Clinical diagnosis: not stated, doctor made ultrasound scan but the inflammation process is too
big to see changes on the photo. Patient is waiting for reduction of inflammation process to
make another ultrasound picture

Actual history: Patient came to clinic because he has an insurance package. He felt pain in his
shoulder during lifting furniture, he heard a quiet pop and he couldn’t move his upper extremity.
He feels pain in anterior part of shoulder joint.

Medical/surgery history: there is not any surgery/medical history

Medication: non

- General observation – posture a little bit bent to right side, because of pain in right
shoulder,

 wrong biomechanics of movements in humeral joint due to scapula dyskinesia

Potential problems:
scapula dyskinesia – due to wrong position of scapula patient can’t stabilize scapula what
causes dysfunctional movements of humeral joint
Shorten muscles: pectoralis major, serratus anterior, subscapularis, supraspinosis
Weaken muscles: trapezius, teres minor and major

Physiotherapeutic diagnose:
Pain VAS 7/10 on the right shoulder ( insertion of rotator cuff) in the movements of the upper
limb above 40 degreases (abduction), 90 degreases (flexion) and external rotation (above 40
degreases) and instability of scapula due to rapture of cuff.

Prognosis:
Reducing pain in one week
Gain more range of motion in one week
Strengthen trapezius muscle and stabilize scapula in 3 weeks

TREATMENT
Goals:

Teach patient how to correctly stabilize scapula during movements

Treatment plan (5 times a week, 1 hour a day):

1. Mobilization of humeral joint to gain more range of movements


2. Passive movements: flexion, abduction, internal and external rotation
3. Massage of muscles: superspinosis, subscapularis, trapezius, pectoralis major
4. Exercises:
- at the first few meetings exercises which stabilize scapule and teach patient corect
bimechanical movements
*contraction of trapezius, serratus posterior, and rhomboideus major with previous
shouler protraction
- passive external and internal rotation
- passive flexion - active extension
- passive adbuction

- after reducing pain adding:


*active external and internal rotation
*active flexion
*resistance flexion
*active external and internal rotation
*resistance external and internal rotation
*active abduction
*resistance abduction

5. Mobilization of diaphragma to gain more ROM in humaral join
