #2; however, even though the light could have changed in
mid-crossing, clearly Driver #2 had possession of the crosswalk, and had made it across 5 lanes of traffic prior to colliding with Driver #1.

Driver #1 was cited for failure to Yield to a Pedestrian in a Crosswalk

2017038126 December 2, 2017 5:25 pm 2299 CAROLINA BEACH On 12/02/2017 around 1725 hours I, Officer R A Roberts responded to the area of Carolina Beach Rd and Southern Ave in response to a vehicle vs.
RD/BORDEAUX AVE pedestrian traffic crash.

The Pedestrian was crossing Carolina Beach Road on the inside, north bound travel lane when he was struck by Vehicle #1. Traffic was heavy at the time,
and the victim was subsequently, and unavoidably run over by the other listed vehicles before traffic was able to stop. The victim was not in a designated
crosswalk at the time of the impact, and was wearing darker clothing during the dusk time frame.

2017039494 December 14, 2017 4:49 pm 1399 WOOSTER ST/S 14TH ST PEDESTRIAN 1 stated she was walking across Wooster St at S 14th St. PEDESTRIAN 1 stated she was crossing the road with a younger family member,
CHRISTANNA T. MOORE DOB 03/17/2012, who was not struck by the vehicle. PEDESTRIAN 1 stated she was walking south on S 14th St at Wooster St.
PEDESTRIAN 1 stated two lanes of traffic had slowed to allow her and the young child to cross. PEDESTRIAN 1 stated a male in a vehicle (Witness) had
stopped in traffic and waved them on to cross. PEDESTRIAN 1 she did not see Unit 2 and only had enough time to push the young child out of the way to
protect the child. PEDESTRIAN 1 stated that she was struck by Unit 1 in the far left lane and quickly got back up on to her feet to check on the child who fell to
the ground when pushed out of harms way. PEDESTRIAN 1 stated Unit 2 came to a stop and did not run over PEDESTRIAN 1.

Driver of Unit 2 stated he was heading west on Wooster St in the far left lane. Driver of Unit 2 stated he was going about 35 mph. Driver of Unit 2 stated his
lane of travel was clear of any traffic or pedestrians. Driver of Unit 2 stated the other lanes were moving slow. Driver of Unit 2 stated that PEDESTRIAN 1 and
young child stepped out from in front of a vehicle in the middle lane into the far left lane. Driver of Unit 2 stated he attempted to brake to avoid a collision.
Driver of Unit 2 stated he made contact with PEDESTRIAN 1 at an unknown speed. Driver of Unit 2 stated he stopped right away to check with PEDESTRIAN
1.

Witness stated he was stopped in the middle lane on Wooster St. Witness stated he had waved the pedestrians to cross. Witness stated he did not see any
vehicles in left lane. Witness stopped to check on pedestrian.

I observed no visible damage to Unit 2.

At S 14th St and Wooster St is not a traffic light intersection with no marked crosswalk. Wooster St has the right of way and S 14th St has stop signs for north
and south traffic.

At time of the motor vehicle collision it was dusk. Call for service was 1649hrs and sunset was reported at 1703hrs.

PEDESTRIAN 1 and young child were transported by EMS.

At the hospital where I obtained a statement from PEDESTRIAN 1, I observed PEDESTRIAN 1 wearing a brown in color shower cap, brown/tan in color
jacket, black in color undershirt, camo pattern green and brown pants, and red in color shoes. I observed the younger child who was crossing with
PEDESTRIAN 1 was wearing a black in color coat, red in color undershirt, red in color leggings, and black in color boots.

I did not observed any visible injuries to PEDESTRIAN 1 and/or CHRISTANNA MOORE.

Based on statements obtained by third party witness and parties involved I did not issue a citation.
2018000966 January 12, 2018 8:24 pm 2198 S 17TH ST/MEDICAL CENTER DR Investigation revealed Mr. Pearson was crossing S 17th St from the area of NHRMC toward the CVS Pharmacy in the marked cross walk when he was struck
by a vehicle traveling south on S 17th St.
Mr. Pearson was carrying several large bags when he was attempting to cross S 17th St. Mr. Pearson advised he looked both ways but that Vehicle 1 "came
out of nowhere." Looking north on S 17th St from Medical Center Dr it is a clear straight shot for over 1000 feet, there is no reason he should not have seen
Vehicle 1 approaching. Mr. Pearson admitted he should not have been in the roadway at the time of the collision.
Driver 1 stated that she was traveling south on S 17th St in the outside lane when she observed a subject crossing in front of her. Driver 1 stated that she had
a green light and attempted to slow down before colliding with the pedestrian. Driver 1 was noticeably shaken up by the situation.
Vehicle 1 sustained little visible damage, most of which appeared to affect the passenger side headlight which was dislodged from its mounting brackets,
otherwise no significant damage was observed on the front or hood of Vehicle 1.
Mr. Pearson was transported by EMS to NHRMC where I spoke to him after leaving the scene. Mr. Pearson was noticeably intoxicated and admitted to
consuming alcohol prior to the collision and submitted to a portable breath test which tested positive for alcohol. Mr. Pearson complained of right knee and
elbow pain, he had not been seen by a doctor or x-rayed before my arrival.
Driver 1 was cited for No Operator License due to a medical cancelation, Mr. Pearson was cited for being in the roadway despite the crosswalk sign indicating
not to cross, no further.

2018004500 February 11, 2018 11:11 pm 1199 S COLLEGE RD/WRIGHTSVILLE On 02/11/18 at 2311 hours I was dispatched to S.College Rd. and Wrightsville Ave. regarding a vehicle vs. pedestrian traffic collision. All times, speeds and
AVE measurements are approximate.

Pedestrian-1, was transported to NHRMC prior to my arrival. I went to her location for a statement. She said that she was crossing the street and got hit.

Pedestrian-1, sustained injuries to her head,face,pelvis, left arm and abrasions.

D-2, said that he was traveling north on S.College Rd. at 35 MPH and had a green light at Wrightsville Ave. when Ped #1 walked out in front of him and they
collided.

D-2, did not sustain any visible injury and did not complain of pain.

V-2, sustained damage to the front end and windshield.

Witness#1, said he was traveling behind D-2, and observed Ped#1 walk out in front of D-2. He said that the light was green at the time of the incident.

This intersection does not have a crosswalk. Witness #1 written statement was placed into evidence at headquarters.

2018003710 February 3, 2018 9:34 am 99 RACINE DR/OLD EASTWOOD RD At 2204, 2/3/2018, I was dispatched to a traffic collision. Prior to my arrival driver-2 had fled the crash scene and pedestrian-1 had been transported to the
hospital. I spoke with witness-1 who told me he had witnessed the entire event. Witness-1 said that pedestrian-1 had been standing in front of vehicle-2
when vehicle-2 started forward and drove over pedestrian-1 and then fled the scene without stopping to provide information or aid. Witness-2 told me he was
present but deferred to witness-1. Witness-2 told me that witness-1 had "seen more of the event." At the hospital I attempted to speak briefly with
pedestrian-1 who no longer wanted to answer my questions and asked me to leave. CSI documented the injuries incurred by pedestrian-1 and the crash
scene. At 1011, 2/4/2018, I was dispatched to the crash scene to meet with driver-2 and both passengers who occupied vehicle-2 at the time of the collision.
Driver-2 and both passengers followed me to the police department where CSI photographed vehicle-2 and the passengers and driver-2 provided written and
video statements concerning the incident. Driver-2 said that as he and his passengers were in the vehicle they were being confronted by pedestrian-1,
witness-1 and witness-2. Driver-2 said that witness-1 was "hitting the passenger side windows with his hands and pedestrian-1 was on the driver side. During
this witness-2 arrived in his vehicle. Driver-2 said he fled the area but was unaware that he had struck pedestrian-1 until seeing a local news report. Both
passengers gave essentially the same account of the event. On 2/5/2018, I spoke with pedestrian-1 at the hospital. He told me that he approached vehicle-2
and its occupants in the pva because he believed that vehicle-2 was actually the vehicle owned by witness-1 and then asked for "a ride home." When driver-2
and the vehicle occupants refused to provide a ride, there was a small verbal exchange as pedestrian-1 walked away. Pedestrian-1 said the he and witness-1
had not confronted the occupants of vehicle-2 but did attempt to retrieve property that pedestrian-1 believed he had left in vehicle-2. Witness-1 and
witness-2 later gave later gave written and video statements concerning the incident. Witness-2 said that he thought vehicle-2 had driven over the curb island
as it left the scene. My investigation revealed that driver-2 failed to see before starting forward that the movement could be made safely. Pedestrian-1 failed
to step away from the vehicle to safety and was standing in a location so close to the vehicle as to be unreasonable as the vehicle attempted to leave the pva.
Driver-2 said he believed that he and the vehicle occupants were being confronted by a number of people and chose to leave rather than be involved in a
physical confrontation. All occupants of vehicle-2 denied knowledge of striking pedestrian-1 as they left the pva. All person(s) involved in this event had been
consuming alcoholic beverages prior to the event. No tests for alcohol or drugs were given or made available to Law Enforcement at the time of this report.

2018004269 February 9, 2018 6:18 pm 598 S 3RD ST/CASTLE ST 1) On Friday February 9th, 2018 at 1818 hours I, Ofc. J.D. Hutchins, responded to the intersection of S. 3rd and Castle streets in reference to a vehicle vs.
pedestrian crash,

2) After investigation it was determined that Pedestrian 1, while under the influence of an impairing substance, entered the roadway to cross same while the
"Don`t Walk" signal was emitting from the crosswalk signal. Driver 2 stated that Pedestrian 1 was wearing dark clothing and he did not have enough time to
stop or avoid him. Witness 1 stated that Pedestrian 1 entered the intersection while Southbound traffic on S. 3rd St. had a solid green light. Witness 2 gave
the same testimony. Driver 2 was not cited due to eyewitness testimony.

3) Pedestrian 1 was taken to NHRMC where his injuries were reported as non-life threatening. Pedestrian 1 was cited for walking against oncoming traffic
(G.S. 20-172).

2018009674 March 30, 2018 1:14 pm 5351 MARKET ST/GINGERWOOD DR On 03/30/2018, I, Officer Griffith responded to 5301 Market St. in reference to a vehicle versus pedestrian. Upon arrival, I was met by Driver 1 in this incident.

Driver 1 advised he was in the process of making a right turn out othe parking lot of Jason`s Deli onto Market St. Driver 1 advised there was a pedestrian
crossing sidewalk to sidewalk of Market St. at the parking lot entrance/exit. Driver 1 advised he bumped into the pedestrian, got out of the vehicle and ask him
if he was ok. Driver 1 advised that the pedestrian said he was ok, and information was exchanged. The pedestrian walked away before LEO arrival.

I made contact with the pedestrian by phone. The Pedestrian advised he was crossing sidewalk to sidewalk, when Driver 1 drove into him. Driver 1 advised
he would seek his own medical attention.

Driver 1 was cited for failure to yield to pedestrian.

2018012424 April 24, 2018 9:04 am 1699 WOOSTER ST/S 17TH ST On above listed date while on uniform patrol of district 3 in vehicle 17025 I, Officer W. Rose responded to the New Hanover Medical Center in reference to a
past occurred hit and run. When I arrived on scene I met with Monique Buie who had been transported by EMS to the hospital after she had called about
being hit by a car.

I spoke with EMS Unit 33 who advised that they transported Ms. Buie to the hospital from 909 N Lincoln Ct. EMS unit stated that the patient was alert, and
could not state what specific part of her body was injured.

I then spoke with Ms. Buie who stated that she was walking North bound on S 17th St. Ms. Buie advised that she was attempting to walk across the street at
the crosswalk and had the walk symbol. Ms. Buie stated that she was halfway across the crosswalk when traffic began to come toward her on Wooster St.
Ms. Buie stated that she attempted to get all the way across but a gray in color sedan hit her. Ms. Buie stated that the impact caused her to go unconcious
and into shock. Ms Buie then stated that the cashier from Family Dollar was on scene and asked if she was ok, but Ms. Buie did not have any information on
the Family Dollar store clerk. Ms. Buie then advised she walked to 909 N Lincoln Ct. to call 911.

While speaking with Ms. Buie I attempted to get further information on the accident. Ms. Buie would only state to me that her whole body hurt due to getting
hit on the right side of her body. Ms. Buie could not remember which part of the car hit her, or where at on her body. Ms. Buie did not have any visible injuries
and stated this was due to her clothing. Ms. Buie stated that she just wanted to report the incident. I provided her with a case number, and cleared from the
hospital.

I made contact with the STING center and attempted to locate video footage of the incident. I was advised that the traffic camera at S 17th St. and Wooster
St. did capture the incident at 1356 hours. I obtained a copy of the video footage that was copied, and is entered into Wilmington Police Department
Evidence.

The video footage shows the traffic signal for the West bound lanes emitting a steadily red light, and no pedestrian in the crosswalk. The video footage then
shows the traffic light turn green, and the traffic in all three lanes begins to move. Once the traffic light turns green you see the pedestrian begin to cross in
front of the lanes of traffic which are now moving. The pedestrian on camera starts off in the crosswalk but then begins to walk out of the crosswalk in a
diagonal direction toward the entrance of the Family Dollar at 722 S 17th St. As the pedestrian is exiting the right lane toward the sidewalk: a dark in color
vehicle stops in the travel lane. The video footage then shows the pedestrian get off the ground from infront of the vehicle, and continue walking toward
Family Dollar. The dark vehicle remains stopped in the lane as the pedestrian walks away, but then leaves due to the pedestrian walking away from the
scene. Due to the angle of the camera, I am unable to determine that the vehicle actually struck the victim as she was crossing the road. Due to a lack of
evidence, and lack of visible injury I am unable to determine if the victim was actually struck by vehicle #1.

Ms. Buie was unable to provide any driver description or additional information.

I spoke to driver of vehicle 1. Driver of vehicle 1 advised she was stopped at the light at 10th and Market St. when she got the green light. Driver of vehicle 1
advised she proceeded to go westbound on Market St. and collided into the juvenile riding his bicycle. Driver of vehicle 1 advised she did not see the juvenile
crossing the street.

I spoke to the juvenile. The juvenile advised he was riding his bicycle northbound on 10th St when he came to the intersection at 10th and Market St. Juvenile
advised he proceeded to crossing the intersection at 10th St. when driver of vehicle collided into him.

I spoke to a William Rozier, a witness of the collision. Mr. Rozier advised the juvenile was crossing the intersection at 10th and Market St. when driver of
vehicle 1 got the green light to go westbound and collided into the juvenile.

The juvenile was taken to New Hanover Regional Medical Center by EMS due to having a broken arm and having bruises on the left side of his head.

Driver of vehicle 1 was cited for a safe movement violation.

2018016863 June 2, 2018 11:20 am 5626 OLEANDER DR/DOGWOOD LN Driver #2 advised she was traveling east on Oleander Dr at approximately 35 mph in a 45 mph zone. Driver #2 stated while driving straight ahead she
observed a pedestrian in dark colored clothing walk in front of her vehicle just prior to her reaching this location. Driver #2 stated she "slammed" on her
brakes and swerved to the right in an unsuccessful attempt to avoid a collision with Pedestrian #1. My investigation reveled the front left bumper, hood and
windshield collided with Pedestrian #1.

I spoke with the passengers in the vehicle who advised they were traveling east on Oleander Dr in the right lane of travel. They advised they did not see
Pedestrian #1 in the street until Driver #1 screamed. Driver #1 then attempted to avoid the collision but was unable to do so.

Pedestrian #1 was transported to New Hanover Regional Medical Center for treatment of his severe injuries. Due to his injuries I was unable to get a
statement from Pedestrian #1.

2017011740 April 19, 2017 1:34 am 299 PRINCESS ST/N 3RD ST Pedestrian stated he was hit by a black Ford sedan in the intersection of Princess St. and N. 3rd St. Pedestrian stated he was walking west cross N. 3rd St.
Pedestrian stated vehicle 1 was traveling east in the left turn lane on Princess St. and making a left turn to N. 3rd St. Pedestrian stated they both had green
light. Pedestrian stated the right front corner of vehicle 1 struck him on his left hip and left knee. Pedestrian stated vehicle 1 pulled to the east side of N. 3rd
St. for a second and then took off.

Witness stated he just parked his vehicle and was standing on east side pedestrian walk of N. 3rd St. when crash occurred. Witness stated Pedestrian was
walking west cross N. 3rd St. and vehicle 1 was traveling east in the left turn lane on Princess St. and making a left turn to N. 3rd St. Witness stated the right
front corner of vehicle 1 collided with Pedestrian. Witness provided the description of driver 1 and vehicle 1. Witness stated the tag was a regular NC tag and
started with letters "WVS".

No injury was reported on scene.

spoke to the driver of the vehicle Eva Bay Bertone. Ms. Bertone stated she was traveling east on Oleander drive in the left turn lane. Ms. Bertone stated she
needed to make a U-turn to head back to her apartment on Randall Parkway. Ms. Bertone stated as she was slowing down for the traffic light at Oleander Dr
and Pine Grove Dr a white male walked out in front of her vehicle. Ms. Bertone stated the white male was walking north across Oleander Dr in between
vehicles stopped for the traffic light.
Ms. Bertone stated she was unable to stop in time to avoid hitting the white male.

The white male subject is Jack Powell 03/11/2001. Mr. Powell stated he was walking north crossing Oleander Dr. Mr. Powell stated he was enroute to
his mother`s house at 4903 Pine St. Mr. Powell stated he did not look left when crossing the turning lane. Mr. Powell sustained minor injuries to his arm
where he hit and shattered the windshield of the vehicle. Mr. Powell was taken immediately to urgent care on Oleander Dr. Mr. Powell was bandaged and
sent to Cape Fear Memorial Hospital for medical treatment.

Mr. Powell`s father, Jason Powell, arrived and provided insurance information. Jason Powell stated to Ms. Bertone to get an estimate for the vehicle and
Jason Powell would pay for the damage out of pocket.

2017011664 April 19, 2017 2:35 am 2 S COLLEGE RD/N COLLEGE RD On 04/19/2017 I, Officer Garay responded to a Traffic Crash involving a pedestrian.

Upon arrival I witnessed Matthew Cooper lying on the ground, he said that he was skateboarding on S. College road when a truck struck him. While EMS and
Fire were working on Matthew, I spoke to the driver of the truck. The driver, Marcos Anayas stated that he was driving down S. College road going
approximatley 45 mph when he heard a loud bang and realized he had hit someone.

Investigation finds that the manager of the Comfort Inn hotel, Christopher Husbands saw Matthew leaving the hotel, and watched him attempt to run across S.
College road. Matthew was crossing the road in a non lit area, and he was also not crossing in a crosswalk. He was wearing all black, with a black bookbag
on and was not able to be easily seen. The driver of the truck was not impaired, and appeared to be in a normal state of mind. The driver was not cited for the
incident due to it being the pedestrians fault for being in the roadway. Matthew was also far enough in the roadway that he was interfering with oncoming
traffic. Marcos struck Matthew with the right front corner panel of his car causing his headlight to bust.

Matthew was complaining about pain in his calf, and stomach.

Matthew was transported to New Hanover Regional Medical Center for his injuries.

After following up With Matthew at the Hospital, he stated to me that the last thing he remembered was leaving the Comfort Inn Hotel.

OCA was given to the driver, and to Matthew.

2017014502 May 15, 2017 12:45 pm 1199 EMORY ST/STEWART CIR Pedestrian #1 stated he did not remember anything after doing narcotics (K2 SPICE). Pedestrian #1 was transported to New Hanover Regional Medical
center to be treated for non life threatening injuries.

Witness #1 stated Pedestrian #1 was impaired by narcotics, and ran toward Vehicle #2 which was in motion going north bound on the 1000 block of Emory St.
Witnesss #1 stated Pedestrian #1 jumped on top of the vehicle, and then fell off. Witness #1 stated Pedestrian #1 then got up, and ran toward her. Witness #1
stated she ran inside her residence because she was afraid.
2017013636 May 7, 2017 5:08 pm 1939 FENWICK PL/BREWTON CT Driver 1 stated that she was backing out of her driveway when she stopped for a vehicle pulling into the street. The driver of the on coming vehicle advised
Driver 1 that she had backed into the pedestrian that was laying on the ground behind her vehicle. Driver 1 stated that she did not see him behind her vehicle,
nor did she feel it when she hit him. Driver 1 told me that she wasn`t going more than 1 or 2 mph.

I spoke to the pedestrian and he told me he was not sure about what happened.

Investigation reveals Driver 1 failed to see before backing.


2017016355 June 1, 2017 5:27 pm 3700 PATRIOT WAY/S 17TH ST The driver of unit 1 said he was coming up to South 17th ST from the side road next to the bank and looked both ways and did not see anything then started
to move when unit 2 was suddenly in front of him and he had to react quickly to stop. He said he knocked the bike rider off his bike.

The driver of unit 2 said he was riding on the city side walk parallel to South 17th St and had crossed over the side road and saw unit 1 coming up the side
street toward him but thought that the truck would stop due to seeing him and kept going straight ahead. He said the truck hit him and knock him down. The
driver of unit 2 said he was checked out by EMS before my arrival but did not go with them. He said he was ok but his left leg was slightly injured during the
fall from the bike.

Witness 1 said she was behind unit 1 and it did not look like the driver of unit 1 saw unit 2 crossing in front of him and unit 1 stuck unit 2 as unit 2 crossed the
side street from the side walk. The witness was not able to recall if the driver of unit 2 was using the sidewalk or 17th St to travel.

Investigation showed that the driver of unit 2 was riding a bike on a city sidewalk and had failed to stop and yield to unit 1 before crossing over the street to get
to the other side of the side walk. The driver of unit 1 did not see unit 1 in time to keep from hitting him but tired to stop at the last second. Investigation
showed that the driver of unit 2 was injured from being knocked off his bike by unit 1. I observed a small amount of blood on the left leg of the driver of unit
and he was walking with a slight limp.

I did not observe any damage to unit 1 or unit 2.

The driver of unit 2 was advised that it is unlawful to ride a bicycle on a city side walk. That he had failed to yield to unit 1 and that he was lucky he did not get
seriously hurt.

2017019279 June 27, 2017 5:40 am 199 DARLINGTON AVE/METTING RD 1) On 06/27/2017 at/around 2234 hours I, Officer Lynch was dispatched to New Hanover Medical Regional Medical Center in reference to a Hit and Run.

2) Upon my arrival, I spoke with the Albert Ellis who advised that he did not want to give any information on what had happened. I observed that Ellis had an
abrasion on the back side of his head, and that it was swelling to form a large lump. With Ellis not wanting to give any information I provided him a card with
my name and number in case he wanted to file a report later.

3) On my way out of the hospital I was met by Ellis`s family. The family provided a phone number of a Willie Williams. Williams stated that she saw Ellis and
Breandra Clay arguing outside in a parking lot at Market North Apartments. The two continued to arguing before entering into a residence. Williams then
observed Ellis and Clay still arguing, exit the residence and head to a vehicle. At the vehicle Clay preceded to put a child (Jamari Clay Ellis) into the child seat
in the back. Ellis was still yelling at Clay from the front of the vehicle as Clay got into the drivers seat. Williams then stated that Clay pressed the gas pedal
causing the vehicle to lurch forward hitting Ellis. Ellis was on the hood of the vehicle for a few seconds before falling off.

4) After I finished talking to Williams, Clay arrival at the hospital. I spoke with Clay who advised that she and Ellis have had an off and on relationship for about
2 years and that they have a child together (Jamari). Clay stated that she was getting ready to leave her house at 209 Darlington Ave when Ellis approached
her. Ellis was trying to take Clay`s car key`s like he had done the previous day. (Clay stated that she had called 911 on 06/26/2017 because Ellis had taken
the car keys and prevented her from going to work. The police were unable to locate the keys, but Clay advised that a report had been done.) When Clay
would not give Ellis the keys, he stood in front of Clay`s vehicle. Clay proceed to put Jamari in his car seat and then entered the drivers seat. At that point Clay
stated that Ellis jumped on the hood of the car and started hitting the windshield. Clay stated that she moved the car a little to try and get him off the hood. Ellis
got off the vehicle, but then jumped back on the hood. At that point Ellis fell from the hood of the car onto the ground. Ellis then preceded to get up from the
pavement and walk away. Clay advised that she thought Ellis was okay, which is why she drove off.

5) I arrested Clay for Felony Hit and Run and Assault by deadly weapon with serious injury. I transported Clay to police headquarters to be fingerprinted. Clay
was then taken to the New Hanover Jail and was released to their custody with no bond due.

6) ID was called out to the Hospital to take pictures of Ellis`s injuries.

7) DSS was contacted and advised on the situation involving the child.

8) I went back to the Hospital and observed the vehicle (EKE6605) sitting in the Hospital parking lot. I did not observe any damage to the hood or bumper.
There was a crack in the front windshield, but it did not appear to be from damage caused by Ellis`s fist.

2017019476 June 29, 2017 11:52 am 3999 MASONBORO LOOP The Pedestrian Stated that she was trying to get their dog that had gotten loose from their residence at the corner of Masonboro Loop Rd and Shoreline Dr.
RD/SHOREWOOD HILLS DR The dog ran across Masonboro Loop Rd and was close to the road. She said she looked both ways and did not see any traffic. She stated that she made it
to the other side of the road and feels like she "tripped (possibly over her feet), fell back and hit her head on the grass." She said she was not struck by the
SUV and she is not claiming any injuries. She was evaluated by EMS and released at the scene, refusing treatment and refusing transport.

Witness #2 (the pedestrians brother) advised that he did not see his sister look either way prior to running across the street from the east side of the road.
She was focused on the dog and he said he kept telling her to stop but she didn`t. He stated that he saw the SUV impact his sister and that he felt like that
impact was very close to the point in time where the SUV came to a complete stop. He did not hear an impact, but just saw her fall off to the side. He ran to
check on her and she kept saying "I`m okay, I`m okay". He stated the driver of the SUV and the driver of the vehicle behind the SUV came to her aid and
waited for emergency personnel.

Witness #1 stated that he was on break and sitting in a parked work truck for the city of Wilmington on Shorewood Dr, facing the crash location. He stated
that that he saw the dog when it got out of the house and the kids began to chase it. The dog ran across Masonboro Loop Rd and stopped in the ditch across
from Shorewood Dr. He said the boy and his sister went to the edge of the road and she looked left, but never looked right before running out into the street
to reach the dog on the other side. He stated that the driver appeared to have slowed down to around 30 mph and that when the girl ran out into the street, he
jammed on his brakes and was almost stopped when he impacted her with the right front right corner of the SUV, knocking her about 5 feet onto the side of
the road.

Driver #1 stated that he was traveling about 45mph with the cruise control, traveling south on Masonboro Loop Rd. He saw a dog that had run across the
street and two smaller children that had run up to the edge of the left side of the road watching the dog. He had hit the brake and had slowed down to what he
described as "coasting", keeping an eye on the dog and believing that the kids were waiting on him to pass so they could cross and get the dog. He did not
think that they would come into his lane of traffic. As he got closer, the young girl moved into the street and without looking, darted across the road, causing
him to jam on his brakes. He came to an abrupt stop, feeling like the impact with the young lady happened at the same moment his truck came to a complete
stop, and "bumping" her, causing her to roll off the front corner of the vehicle. He got out of his truck and went to the aid of the young lady who kept wanting
to get up, but he insisted she stay on the ground. She asked him to "please don`t tell my mom".

Vehicle #1 had very minimal damage, appearing that the trim between the parking light and the headlight had a light crack in it. The right front parking light
was intact but dislodged and loose sitting in place. This was the only apparent damage on the vehicle.

No charges or infractions were issued for this event.

2017019473 June 29, 2017 10:37 am 299 GRACE ST/N 3RD ST Driver # 1 stated he was traveling east on Grace St. when he approached the light at N 3rd St. which was red. Driver # 1 stated he came to a stop and
proceeded to make a right turn on red after he had looked both ways on N 3rd St. to check for vehicles coming in either direction. Driver # 1 stated when he
did not see vehicle coming in either direction he proceeded with pulling up to the corner to make a right turn on red when he collided with vehicle # 2.

Driver of vehicle # 2 stated he was traveling north on N 3rd St when he came to the corner of N 3rd St. and Grace St. Driver # 2 stated the crosswalk light was
red and he stopped his bicycle prior to the crosswalk. Driver # 2 stated after he had waited approx. 45 seconds his light turned to allow him to cross and as he
started forward, he was struck by vehicle # 1 which threw him off his bicycle and onto the ground.

2017020205 July 5, 2017 6:18 pm 298 S 3RD ST/ANN ST 1) On Wednesday July 5th, 2017 at 1833 hours I, Ofc. J.D. Hutchins, while working the front desk of WPD`s NW Patrol Headquarters, had a female walk in to
report a hit and run. FELIPA REANO (02.05.1978) advised that an unknown make and model white panel van had struck her as she crossed S. 3rd St.
heading Westbound from Ann St.

2) Ms. Reano advised that she was walking across S. 3rd St. on Ann. Traffic in the left-hand lane had come to a stop to let her pass, and she entered the
roadway to cross. As she continued to walk to the right-hand lane, a white panel van with a roof rack on top failed to yield to her and struck her on her right
side, causing her to fall to the ground. After striking her, the van continued North on S. 3rd until it made a right hand turn unto Market street heading
Eastbound. Ms. Reano advised that EMS arrived and transported her to the hospital for treatment. I asked Ms. Reano why she never had NHC Central
Communications dispatch a WPD officer. Same advised that she thought that they would have been notified, but no officer came.

3) I gave Ms. Reano information regarding this report and watched her safely leave WPD. Due to the location of the accident, as well as the Ms. Reano`s
statement that the vehicle turned right unto Market St., I went and spoke with Cpl. G. WIllett and Cpl. J. Nichols in the WPD`s STING center. After a lengthy
search of the STING centers camera placements in the area of the accident, it was revealed that no footage of either the vehicle or the incident was captured
on any camera.

4) Due to Ms. Reano`s lack of descriptive information, and absence of eyewitness testimony or camera footage, I am closing this case CLOSED/LEADS
EXHAUSTED

2017016989 June 7, 2017 1:45 pm 4499 WRIGHTSVILLE AVE/S KERR AVE On 6/7/17 at 1345 hrs. I was dispatched to the 4500 blk. of Wrightsville Ave. in reference to a report of a vehicle vs bicycle collision.

Cyclist #2 stated that he was crossing the 4500 blk. of Wrightsville Ave. in the middle of the block when he was struck by vehicle #1. I observed abrasions on
the cyclist`s right arm. Wilmington FD got out with the cyclist who stated that he was okay and that he did not need EMS. WFD cancelled EMS at the cyclist`s
request.

Driver #1 stated that she was traveling east in the 4500 blk. of Wrightsville Ave. when cyclist #2 entered her travel lane and caused a collision between their
vehicles. She stated that she stopped her vehicle and rendered necessary aid to the cyclist.

pedestrian.

Upon arrival, the three year old, Jayden Brunsen, was in his grandmother`s arms. He was not crying and did not seem to be in any pain. I asked him if he
was hurt anywhere. Brunsen touched his head. He did not have any signs of physical injury.

The Driver, Helena King, was present upon my arrival with a concerned look on her face. She stated that she was just driving through the front of the parking
lot normally when Brunsen suddenly ran out in front of her. She then hit Brunsen and immediately stopped the vehicle. King stated that she never saw him
coming.

Witness 1, Jerlean White, is the grandmother to Brunsen. She stated that King was not driving fast at all. She advised that Brunsen would be ok but she still
wanted him to go to the hospital just to make sure.

I observed the security footage from outside the front doors to the food lion. I observed a large black truck parked in the fire lane obscuring King`s view of the
doors. King proceeded at a normal rate of speed across the front of the parking lot when I observed Brunsen sprint out from behind the black truck parked in
the fire lane and into the parking lot in front of King`s vehicle.

Brunsen was transported to New Hanover Regional Medical Center in reference to his head hurting.

I turned a copy of the security footage into property and evidence.

No other injuries were reported.


2017028205 September 7, 2017 1:22 pm 99 N FRONT ST/PRINCESS ST Driver 1 stated that he was headed north on N Front St when he conducted a U-turn in the intersection of N Front St and Princess St and struck a pedestrian
in the crosswalk. Driver 1 stated that he never saw the pedestrian.

Pedestrian stated that he was crossing Princess St when he was struck by vehicle 1. Pedestrian declined EMS but stated that he was injured. Pedestrian
stated that he would be heading to the doctor as soon as he got back home to Myrtle Beach.
2017030602 September 26, 2017 10:43 pm 699 WOOSTER ST/S 7TH ST Driver 1 advised that she and her male friend, Chris, were leaving her co-worker`s apartment in Mill Creek and going to Leland to drop off Chris. She stated
that she was driving West on Wooster when she saw what she thought was a box or debris in the roadway. She slowed down but was unable to get in the
other lane because a car was there. She ran over what she thought was debris but as soon as she hit it, she pulled over and Chris looked under the car and
said it was a man.While talking with Lily, I noticed no odor of alcohol or any other signs of impairment. I asked Lily if she would consent to a breath test, she
did and was negative.
The passenger advised that they were leaving Lily`s co-worker`s apartment in Mill Creek and were on their way to take him home in Leland. He stated that
they were traveling on Wooster and that he had his hand on her thigh and was looking at her. When he looked back he saw what he thought was a box in the
roadway. Lily hit it but she said "I think that was a person" and pulled over. Both Lily and Robert filled out witness statement forms, which were turned into
records.
Witness 1, who lives at 610 Wooster St directly in front of where the collision occurred, advised that she was on her porch. She heard the pedestrian yelling
and cursing, she observed him lying naked in the roadway and was on the phone with 911 and going to the roadway when the truck struck Robbins.
Witness 2 stated she heard a lot of yelling and saw a man lying in the street with his pants down. She advised that he wasn`t moving when the truck ran
over him.
Witness 3 was driving down Wooster to his residence at 610 Wooster when he saw a naked man standing in the road cursing. He said he got out of his
truck and was calling 911 when the male laid down in the road and was hit by the blue truck.
Witness 4 was traveling West on Wooster when he passed a man lying in the middle of the road with his pants around his ankles. He turned around to
check on the male but by the time he got back to the area it was too late, the male had been struck by the truck.
Each of the witnesses wrote a statement and they were turned into records, along with Driver 1 and passenger`s statement.

as he got into the turning lane and started speeding up to proceed down the lane, the bicyclist pulled out in front of him. He stated that he tried to slow down
and he turned left to avoid colliding with him, but was not able to and they collided. He advised that the bicyclist flew off the bicycle and hit his windshield.

The bicyclist was transported to New Hanover Hospital prior to my arrival.

Once the scene was clear, I responded to the hospital and spoke with the bicyclist who stated he does not remember what took place. All he knew was he
was returning to work from making a delivery for Jimmy John`s.

The bicyclist was in room 79 and his condition was fair. A Doctor had not been assigned to him at the time I was there.

Driver 1 was cited with fictitious tags and the bicyclist was cited for failing to yield the right of way.

Both witnesses`s advised the same thing. They stated that they were in the far left straight lane, westbound. They advised that the bicyclist drove his bicycle in
front of them and did not stop for the turning lane traffic and driver 2 collided with him.

2017030928 September 29, 2017 12:46 pm 4106 SHIPYARD BLV/PRIVATE On 9/29/2017 around 10:00am I, Officer Moore responded to Hoggard High School reference to a traffic crash.

Pedestrian 2 stated that she was walking along side of the PVA connecting the Senior Center parking lot with Hoggard High School parking lot when Vehicle 1
struck her from behind.

Driver 1 stated that there were 4-5 teenagers blocking the street. She pulled up behind them and 2 moved out of her way. She stated that she had to drive off
the road to get around the pedestrians without hitting them when Pedestrian 1 hit Driver 1s side view mirror with her hand.

Driver 1 stated that she backed up and the group of teenagers began yelling a cursing and throwing rocks at Driver 1. She stated that she was worried that
things would escalate and left the area.

No citations were issued due to conflicting stories.

2017037350 November 25, 2017 12:22 pm 399 S FRONT ST/NUN ST I Corporal R. J. Evans, while on routine patrol, responded to the area of South Front and Nun Streets in reference to a traffic accident involving a motor
vehicle and a bicycle.

Upon arrival I noticed a white male laying on the ground to the side of the road. He was surrounded by a number of citizens in the area. The male had
obvious injuries to his body; to include, bleeding from the head area and a very large bump on his head. The male was later identified as Mr. Bruce Androphy.
Mr. Androphy stated he did not remember what happened. He advised that he was riding his bicycle and the next thing he knew, he was laying on the ground
with a group of people standing around him. Mr. Androphy was quickly transported to New Hanover Regional Medical Center (NHRMC) by Emergency
Medical Services (EMS) personnel.

I then spoke to the driver of the vehicle that struck Mr. Androphy, Ms. Parker. Ms. Parker stated that she was driving on South Front Street and she did not
see the man on the bicycle.

I then spoke to a witness in the area, Ms. Moore. Ms. Moore stated that she first noticed Mr. Androphy because it was an unusual bicycle he was riding. She
stated that Ms. Androphy was on the sidewalk at first and then rode his bicycle into the street; he was then struck by a vehicle.

Mr. Androphy was not wearing a helmet or any other type of protective gear. He was also not in a crosswalk when he went into the road.

After leaving the accident scene, I visited Mr. Androphy at NHRMC to received more information. He was subsequently cited for a safe movement violation.

2017036145 November 13, 2017 2:39 pm 299 N 3RD ST/GRACE ST D1 stated he was making a left turn on to N. 3rd st from Grace st. D1 stated that he did not see the pedestrian walking on the crosswalk until it was too late.
D1`s vehicle struck the pedestrian. D1 had no injuries.

Ped stated he was walking across the intersection when the vehicle hit him. Ped was taken to the hospital by EMS. Information was exchanged between
parties. D1 was cited for failure to yield to pedestrian traffic. The collision was very low speed.

2017030546 September 26, 2017 7:10 am On 09/26/2017 I, Ofc Sanchez, was dispatched to a hit and run involving a car versus a bicycle at the intersection of Oleander Dr and Independence Blvd.
Upon arrival, I spoke with the victim, Ms Jennifer Vique.

Ms Vique stated that she was waiting at the intersection to cross Oleander Dr. When the light changed, she began to cross the intersection when a silver
sedan accelerated to try to turn in front of her. Ms Vique stated that she was unable to avoid a collision and collided into the front passenger side of the vehicle
and fell to the pavement. She then stated that the vehicle continued driving with out stopping. She then called 911.

A witness who saw the accident followed the vehicle and called 911. Ofc Mitchell was able to locate the vehicle on the 1900 Blk of Dawson St. Ofc Mitchell
interviewed the driver, Ms Laura Garnica. Ms Garnica told Ofc Mitchell that she was making a turn from Independence Blvd onto Oleander Dr when she heard
an impact of the side of her car. She looked through her rearview mirror and saw that a bicyclist was getting up off the ground. She stated that she was scared
and did not know what to do so she kept driving. She also stated that she had no cell phone to call for help.

EMS transported Ms Vique to Cape Fear Memorial Hospital.

I then drove to the 1900 Blk of Dawson St to interview Ms Garnica. While Interviewing Ms Garnica I observed that there was damage to the front passenger
side of her vehicle and that her passenger side mirror was broken and damaged.

Based on the statements obtained and the damage observed on the vehicle I cited Ms Garnica with failure to yield and hit and run.

2017040159 December 20, 2017 2:43 pm 1 LULLWATER DR/MARKET ST I, Officer J. Knott, responded to a traffic crash at Lullwater Dr. and Market St. on 12/20/2017 at 1443 hours.

Upon arrival I spoke to driver 1, Kristin Somers, whom stated she was operating vehicle 1, NC tag TEBOWFAN, on Lullwater Dr making a right turn onto
Market St. She stated her light turned green and she started her right turn when vehicle 2, 26" yellow Schwinn Varsity bicycle, didn`t stop and came off of the
sidewalk and attempted to cross the crosswalk while riding his vehicle and the two collided.

I spoke to driver 2, Tami`r Smith, whom stated he was riding vehicle 2 west bound on Market St. trying to get home. He stated he crossed the crosswalk and
vehicle 1 collided with him. He was unsure if his light was green or if he had a go or stop signal on the crosswalk.

Upon my investigation I found driver 2 failed to yield the right of way. No citations were issued.

Vehicle 1 suffered no damage. Vehicle 2 suffered no apparent damage.

Driver 2 complained of right wrist pain and was seen by New Hanover EMS.

No further at this time.


2018000455 January 8, 2018 9:51 pm On 01-08-2018, victim/pedestrian 1 advised that Driver of Vehicle 1 intentionally drove her vehicle up onto the grassy area in front of 1602 East Plaza and
struck her, pinning her against the building.

There was a video which surfaced a few hours after the incident on facebook. After investigation and video evidence, it was determined that Driver of Vehicle
1 did intentionally operate her vehicle in an erratic manner and strike victim/pedestrian 1 intentionally, and then fled the scene.

began to make her turn she felt a collision and saw glasses flying up in front of her. She immediatly backed up and saw that she had hit someone riding a
bicycle. She also stated that there was a large truck parked to her right in the far lane by the building.

Witness 1 advised that he saw Vehicle 1 stop at the stop sign and the proceed to make a turn. He stated that he saw Pedalcyclist 2 making a left as Vehicle 1
was turning. He advised that Vehicle 1 then collided with Pedalcyclist 2 but did not run over him. He stated that it did run over the bicycle.

Pedalcyclist 2 advised that he was heading to work in the pva and that he made a left turn to head north when Vehicle 1 hit him. He stated that he was run
over in the process. He also advised that he had consumed four alcoholic beverages prior to leaving for work.

Investigation revealed that Vehicle 1 failed to see before making a turn colliding with Pedalcyclist 2.

Driver 1 was cited for Failure to See Before Turning.

2018006368 February 27, 2018 3:28 pm 5100 RANDALL DR/S COLLEGE RD On 02/28/2018 at 1353 hrs, I Ofc Broadstreet responded to Bank of America (502 S College Rd) in reference to a past occurred hit and run. Upon arrival I
spoke with Erika Olsson 12/13/1992 (victim).

She stated that on 02/28/2018 as she was skateboarding across the cross walk from Randall Dr to Randall Pkwy that she was hit by a vehicle at
approximately 1930 hrs. She stated that she had just left UNCW Campus and was headed back home. She stated that a vehicle was traveling West on
Randall Dr and was attempting to make a left hand turn onto S College Rd. She stated that as she was skateboarding that she planted her right foot on to the
ground to stop herself from moving forward and then her right foot was hit by a white four door passenger sedan. She stated that she then fell to the ground
landing on her elbow. She stated that the vehicle pulled into the Bank of America PVA and stopped for a second and then pulled back out of the PVA taking a
right hand turn onto S college Rd. She stated that the model of the vehicle that hit her is typically driven by an elderly person but she was unable to give a
make and model or any license plate information.

2018005977 February 24, 2018 4:43 pm 4018 LAKE AVE/41ST ST Pedestrian reported hours after the incident took place while at the hospital. She was being checked for a bruise (hematoma) to her right hip.

Pedestrian explained she was walking eastbound on the side of the road on Lake Avenue toward S. 41st Street. While walking she was struck by the side
mirror of red Honda driven by a female. The female driver stopped and check her well being. She told the female driver she was ok before the driver left. She
had a piece of paper with the name (Johnton Arthur) 910-386-6996, 4010 Lake Ave. It was explained he was a neighbor that came outside shortly after the
accident and checked to see if she was ok. He wrote down the drivers registration plate # WWF-4056 which was also on the piece of paper.

I later met the female driver at her residence. She explained she had struck a female who was walking down the road on Lake Avenue while she was traveling
to Whole Foods earlier. She did not see the female walking for all the bushes on the side of the road. She stopped and checked on the female, who told her
she was ok before she left. She rode with me to point out the location on Lake Avenue where the accident occurred. I did observe the large vegetation
(bushes) on the shoulder of the road.

2018005873 February 23, 2018 9:08 pm At 1527, 2/23/2018, I was dispatched to a traffic collision. Upon my arrival I spoke with driver-2, the witness and the passengers. Witness (Caulder) and
pedestrian-1 (Wilson) had been passengers in vehicle-2 just prior to the collision. Pedestrian-1 had been asked by the driver to exit the cab due to his unruly
behavior inside the taxicab. Witness (Caulder) also left the vehicle but did so voluntarily. The remaining passengers were Wilson`s mother and girlfriend. As
vehicle-2 drove around the Dunkin Doughnuts building in the drive through lane, pedestrian-1 ran at the cab and purposefully threw himself on the front of the
vehicle, damaging the vehicles windshield. Driver-2 said that as he drove forward, pedestrian-1 threw himself on the vehicle. At the hospital pedestrian-1
told me he was trying to get the vehicle to stop so he could retrieve items he had left and driver-2 simply refused to stop and drove into him. I later spoke by
telephone with passenger (Morgan) who told me that driver-2 may have been subject to an impairing substance at the time of the collision. I saw no signs of
impairment by driver-2 during my interaction with him and no one at the crash scene mentioned that concern. My investigation revealed that pedestrian-1
failed to yield right of way to vehicle-2 and purposefully damaged the windshield of vehicle-2.
2018008397 March 18, 2018 12:17 pm 329 MILITARY CUTOFF Driver #1 stated she was exiting Landfall on Arboretum Dr. Driver #1 stated she was on Arboretum Dr in the right lane at Military Cutoff Rd. Driver #1 stated
RD/ARBORETUM DR she was making a right turn on Military Cutoff Rd. Driver #1 stated she had a red light. Driver #1 stated driver #2 was riding a bicycle on the pedestrian cross
walk going south. Driver #1 stated when she began to turn right the front of vehicle #1 collided into the left side of the bicycle or vehicle #2.

Driver #2 stated she was traveling south on the pedestrian path at Military Cutoff Rd and Arboretum Dr. Driver #2 stated she had the right of way and the
cross walk was indicating to cross. Driver #2 stated vehicle #1 was turning right and the front of vehicle #1 collided into the right side of her bicycle.

Witness #1 stated that vehicle #1 had a red light. Witness #1 stated that the impact from the crash was very minor.

The investigation revealed vehicle #1 was on Arboretum Dr and Military Cutoff Rd at the traffic light. Vehicle #2 (bicycle) was traveling on the pedestrian
crosswalk south. Driver #1 had a red light and vehicle #2 had the right of way to cross. The front of vehicle #1 collided into the left side of vehicle #2.

2018008638 March 20, 2018 4:06 pm 199 S 17TH ST/ORANGE ST Driver #1 stated that he was traveling north on S 17th St. A white SUV pulled out in front of him. He switched lanes to avoid hitting the SUV. He said he
probably accelerated a little bit, but he knew that he was going like 35 mph, at the most going around 38. The light was green, and then a bicyclist just went
across the street. He tried as hard as he could to avoid hitting him. He tried to stop, but he just couldn`t.

Pedacyclist stated that he could not remember any events from the day. The earliest thing he could remember was getting off work the day before. He
suffered extensive injuries including a neck fracture, 35 staples in his head, multiple ribs with hairline fractures, back and shoulder injuries.

Witness #1 (witness #2 on the incident report) stated that he turned onto S 17th St from the PVA of the corner store. At the time he turned out, Driver #1 was
a block away and had just moved into the left lane as Witness #2 was pulling out. He described Driver #1 as "Hauling Ass". He estimated Driver #1 speed to
be in excess of 45 mph. He said Driver #1 shifted lanes back to the right lane. Driver #1 came around him, saw the dude on the bicycle. The impact
occurred around the middle of the two lanes, but was sure that Driver #1 was braking and changing lanes to avoid the Cyclist. Immediately following the
collision, Witness #1 stated that he went up to Driver #1 and screamed at him "Why were your going so God Damn Fast!?" He said Driver #1 just kept saying
over and over, "I was only going 35."

Witness #2 (witness #5 on the incident report) stated that she was traveling north on S 17th St. in the left lane. She stated that Driver #1 went past her
"recklessly, like he had speed street rage." Driver #1 was in the left lane approaching the store entrance when a white truck (Witness #2) pulled out. Driver
#1 moved to the right lane, went around him and tried to move back to the left lane because there was a blue car in the right lane. That is when he struck the
cyclist.

My investigation revealed that Driver #1 was traveling in excess of 55 mph approaching the intersection. The speed calculations were derived from security
camera video footage from the store located at S 17th & Dock St. Witnesses stated Driver #1 was driving at an excessive rate of speed.

Driver #1 was initially charged with misdemeanor DWI pending the investigation of the crash. At the time of this report, a warrant has been obtained on Driver
#1 for Felony DWI inflicting serious injury, Careless & Reckless Operation of a Motor Vehicle, and Exceeding the Posted Speed Limit.

2018007967 March 14, 2018 7:57 pm 1503 S 3RD ST Investigation revealed that unknown Driver 1 hit Pedestrian 1 while on his skateboard and then fled the scene without stopping or reporting the accident.

Pedestrian 1 advised that he was skateboarding down the road after leaving the skate park in the PVA of 1503 S 3rd St (Greenfield Lake Park). Pedestrian 1
advised that he attempted to do a skate trick, an "Ollie," when after he landed he got hit by a car. Pedestrian 1 advised that he never saw the car coming so
believes that it came up from behind him. Pedestrian 1 advised that he got hit and landed on the hood of the suspect vehicle. Pedestrians 1 then advised the
suspect vehicle backed up to get him off of the vehicle and then ran over the victims prosthetic leg as he fled the scene. Pedestrian 1 advised that everything
happened so quickly that he did not get a good look at the suspect vehicle or the person driving the vehicle.

Witness 1 stated that he was at the skate park with his friend, Pedestrian 1, when they left and started to skateboard down the road. Witness 1 advised that
he was skating behind the suspect and watched Pedestrians 1 do a skateboard trick. Witness 1 stated that after Pedestrians 1 landed his trick, the suspect
vehicle came flying around the curve and hit the Pedestrian head on. Witness 1 advised that the vehicle hit the victim, stopped for a moment, then ran the
victim over and left the scene. Witness 1 stated that the suspect vehicle was a red Mazda 6, possibly an 2006-2008, with a partial license plate of "DJH."
Witness 1 stated that he believed the driver of the suspect vehicle was an middle aged man, approximately late 30`s early 40`s, with a tan complexion, dark
medium length hair, and was wearing sunglasses.

Pedestrian 1 was at the hospital at the time of the report with obvious injuries to his arm and hip area. Pedestrian 1 was also waiting for x-rays on his legs for
any evidence of any more injuries. Damage was also done to Pedestrian 1`s prosthetic leg.

CSI responded to take photos of the victims injuries and damage to his property.

2018011814 April 19, 2018 7:27 am 1198 MARKET ST/S 12TH ST Driver of Unit 2 stated she was heading east on Market St. Driver of Unit 2 stated she was in left lane going straight. Driver of Unit 2 stated she was going
about 35 mph and then slammed on the brakes when she saw the pedestrian in the middle of the roadway. Driver of Unit 2 stated it happen so fast she only
had time to slam on brakes but Unit 2 skidded. Driver of Unit 2 stated he vehicle did decease some in speed at time of impact. Driver of Unit 2 stated the
young female pedestrian had just ran across the roadway at Market St and S 12th St. Driver of Unit 2 stated she did not see pedestrian due to other moving
vehicle and when she finally saw the young female pedestrian she was at the white in color fog lines in the middle of the two eastbound lanes.

Witness stated he was heading east on Market St. Witness stated he was in the right lane beside Unit 2. Witness stated he had to come to a complete stop or
he would of struck the pedestrian in the roadway. Witness stated the pedestrian just ran across Market St at S 12th St. Witness stated Unit 2 could have
never seen the pedestrian until it was too late. Witness kept stating it was not Unit 2 at fault. Witness stopped to check on parties involved and provided a
witness statement.

Pedestrian 1 stated she was walking to school. Pedestrian 1 stated she had been waiting for a ride to take her but the ride did not come so she started
walking. Pedestrian 1 stated at 0723 she saw the time and started to jog east on Market St. Pedestrian 1 stated she then received a call from her ride and told
her ride she started to walk so she would not be late. Pedestrian 1 stated she hung up the phone and took headphones out of her ears and put same around
neck. Pedestrian 1 stated she crossed east over S 12th St then allowed some vehicles to pass on Market St before running north across Market St at S 12th
St. Pedestrian stated next thing she knew she was in the air. Pedestrian 1 stated she thought Unit 2 was going about 30-35mph. Pedestrian 1 stated she did
not see any other vehicle slow or stop for her. Pedestrian 1 stated she was struck on the left side of her body.

Pedestrian 1 was transported by EMS to NHRMC.

I observed damaged driver side windshield wiper, several cracks in the front windshield at the base of windshield on driver side, and a large dent to the hood
of Unit 2.

The intersection of Market St and S 12th St is a non-marked crosswalk with no traffic light, or crosswalk guard; just stop signs controlling north and south 12th
St traffic.

Pedestrian 1 was wearing a black in color ball cap, black in color glasses, red,blue, white in color plaid pattern button up shirt, had on a black in color
bookbag, black in color athletic pants, and black/white sneakers.

Based on statements obtained it was determined inattention and distraction by a communication device were non-motorist contributing circumstances. No
citation issued.
2018010068 April 2, 2018 11:07 pm 899 S 13TH ST/DAWSON ST Unit 2 was going North on S 13th St towards Wooster St. He was unable to tell me what happened. He was leaning on Unit 1 with his right leg in the air. He
said that he was riding his bike and was able to provide his name. I asked him if he collided with the vehicle and he kept saying that he was riding his bike. He
said his leg was hurting. I asked if he wanted to be seen by EMS and he said yes.

Unit 1 was parked next to the curb out of the roadway. Unit 1 was unoccupied during the time of the accident. I contacted the owner of Unit 1. He advised that
the truck was parked at the automotive shop waiting for a new motor. He then said that he would have the shop owner move his vehicle tomorrow.

My investigation revealed that unit 2 collided with unit 1. The owner of Unit 1 was advised on taking out property damage charges. Unit 1 driver was taken to
New Hanover Regional Medical Center to be treated for his injuries.

2018012638 April 26, 2018 12:08 pm 1 S 16TH ST/MARKET ST On 4/26/18, I, Officer Genda responded to the area of S. 16th St and Marsteller St reference an accident with injuries.

Upon arrival, I spoke with Mr. Christopher Bako, the driver of vehicle #1 and he stated he was heading eastbound on Marsteller St. and stopped at the stop
sign at S. 16th St. He checked to see if the traffic on S. 16th was clear to cross over. As he begin, vehicle #2 crossed in front of his vehicle northbound and
collided.

I spoke with Mr. Roderick Barker, a witness to the accident, and he stated that he saw vehicle #1 riding along the sidewalk and not stop before attempting to
cross Marsteller St heading northbound on the one way street. He saw vehicle #2 and vehicle #1 collide.

I then spoke with Mr. David Pike, and he stated he was heading home on his bicycle riding northbound on S. 16th St on the sidewalk. As he approached
Marsteller St, he entered the roadway, when vehicle #1 struck him.

Mr. Pike`s bike was locked up against a sign at this location per his request.

Drivers information was exchanged.

Mr. Pike was cited for failure to yield right of way.

No further information at this time.


2018017420 June 7, 2018 10:48 pm 1011 N KERR AVE/MARTIN LUTHER My investigation revealed that Pedestrian #1 crossed the highway when the signal indicated for a red light.

Pedestrian #1 stated she was walking North on N Kerr Ave. She stated that the light was red on N Ker Ave and she didn`t see any cars. When she began to
walk a car out of nowhere hit her. She stated she had no idea where it came from.

Driver #1 stated that he was Driving East on Martin Luther King Jr Parkway. When he approached N Kerr Ave intersection, he noticed a female last second in
the middle of the road and he severed. Driver #1 stated when we swerved and hit Pedestrian #1 with his mirror. Driver #1 stated that he did not see
Pedestrian #1 until last second.

Driver #1 reported no injuries and his vehicle was drivable. Pedestrian #1 reported injuries to her hips and right leg.

Nothing further.
2018017340 June 7, 2018 1:16 pm 2499 INDEPENDENCE BLV/SHIPYARD OFFICER`S INVESTIGATION REVEALED THAT VEHICLE 1 FAILED TO YIELD TO THE BICYCLE. THE BICYCLE WAS CROSSING THE
BLV INTERSECTION, IN A MARKED CROSSWALK, WITH A GREEN CROSS LIGHT. VEHICLE 1 WAS MAKING A RIGHT TURN AND DID NOT SEE THE
BICYCLE. THE TWO COLLIDED, THROWING THE BICYCLE RIDER TO THE GROUND. THE WITNESS OBSERVED THE COLLISION AND CONFIRMED
THE INVESTIGATION.
2018017046 June 4, 2018 5:11 pm 1 S KERR AVE/MARKET ST I met with the pedestrian who was struck at the hospital (NHRMC). He advised he was crossing from Food Lion West across Kerr Ave when he was struck by
Vehicle 1 which was going approximately 10 Mph. He advised the Driver initially stopped, however then kept going once he realized he struck a pedestrian.
He could only advise it was a black male driver and a silver 4-door vehicle. He stated he never saw the vehicle before he was struck but only caught a
glimpse as it was leaving.

The pedestrian had minor injuries which were being treated at the hospital. No further is known about Driver 1 or Vehicle 1.
