Reconstruction Post Wide Excision of Basal Cell Carcinoma with

Cutler-Beard Bridging Flap

ABSTRACT

Background: Basal Cell Carcinoma (BCC) is the most common malignant tumor occur

in the eyelid. Incidence of BCC is 90% of palpebral malignancy. The aim of this article is

to report BCC that has been treated with wide excision and palpebral reconstruction with

cutler beard technique.

Method: This case reported a man 42 years old, with tumor in his left upper eyelid. At the

examination we found mass at superior palpebra 7x5 mm size centrally with clear margin,

irregular, fixed, and no inflammation. We performed wide excision on superior palpebra

4 mm from the edge of the tumor so the defect size 11x9 mm. A marker was made in

inferior palpebra with the same size of tissue used for flap. Incision was performed under

the inferior edge of palpebra around 3 mm until donor flap turned to U shape. Inferior

palpebra flap was pulled up, and sutured layer by layer to the skin. Histopatology

examination found basal cell carcinoma nodular type. After operation, the defect closed

well. The flap was opened after 6 weeks

Result: The cutler-beard flap closed the defect perfectly with no significant side effects

or complications. The flap opened after 6 weeks and the result is satisfactory.

Conlusion: In this case BCC handled with wide excision and cutler beard reconstruction

technique gives the best result cosmetically and in this patients the visual acuity is 6/6

which is satisfactory.
