DISCHARGE PLAN

Medication

-Advise the patient to continue medications at home following the correct timing, dosage and
considerations.

-lozartan 500mg 1 tab Once a day

-Cefixine 400mg 1 tab twice a day

-Salbutamol 2mg neb twice a day

-montelukast 10mg 1 tab Once a day

Environment

-Instruct patient to stay in a clean and well-ventilated environment.

-Advise the patient to have a quiet environment free from pollution.

-Home environment must be free from slipping or accident hazards such as wet floor or sharp
objects.

-Advised to keep environment free form contamination such as dirt.

Treatment

-Advise to come back 1 week after discharge for the following check-up; July 19, 2018.

-Consult a physician for problems or any symptoms that are found DOB, chest discomfort, cough,
slight fever and chills

-Advise to have a regular check-up at the nearest Health Center.

-Advise to seek immediate medical care if experienced such as difficulty of breathing and Chest
Discomfort.

Health Teachings

-Advise patient to have adequate rest periods.

-Instruct patient to keep semi fowlers during rest periods.

-Promote early ambulation and other activity of daily living when possible.

-Instruct patient to avoid overexertion to prevent stress or trauma to the surgical sites.

-Encourage patient to avoid significant labor and emotional stress to avoid fatigue.

- Instruct patient to limit exposure to persons with respiratory infections.


-Instruct family in complications of disease and importance of maintaining medical regimen,
including when to call physician.- Teach patient the proper way to use nebulizer.

- Educate patient Sputum,coughing, deep breathing, and splinting techniques

Teach the patient or SO the importance of avoiding contact with individuals who have infection
or colds.

-Teach patient the proper way to use nebulizer

Observations

-Advice the patient to consult physician if there will be signs and symptoms or any complication
that may occur such as:

 Cough.
 Production of mucus (sputum), yellowish-gray or green in color.
 Fatigue.
 Shortness of breath.
 Slight fever and chills.
 Chest discomfort.
 frequent lung infections.
 a lot of mucus.
 reduced appetite and weight loss..
 blue-tinged lips or fingernail beds, or cyanosis, due to a lack of oxygen.
 anxiety and depression.
 sleep problems.

Diet

-Encourage patient to decrease intake of alcohol and other caffeinated drinks such as coffee and
cola.

-Advise patient to eat lots of fresh fruits and vegetables.

-Advise to eat green leafy vegetables such as broccoli and etc.

-Encourage patient not to eat lot of processed foods such as sardines, beef loafs, hams, hotdog and
etc.

-Encourage for daily requirement intake of Vitamin C such as citrus fruits.

-Encourage to eat rich in protein foods

-Emphasize to drink plenty of water 8-10 glasses per day.

- Emphasized to eat well balanced diet.

-Emphasized to eat high-fiber foods like whole grains, fruits and veggies, and beans and legumes

-Encourage increase Vitamin A (Retinol) in the diet such as potatoes, carrots, squash and
watermelon.

-Encourage Healthy fats from olive oil, avocados, coconut oil, nuts and seeds will improve immune
response

-Encourage patient to eat zinc such as baked beans and pumpkin seeds helpful for wound healing.
Spiritual

-Emphasize relevance of patient to attend regular mass in church every week with family

-Emphasize relevance of the patient to attend regular mass in Church every week with the family.

-Encourage patient to maintain good relationship with God through directed prayer and faith.

-Encourage to join spiritual growth activities for learning and to strengthen family bonding.

-Encourage to pray for complete healing and full recovery.
