SUMMARY MIDWIFERY CARE
Maternal and Child Health Problems (KIA) is still high in Indonesia, According to Indonesia Demographic and Health Survey (SDKI), Maternal Mortality Rate (AKI) is 359 / 100,000 to 305 / 100,000 live births.One efforts to decrease Mortality Rate and Mortality Infant is a Program of Birth Planning and Prevention Complications (P4K). And the implementation of care with the concept of Continuity Care is a continuous service and record-keeping and documentation. Methods were performed using the midwifery management approach at Ny.U from the 3rd trimester pregnancy to the family planning at the self employed midwife Eka Sriwahyuni from February to May 2018. The care given to Ny. D is Ante Natal Care (ANC) visit is 3 times. The care of the first stage of childbirth until the fourth epoch is normal but the mother has a fever before delivery so that the care of the midwifery is tight. Conducted childbirth care as much as 4 times and not found problems and complications. In newborn babies do newborn care as well as visits as much as 3 times and found complications in newborns that infants have asphyxia and then refer to Mitra Sejati Hospital. Through KB counseling, Ny.D decided to use 3 month injection. Assessment of Ny.D from pregnancy to family planning went normal and there were complications of fever before labor and the baby had asphyxia. It is expected that midwives at the self employed midwife Eka Sriwahyuni apply continuous service standards to pregnant women, childbirth, childbirth, BBL and KB to improve maternal and fetal well-being.
