PUNCTUALITY CONTRACT

This Punctuality Contract (“Agreement”, for brevity) is made and entered into this [Day] of
[Month] [Year] in __________, Philippines, by and between:

__________ (EMPLOYER)

and

__________, (EMPLOYEE)

RULES OF ATTENDANCE

General Rule

All employees are required to be punctual in reporting for work. Whenever the employee is
tardy or absent, he/she expected to assume full responsibility for the subject matter discussed.

Tardiness

1. The EMPLOYEE hereby agrees that if he/she will be tardy or late for three (3)
consecutive or successive instances, it shall constitute or be considered as one (1)
unexcused absence, unless the 3rd instance can be justified.

In the above-mentioned case, the EMPLOYEE shall provide a letter explaining the reason
for such incurred late.

Failure to submit the letter and explanation to the EMPOYER shall be considered a waiver
to explain further, unless the EMPLOYER will allow.

Attendance

2. The EMPLOYEE mutually agrees with the EMPLOYER and undertakes that if he/she
will incur absences, the latter will provide a letter explaining the reason for such absence
and proof, if required.

3. The EMPLOYEE agrees that if he/she will be absent for two (2) consecutive or successive
instances, and such absence is unexcused, unjustified or unauthorized within the period of
one (1) month, the employee’s entire allowance shall be forfeited in favor of the company.

4. Excused, Justified or Authorized absence is limited to the following:

a. Illness or Accident, as evidence by a duly notarized medical certificate.


b. Death of an immediate family.
c. Any other cause that the EMPLOYER may deem fit.
5. Failure to submit the letter explaining such absence and proof, if any, shall constitute as a
waiver to further explain its position.

6. The EMPLOYEE hereby agrees that the entire allowance to be forfeited because of the
unexcused, unjustified or unauthorized absence is with his/her consent.

Leave Entitlement

7. The EMPLOYER hereby grants to an EMPLOYEE (no. of leave) leave credits.

Procedure for Filing Leave

8. The EMPLOYEE hereby undertakes to submit a letter requesting to permit to go on leave,


or to fill out and complete an application for leave form, if any.

9. The EMPLOYEE’s request shall be subject to review and approval of the EMPLOYER.

10. Where the EMPLOYER denies the request for leave, the EMLPOYEE must report to
work as scheduled.
