Midterm (1st Quarter) Release of Cards and Portfolio Day

To : All Teachers

1. School performance is measured by the efficiency and effectiveness in the delivery of

services to its clientele (students). Likewise, problems and issues will be addressed at once
when there is cooperation by every member of the academic community as well as accurate
and timely submission of reports.

2. The fact that the division office requires a speedy submission of reports by all schools,
the following should be submitted on or before their respective due dates.

REPORT DUE DATE

1. DTR (accurate record from logbook) END OF THE MONTH 2ND DAY OF THE MONTH
2. FORM 2 END OF THE MONTH 2ND DAY OF THE MONTH
3. ACTIVITY COMPLETION REPORT 2 DAYS AFTER THE ACTIVITY 3RD DAY after the ativity
4 MPS/RESULT OF THE EXAM 2 DAYS AFTER THE TEST 3RD DAY AFTER THE TEST
5. TOS/TEST QUESTIONS WITH 1 WEEK BEFORE THE TEST SCHEDULE( THOSE WHO WILL BE LATE WILL
ANSWER KEY BE THE ONE TO XEROX HIS/HER EXAM
6. Quarterly Pretest 1 week before the next quarter
7. Midterm Portfolio Day Saturday August 18, 2018 –Division Monitoring Team will come
