We, MARICEL ROSETE-OMINGA AND JANICE V. PATRICIO, of legal ages,

married, Filipino citizens, , and residents of Isidro Paredes, Diffun, Qurino and Andres
Bonifacio, Diffun, Quirino, respectively, after having been sworn in accordance with law
aver that;

1. We have known JANNA KARYLLE U. PALLAYA since as an infant because her

father is our High School friend;

2. That her father is our very close friend and was our schoolmate in our high school
days;

3. That we know JANNA KARYLLE U. PALLAYA because we attended her

baptism;

4. That JANNA KARYLLE U. PALLAYA, IS THE DAUGHTER OF RONA U.

PALLAYA and JAYSON U. PALLAYA, whom are our high school friends and we
reside in the same Muncipality;

5. We are executing this affidavit to attest that “JANNA KARYLLE U. PALLAYA” is

the same person who was petitioned to Canada with her family;

6. That we are not related by affinity or consanguinity to the above mentioned person;
and

7. That we are executing this affidavit to attest to the truthfulness and honesty of the
foregoing facts and for all intents and purposes it may serve.
