METHOD STATEMENT
1. SCOPE

Transport prefabricated office Van to Site

2. MATERIALS

2.1 2 units 40 Footer Container Van Loaded with Electrical Rough-ins, Windows,
Doors and double walled with fiber cement board including ceiling at around
5 tons each

2.2 1 unit 20 Footer Container Van Loaded with Electrical Rough-ins, Windows,
Doors and double walled with fiber cement board including ceiling at around
3 tons each

3.1 Mobile Crane:

Rough Terrain Telescopic Crane at PICMW Yard – 60 Tonner Capacity

Truck Mounted Telescopic Crane at PKI Site – 25 Tonner Capacity

4. TOOLS

4.1 Sling: Lifting Belt 10 tons Capacity – 4 Units

4.2 Hard Fitting: Shackles – D type 2 tons capacity – 4 pcs.

4.3 Ladder – 1 pc

4.4 Tag Line - 2 pcs

1. UNLOADING PROCEDURE

1.1 Conduct toolbox meeting to discuss this rigging, lifting and transporting plan.

1.2 While travelling on site mobile cranes shall have hoisting tackle such as main
hook blocks and auxiliary hook blocks firmly secured to their car body and
outrigger beams secured with lock pins. Site speed limits shall be followed at
all times.

1.3 Crane shall be set up on firm ground maximum +/- degree out of level.
Check all the necessary parts during outrigger position.

1.4 Provide barricade within the lifting area.

METHOD STATEMENT
1.5 The truck is in reverse position. Assign rigger to signal the upcoming crane.

1.6 Established the maximum radius and ensure that access to load lifting points
is safe during the rigging as well as de-rigging.

1.7 Ladder shall be used as working platform only in case there is a secure
overhead point to attach a body harness to the ladder firmly.

1.8 Upon completion of rigging signalman shall confirm that the load is secured
to the crane and all personnel are clear of immediate lift area. Care shall be
taken to ensure that there are no loose objects along with the load that can
dislodge and fall during lifting.

1.9 All unessential personnel shall be advised to leave barricaded area and until
lift is finished lead rigger has ultimate responsibility of preventing
unauthorized entry.

1.10 Signal then shall be given to the crane operator to hoist up until all rigging is
tensioned.

1.11 Crane hoist lines then shall be brought into vertical position by manipulating
the boom. Signalman shall visually confirm that crane hook block assembly is
directly above center of gravity of load. Dragging or side load shall not be
tolerated by Sub-contractor under any circumstances.

1.12 Weight shall be gradually applied to the crane until the load is fully
suspended while watching crane radius and hoist line vertically for any
change that can be caused by boom deflection. All assisting personnel shall
stay clear and alert in anticipation of unexpected dislodgement of the load.
On the other hand all crane motions shall be performed in the slow and
gradual way to prevent the same.

1.13 Once the load is fully suspended above its supports hoisting shall cease to
allow riggers to perform final checks and crane operator to confirm the
weight. Load then shall be lowered to the ground to ensure balancing of load
and stability of the crane.

1.14 Any irregularities observed shall be reported to person in charge who has
ultimate responsibility of taking final decision.
