g 401(k) Investment Plan. I understand that, if I do not give different instruct
ions at the time of my eligibility, the Company may automatically enroll me and
deduct from my paycheck any contribution required under the automatic enrollment
provisions of the plan.
I understand that the Company reserves the right to monitor and inspect any and
all activities performed on or with the Company's property, work spaces, equipme
nt and systems. I understand that, if hired, my job may involve customer contact
by telephone. If so, my telephone calls may be periodically monitored for purpo
ses of training, customer service, and evaluating my job performance. I will not

I understand that if I am hired, I will have thirty (30) days from the date of h
ire to decide whether I want to remain covered by Step 4, the final step of the
Company's early dispute resolution program, Solutions InSTORE, which is final an
d binding arbitration, or whether I want to act to exclude myself from coverage.
I also understand that it is important to read all materials about the program
and ask any questions so that I am fully informed about Solutions InSTORE.
I understand that nothing contained in any handbook, manual, rules, regulations,
practice, policy or procedure may be interpreted as an employment contract betw
een the Company and me. The Company maintains a strict policy of at-will employm
ent with respect to both the terms and conditions of the employment relationship
. The Company may change the terms and conditions of the employment relationship
or terminate that relationship at will, with or without cause or notice, and wi
thout liability. No employee, supervisor, manager, or other representative of th
e Company has any authority to enter into any express or implied agreement to th
e contrary, and no promise, representation, inducement or agreement to the contr
ary is binding upon the Company unless it is in writing, it expressly states it
is an employment contract, and it is signed by the Vice President of Human Resou
rces of the Company and me. In applying for employment, I am not relying upon an
y promises or representations regarding the nature or duration or my employment.
