i) I will find a training establishment by my own. Details will be provided to Training
Coordinator through the form [FET/TRG/03] for recommendation and arranging training.
ii) I have no contacts with the Industry; please arrange a suitable training place for me.
ii) Clearly write and mark all the details, as they all are compulsory information. Double check the information included,
especially Student Name, Address of place of stay during the training period preferably, Reg. No, NIC No, Industrial
Training Module Code, Field of Specialisation and Contact Numbers. Make sure your signature is properly placed at the
bottom of the form along with the date.
iv) Those who possess relevant work experience in the industry and confident of obtaining exemptions from industrial training in
lieu of their work experience, SHOULD NOT FILL out the form [FET/TRG/01], even though they have already registered
for relevant training module/s. Instead they are advised to duly fill the [FET/TRG/07] form and submit to Training Engineer
along with relevant service certificates / work experience letters which should reflect the actual work performed by them
during employment. Anyone having work experience and already registered for industrial training module/s, can drop that
module/s during the “Period of Dropping Courses”. For the purpose of downloading the Exemption Requesting Form
[FET/TRG/07] along with the Training Guideline [FET/TRG/00], students are requested to follow the path given below.

v) Students who seek to secure a suitable training establishment on their own should inform the details of the place of training to
the Training Coordinator of the relevant department through the form [FET/TRG/03]. Then the Training Coordinator would
evaluate the place of training and will recommend to the Training Engineer regarding the suitability of the training place.
ii) Students who intend to DROP the industrial training module/s that they have already registered during the “Period of
Dropping Courses”, should strictly AVOID filling out the [FET/TRG/01] form.
iii) For all training matters students are requested to contact Training Engineer by phone on 011-2881223 / 011-2881429 or by
email through treng@ou.ac.lk. All types of forms and guideline pertain to industrial training will be sent to respective
student email in due course.
iv) [FET/TRG/02] form uploaded along with this form should have to be used by the students when their industrial training is
over. It should have to be dully filled and returned to the Training Engineer ONLY after the industrial training is completed.
It should by no means handed over or sent by post with the dully filled [FET/TRG/01] form.
v) The common CV format uploaded along with the two forms must be used to prepare individual CV of the student. It should
only be forwarded when requested by Training Engineer from the student, only through email and as a PDF softcopy only.
The PDF File naming convention should strictly be as follows.
CV_“Student Name with Initials in Block Capital Letters”_OUSL (E.g.: CV_A.B.C.PERERA_OUSL)
The CV should NOT be submitted to Training Engineer as a hardcopy unless he asked to do so from the student.
