RELEASE FORM (Musician)

I, ______________________________(Artist/Composer), have the authority to grant permission for use of the

attached listed songs; and I grant RGV Titan Radio permission to broadcast at their discretion any of the listed
songs; until permission is rescinded in writing with fair and reasonable time for transmit and reception thereof; in
exchange for promotion of musician(s), merchandise, and/or performances.

This document confirms the agreement between the Artist/Composer and RGV Titan Radio for use of the
referenced listed songs, Band/Artist name, and images associated with Artist/Composer in broadcast publications.
This form is to provide authorization for RGV Titan Radio to use Artist/Composer’s images and songs in order to
help promote Artist/Composer’s music in articles, on rgvtitanradio.com, RGV Titan Radio’s social media outlets and
at future RGV Titan Radio events. RGV Titan Radio has no authority to transfer permission. Likewise,
Artist/Composer’s listed songs will not be made available for free downloading.

Specifically:

Ownership

The Artist/Composer warrants that:

a. The referenced sound recording and its copyrights belong exclusively to the Artist/Composer.

b. The Artist/Composer knows of no disputes or encumbrances affecting ownership.

c. The Artist/Composer has the right and authority to make this agreement and believes it to be
enforceable.

Recording Release

The Artist/Composer agrees:

a. The attached listed songs can be used for broadcast release to the general public on radio, television,
Internet, or other accessible media for public use.

b. There will be no royalties or other payments provided to the Artist/Composer by RGV Titan Radio
associated with the broadcast of the attached listed songs.

Use of Name and Images

The Artist/Composer agrees:

a. The name of the Artist/Composer and the name of the attached listed songs can be used along with any
related listed songs or sound collections to identify the source of the listed songs for promotional
purposes expressly relating to RGV Titan Radio.

b. Any images – including album liner photos, trademarks, and publicity photos – or likenesses of the
Artist/Composer related to the attached listed songs can be used in conjunction with or independently of
the listed songs for promotional purposes expressly relating to RGV Titan Radio.
