To-Whom-It-May-Concern:

This letter constitutes the technical and preliminary introduction cost proposal of the -
United Block Captains' Association (UBCA) - Community Trust for Family Life Improvement, Inc.
(UBCA-CT) to provide its' copyrighted proprietary Nutrition Support System Programs for stress
management featuring nutritional therapy and education. The nutritional therapy is individualized
to the needs of each individual through the integration of assessment of stress status with instruction-
treatment of each individual. The nutrition therapy includes the provisioning of Vitamins, Herbs and
Specialty Foods as supplements to program participants' diets. Stress management instructional
services, and materials are provided to each program participant.

SITUATION

A review of direct escalating traditional health costs has motivated you to explore
alternative approaches to health cost containment. Your search has leaded you to conclude that a lack of
proper stress management by your organization's health services panel members is causing much of
the escalation of health-related costs. You have decided to implement a stress management
education program that features individual assessment with direct intervention programs tailored to
each individual’s condition.

Our Community Trust recognizes that nutrition education is generally not available in the
American educational or healthcare system. Our Community Trust has available through its
Partner Organizations a simple computer-based nutritional assessment program that enables our
Community Trust to provide our program participants (i.e. customers) with the best nutritional
assessment available. The computer based program permits individuals to participate in
nutritional status self- assessments. The nutritional self-assessment computer program uses each
individual’s nutritional self-assessment profile to develop a diet program proposal customized to
each individual’s particular needs that is offered under the Preferred Food Provider™ (PFP™)
Program.

PROGRAM TASKS MATERIALS SUPPLY

UBCA-CT provides an automated testing and evaluation program for placement of


program participants on nutritional therapy program levels.

UBCA-CT provides Stress Management Education Materials keyed to program participant


language skill levels and other demographic information.

UBCA-CT provides a nutritional support program for individuals that include herbs,
vitamins, natural juices and specialty food supplements as dietary supplements aimed at individual
stress management and substance abuse prevention. The program of provisioning nutritional
supplements controls stress by providing a physical counter to the impacts of emotional (i.e. personal
and job related situations) and physical exertion (i.e. trauma) stress on program participants. The food
supplement therapy program is geared to prevent substance abuse by individual through providing a
nutritional basis for the employee having a physical sense of being well (i.e. "a natural high").

The Health Care Management and Cost Control Life Stressors Management Program™
is a positive solution to the health and budget issues of providing wellness based healthcare.

The program reduces Life and Health Insurance Costs through the Life Stress Management
Program™ by:

1. Nutritional Stress Management


The Nutritional Stress Management Program is a health and nutrition explanation
workshop Outreach Program that operates by providing computer generated Health
Management Advice Individual Nutrition Diet Counseling Prescriptions. The computer based
assessment takes the place of conventional blood and hair analysis in identifying health issues.
The computer based assessment empowers program participants with the opportunity to take
control of the management of their health condition. The Nutrition Diet Counseling
Prescriptions are based on historical positive health practices that when applied to persons
with similar circumstances have in the past resulted in positive health outcomes.

Positive Health outcomes that include nutrition-based substance abuse prevention, acid
reflux disease management, headache prevention, muscle pain relief, diabetes (sugar)
management, high blood pressure management, arthritis pain relief and etc..

The Nutrition Diet Counseling Prescriptions provide specific diet item recommendations
available through Online Food Item Purchasing Internet Portal Sites. The Nutrition Diet
Counseling Prescriptions provide recommendations that are divided into the following food item

categories:
- Minerals
- Vitamins
- Herbs
- Specialty Foods and
- Whole Foods.

Our organization provides the Nutritional Stress Management Program computer technology of the
Life Stress Management Program™ and operates the Preferred Food Provider Program (PFP™).

2. Individual Diet Plan


Each Individual Diet Plan (IDP) has specific recommendations for each category of food
items (i.e. by minerals, vitamins, herbs, specialty and whole foods) that have a history of
helping to address specific health distress conditions identified in the nutritional stress
evaluation workshops. The IDP Counseling Self-Help Program through the computer-based
assessment program provides custom diet guidance. The program participant agrees to follow
the Individual Diet Plan (IDP) and enrolls in the Food & Health Spectrum Preferred Food
Provider Program (PFP) to receive orders on a regular schedule of the food items defined in
their IDP. The PFP is the operational implementation arm of the Nutrition Stress Management
Program by which program participants receive the nutritional diet counseling prescription food
items that transform their negative health status to a positive health state.

STRESS MANAGEMENT PROGRAM:

Environmental and work-related stress is becoming the number one cause of poor
life productivity and increased illness [i.e. individual poor health]. Our Community Trust
has available through its Partner Organizations a stress management program to assist in the
reduction of the negative impacts of stress from all sources. Environmental and work-related
stress can break down the body's immune system. Immune system weakness sets the stage
for individuals to acquire a number of diseases. Diseases are conditions of such severity,
that an individuals absence from the workplace is required; in order for the individual to
recuperate. Our Community Trust's stress management program is nutritionally-based and
provides the client-organization with an individually tailored program for each individual to
combat this situation.

NUTRITION STRESS MANAGEMENT
THERAPY’S PROGRAM FOR THE
PREVENTION OF DIABETES AND OBESITY
DATA COLLECTION AND
ANALYSIS
The project will use an integrated approach to data collection and analysis which is
ethnographic and functional in nature. The ethnographic study consists of observing
and interviewing clients in their natural habitat to discover their worldview, beliefs,
classification systems, rules of behavior, and patterns with the physical world and each
other. This approach is integrated with the functional approach which involves
automated collection design, implementation and analysis with constant feedback
periodically modifying outcomes throughout the duration of the grant.
The instruments for data
collection include:
(1) Nutrition Based Inventory (different inventories measure periodic
progress or regression),
(2) Drug Index,
(3) Life Events Survey and Interview (this will include the socio-economic factors
gleaned from the assessment instruments.
Through the use of a cluster analysis investigators will consider the multiple variables
that comprise life events of clients. The analysis will identify intervening factors
which may give rise to correlations between client experiences related to diabetes
and obesity and other aberrant behaviors. The nature of data solicited may begin
to establish links between elements lacking in the diet or environment of the client
and the nature and severity of diabetes and obesity and/or aberrant behavior
activity. The statistical inference derived from the cluster analysis will further
indicate itemized characteristics of diabetes and obesity and reveal the relationship, if
any, between identified negative behaviors diabetes and obesity.
The null hypothesis will determine whether the treatment modality results in a
statistically significant difference between the clients who participate in the myriad of
activities and the control group which did not participate. The independent variable is
the treatment modality which may or may not exert an effect on the experimental
group. In any event, the population of both the experimental and control group are the
same. Whether they are adults or juvenile, all are at high risk for diabetes and obesity.
These processes of data collection and analysis were selected because they lend
themselves to measuring the information obtained from the instruments used. For
example, field supervision and incapacitation option impacts are susceptible to both
cluster analysis and the null hypothesis. Further, these processes will easily allow the
correlations between life events, diabetes and obesity and other aberrant behaviors to
surface.

MONITORING
SYSTEM
The project staff will monitor the progress of the research project by an automated
program designed by Data Spectrum, Inc. One of the technological features of the
monitoring system is its ability to track clients even if they are no longer required to report
for program intervention. The tracking system is designed to follow the client for the
duration of the project irrespective of the whereabouts of the client.
Throughout the project clients are expected to complete periodic inventories and
surveys to determine the impact of the nutritional supplements on their propensity to
abuse substances, commit crime and engage in other aberrant behaviors. The client's
ability to conform to these and other requirements will be investigated at the outset. The
more non-conforming the client has been regarding other treatment modalities the more

project staff would be interested in tracking the client to discover the factors that
culminate in an client's conformity or non-conformity to societal norms and the
development of diabetes and obesity.
The monitoring system will generate for the funding agency status reports which indicate
the manner in which the project is achieving the results or benefits above outlined. It will
also report any unexpected events that occur and the manner in which the situation is
handled. The status report can also indicate the individual progress or regression of each
client.
EVALUATIVE
DEVICE
The project uses both a product and process evaluation as a single evaluative device. The
product evaluation measures the extent to which the project achieved its stated objectives
and to what degree the accomplishments can be attributed to the research project. The
process evaluation conducts an inquiry into whether the research project was conducted in
a manner consistent with the original plan. The process evaluation also determines the
relationship of different project activities, such as the orthomolecular approach, to
the effectiveness of the research project.
The evaluative device is automated. Both the principal investigator and the systems analyst
are involved in the evaluation. The investigator concentrates on the product evaluation
and the analyst handles the process evaluation. The systems analyst designed the
evaluative device so that the process of data collection parallels the process of analysis. The
data collection instruments include questionnaires, surveys and personal interviews.
Probation supervision staff will also be incorporated into the evaluation process. This
includes an examination of how the orthomolecular approach, if integrated with other
diabetes and obesity treatments are operationally effective.
The evaluative device will be activated within three months of the termination of the
research project as provided in the Time Chart. The time chart will be developed at the
beginning of the project in conjunction with funding agency personnel. The Time Chart
reflects the chronological order of the schedule of accomplishments and their target
dates.
