ANNEX A - TERMS AND EMPLOYMENT STATUS

Your employment status shall initially be probationary for a period of 6 MONTHS, during which time the company
shall observe and evaluate your performance to determine whether or not you are qualified for a permanent
employment.

During the 5th month of your probationary period, your performance shall be assessed based on the standards detailed
in your employment contract and, if satisfactory, your probationary status of employment shall be converted to regular
status.

SCOPE OF WORK:

Job Description will be specified by the general manager and / or team leader upon signing of probationary contract.
During your employment, the job description will be revised and updated in which you will be informed of the changes.

One of the scope of works tasked to you is to make a daily report to visualize your performance, activities,
accomplishments, and achievements in each work day. This report is standardized by using a certain software (e.g.
Trello) or you may use your own way of reporting, provided that all duties for the day is well documented.

Your compensation and other monetary benefits shall be paid every month in accordance with the Company’s pay out
schedule for the year. Your monthly pay shall be subject to statutory deductions such as withholding taxes, SSS
contribution, PhilHealth, HDMF, and other authorized deductions.

ACCEPTANCE AND VALIDITY OF OFFER


This job offer shall deem be accepted upon the fulfillment of three conditions.

1st – your conformity to this Job Offer shown by affixing your signature on every page of this contract.
2nd – submission of all pre-employment documentary requirements.
3rd – the favorable results of your medical exam and NBI clearance. A copy of the list of pre-employment requirements
is hereto attached as PRE-EMPLOYMENT REQUIREMENTS.

These conditions are not waived by the company’s acceptance of your services into the Program. Non-compliance with
any of these conditions shall constitute a ground for your disqualification from your employment notwithstanding your
conformity to this Job Offer or execution of your employment contract.
