Preface

This Plan has been prepared by the Emergency Planning Unit, Orkney Islands Council, based on the
information provided to the Council by the Flotta Oil Terminal Operator regarding the nature, extent
and likely off-site effects of a major accident involving dangerous substances covered by the Control
of Major Accident Hazards Regulations 1999 (COMAH).

The Plan contains commercial information provided in confidence for official purposes. Its contents
are only to be disclosed on a "need to know" basis to Authorities concerned with implementing the
Plan in the event of a major accident at the Flotta Oil Terminal.

Recipients of this Plan are asked to:

(a) Read it and check all information for accuracy;

(b) Send any amendments immediately to the Emergency Planning Officer (EPO),
Orkney Islands Council, at the address shown below;

(c) Insert amendments to the Plan immediately upon receipt, ensure that the pages
removed are destroyed and the accompanying ‘Amendment Certificate’ returned to
the Emergency Planning Office;

(d) Ensure its safe custody.

PART 1 - INTRODUCTION

1 Legal Duty

1.1 The Local Authority, in whose area there is an establishment, shall prepare an emergency
plan (referred to as an off-site emergency plan) in respect of that establishment, and such a
plan shall be adequate for securing the objectives specified in the COMAH Regulations 1999,
Part 1 of Schedule 5 and shall contain the information specified in Part 3 of that Schedule.

1.2 Control of Major Accident Hazards Regulations 1999 (COMAH).

1.3 The Flotta Oil Terminal is a site to which the COMAH Regulations apply.

2 Authority

2.1 This Plan is issued under the authority of the Chief Executive, Orkney Islands Council,
following consultation with the Emergency Services, NHS Orkney, the Terminal Operator and
the Health & Safety Executive, to meet the Orkney Islands Council's responsibility to prepare
an “Off-Site” Emergency Plan under the Control of Major Accident Hazards Regulations 1999.

3 Aim

3.1 The aim of this Plan is to identify the roles and responsibilities and procedures to be adopted
to minimise the consequences to the health and safety of the community in the event of a
major incident/accident involving Flotta Oil Terminal. All of the response organisations
involved have their own emergency procedures and this Plan is not intended to replace or
supersede these.

4 Objectives

The objectives of this plan are to:

4.1 Minimise the effects of an incident to limit damage to persons and property;

4.2 Implement the measures necessary to protect persons from the effects of major incidents;

4.3 Communicate the necessary information to the public, the emergency services and authorities
concerned in the area.

5 Incident Probability

5.1 In view of the built-in safety employed in this industrial complex and the isolated location of
the site, the chances of an incident that would put the public/environment at risk off-site are
remote. Nevertheless, the possibility of such an incident occurring cannot be entirely
discounted and it is necessary to have an emergency plan ready for use if circumstances
require it.

6 Assessment

6.1 An assessment has been made of the worst possible scenarios that might arise from the
operations at the site. It is, however, possible that any actual incident may not necessarily
reflect the foreseen occurrence and this Plan must therefore retain the flexibility to meet
varying situations that can only be assessed at the time an incident has occurred.

PART 2 – SITE AND POTENTIAL ACCIDENT SCENARIOS

1 Location

1.1 Flotta Terminal is located on the island of Flotta, as shown in the maps at Part 6, Appendix 2.

2 The Local Community


2.1 Flotta Terminal is relatively isolated from the local community as can be seen from Maps at
Part 6, Appendix 2.

3 Industrial Premises around Flotta Terminal


There are no industrial premises around Flotta Terminal.

4 Site Activities

4.1 Unstabilised crude oil is processed, stabilised and stored in six floating roof crude oil tanks.
The liquefied petroleum gasses (LPG) are stored in one 1000,000 bbl tank.

4.2 The stabilised crude oil is pumped from the tank, via the pump house and on to either the jetty
or the Single Point Mooring where it is loaded onto tankers. Liquefied propane is loaded
solely at the jetty.

4.3 The site is normally in continuous operation and is staffed on a 24-hour basis.

5 Off-Site Pipelines

5.1 Pipelines to and from the site are not specifically covered by this Plan. Separate emergency
procedures exist to cover pipeline incidents.

6 Potential Accident Scenarios

6.1 In compliance with the COMAH Regulations 1999, the site operator has submitted an
“Establishment Safety Report” to the competent authority (as defined in the COMAH
Regulations 1999, Part 1, paragraph 2(1)(b)). The emergencies identified for COMAH at
Flotta Terminal were derived following systematic analysis as described in Part 2 of the
Safety Report

7 Hazard Identification

7.1 The Flotta Terminal Emergency Procedures details procedures for use on-site in the event of
any emergency.

7.2 The identification of hazards is summarised in the COMAH Safety Report at Appendix C to
Part 2, page 2.C.1 to page 2.C.9

7.3 This Off-Site Emergency Plan will be activated in the event of an emergency at Flotta
Terminal with off-site implications.

8 Emergency Definitions

8.1 The Flotta Terminal Emergency Procedures (part 1.3, sub paras 1.3.1 and 1.3.2) defines an
emergency as follows:

"1.3.1 Minor Emergency

This is an incident which can be controlled by the Terminal emergency resources. Limited
assistance may be required from specific offsite staff or external agencies.

1.3.2 Major Emergency

This is an incident which cannot be dealt with solely by the Terminal emergency resources
and requires the call-out, mobilisation and co-ordination of off-duty personnel and external
resources.

The only personnel with the authority to declare an emergency at the Terminal are the
Terminal Manager and the duty Terminal Superintendent."

8.2 This Orkney Islands Council Off-Site Emergency plan will normally only be activated in the
event of a major emergency.

PART 3 – RESPONSE

1 Activation of Plan

1.1 It is vital that at any incident at the Flotta Terminal complex an immediate assessment is
made of the particular hazards and a decision taken as to whether or not the OIC Flotta
COMAH Off-Site Emergency Plan requires to be activated. Responsibility for declaring a
major off-site incident will lie with the Police Incident Officer who will request the activation of
the Off-Site Emergency Plan when necessary. In the event of non-activation of the Off-Site
Emergency Plan the situation will be kept under continual review.

1.2 The OIC Flotta COMAH Off-site Emergency Plan, whether in whole or in part, will be
activated on the authority of the Chief Executive as the Council’s Emergency Controller.
Should, for any reason the Emergency Controller not be immediately available authority
should be sought from the Deputy Chief Executive or failing whom, the Council’s Emergency
Planning Officer.

1.3 Initial contact with the OIC should be via the pre-notified normal callout procedures as follows:

9am to 5pm, Monday to Friday 01856 873535

Out of Hours 01856 876338 (Roads Division Duty Foreman)

2 Control Responsibility

2.1 The responsibility for control and co-ordination of public safety measures will lie with the
Police Incident Officer, acting on advice given to him by the Senior Fire Officer, the Senior
Terminal Representative and other appropriate persons. During any off-site incident the
Police will be supported by other organisations. This may involve the deployment of OIC
resources.

2.2 While each organisation involved will have its own tasks to perform and emergency
procedures to follow, it is important that the overall response be based on an integrated
approach, co-ordinated by the Police Incident Officer.

3 Control Locations

3.1 In the event of an emergency on-site, initial action will be taken in the Terminal Incident
Control Room

If the incident is of significant proportions, the Major Emergency Control Room (Vanguard) will
be manned. Within the building communications and support facilities are available. This will
be known as the Major Emergency Control Room. In the event of the terminal building being
unavailable, an alternative Major Emergency Control Room will be established in liaison with
Northern Constabulary.

4 Immediate Response

4.1 Some of these actions may already have been initiated as a result of the initial response to an
incident by the Emergency Services. The primary responsibilities of emergency and support
services are detailed in Appendices 3 to 8.

5 Northern Constabulary (Talisman Multi-Agency Emergency Response Plan, Section 4.2


refers)

5.1 The duty officer, Force Operations Centre, will notify services and personnel as appropriate in
accordance with their Operational Orders. This will include notifying the other emergency
service controls; the Control Hospital (Balfour Hospital); the EPO (OIC) and the Health and
Safety Executive.

5.2 The Police will take immediate and appropriate action to protect any public in the area or
sectors affected or likely to be affected.

5.3 Arrange for a Police Officer to attend at the Major Emergency Control Room and to assume
the role and responsibilities of Police Incident Officer.

6 Highlands and Islands Fire & Rescue Service (Talisman Multi-Agency Response Plan,
Section 4.3 refers)

6.1 The Highlands and Islands Fire & Rescue Service Control Room will mobilise resources in
accordance with their Standing Orders.

6.2 This will include:

§ Immediate mobilisation of the pre-determined attendance to the Rendezvous Point

7.1 The Ambulance Control Centre will mobilise ambulance resources and alert the Receiving
Hospital in accordance with their Standing Orders.

7.2 This would include:

§ An initial attendance to the incident

§ Notification of the situation to the designated Receiving Hospital

8 NHS Orkney - Control Hospital (Talisman Multi Agency Response Plan, Section 4.7
refers)

8.1 The Control Hospital - (Balfour Hospital) will mobilise resources as required in order to give
medical care to casualties and initiate their own NHS Orkney - Major Emergency Procedure.

9 Orkney Islands Council /NHS Orkney- Emergency Planning Officer (Talisman Multi
Agency Response Plan, Section 4.5 refers)

9.1 The Emergency Planning Officer or their representative will be contacted by the Duty Officer,
Northern Constabulary Force Operations Room using the normal notified callout system.
They will:

§ Establish whether OIC assistance/resources are required in backup/support

§ Notify the Chief Executive, Orkney Islands Council, or in his absence the
Deputy/Assistant Chief Executive

§ Arrange for the OIC Emergency Centre to be established if necessary and instigate the
cascade callout list

§ Make contact with the Chief Executive, NHS Orkney, or in his absence the Consultant in
Public Health Medicine.

§ Alert, if required, local authority departments/sections and inform them of the possible
level of response required.

§ Inform the Civil Contingencies Division and the NHS Scotland Emergency Planning Unit of
the Scottish Executive Justice Department and the National Emergency Planning Officer,
Emergency Planning Unit, Scottish Executive Health Department.

10 Maritime and Coastguard Agency

As stated in the Talisman Multi-Agency Response Plan, section 4.6:

“The Maritime and Coastguard Agency will respond within its overall remit to any reasonable
request for assistance from any other authoritative agency."

PART 4 – IMPLEMENTATION

1 Identification of Key Personnel

1.1 To enable the rapid identification and movement of emergency services and key personnel
the following arrangements apply:

(a) Emergency Services: Terminal Operator Security Gate Staff will direct and assist
public emergency services on arrival at the site entrance. The emergency services
can be assumed to be adequately identified;

(b) Flotta Terminal Staff: Key call-in staff have authorised passes and should be given
every assistance by the Police in reaching the incident;

(c) OIC Staff: In the event that any OIC personnel require to approach or enter the
Terminal, arrangements will be made by the Emergency Planning Officer or their
representative to identify those staff to police road blocks.

2 Incident Development / Monitoring

2.1 As the emergency organisation is built up, response strategy may require to be adjusted in
the light of incident development and continual monitoring, frequent briefings and liaison
between all involved parties will be crucial.

3 Evacuation

3.1 Procedures for evacuation of people from the Terminal itself are detailed in Part 6, Appendix
9 of this plan. Emergency transport facilities may be required, and will need arranging
rapidly.

3.2 Any evacuation of persons within the off-site hazard area will be under the control of the
Police, and evacuation procedures will be decided at the time, taking into account the type of
incident, weather conditions and technical advice given by the site management.

3.3 If the situation warrants it the decision may be taken to evacuate not only the non-essential
people in the Terminal but also the island residents. It has been agreed with the Police,
Talisman Energy (UK) Limited., Flotta Terminal , the Orkney Islands Council and the
residents on Flotta, through the Flotta Community Council (meeting of Monday 18 October
2004, minute 1, refers) that the evacuation will be carried out on behalf of the local authority
by Talisman Energy (UK) Limited.

This is to avoid the practical problems associated with transporting local authority employees
into Flotta to carryout an evacuation when the need for essential transport would be at a
premium.

3.4 The procedure for the evacuation of Flotta residents can be found at Appendix 9 – Advice to
the Public.

3.5 Arrangements are in place for the Emergency Co-ordinating Officer to request assistance
from HM Coastguard to use SAR resources when not deployed in their primary role.

4 Casualties

4.1 Ambulance and medical services will deal with any casualties and arrange for their local
medical treatment or removal to facilities elsewhere as appropriate.

4.2 Normal Police procedures will be followed if fatalities occur.

5 Press and Public Relations

5.1 The Terminal Operator will set up a Press Information Centre at an appropriate location - the
telephone number will be issued at the time of the incident. All press statements about on-site
matters should be agreed in advance by the terminal management and the Police. All press
statements regarding off-site matters should be agreed in advance by the site management,
the OIC Chief Executive or his nominated representative and the Police.

6 Other Interests and Resources

6.1 Depending on the nature and longevity of any incident, support and assistance may be
obtainable or offered from a number of sources.

7 Meteorological Information

7.1 It will be important to establish wind and weather conditions at the site and to arrange for
continual forecast updates. The Police Incident Officer will ensure that a link is established
with the Meteorological Office, Aberdeen to monitor likely developments.

PART 5 – EVALUATION

1 Recording Events

1.1 An Event Log will be kept during the course of the emergency by all response organisations
recording events in 'actual time' from initial callout.

2 Financial Procedures

2.1 In an emergency situation, expenditure is likely to be necessary and because of the urgency it
may not be practicable to use normal approval procedures (e.g., hiring of plant and
equipment). In such circumstances the Chief Executive, Orkney Islands Council, through his
Director of Finance or representative, may authorise immediate expenditure of funds in
accordance with statutory provisions. It is important, therefore, that a record of expenditure is
kept in order that apportionments and claims may be settled at a later date. (Orkney Islands
Council Major Emergency Procedure, Part 5 refers.)

3 Stand Down

3.1 The responsibility for declaring any incident over and permitting 'stand down' in whole or in
part will lie with the Police Incident Officer after consultation as appropriate.

4 Review of Effectiveness

4.1 After the emergency situation has passed and relative normality returned to the affected area,
the Chief Executive will arrange a review of the effectiveness of the Off-Site Emergency Plan
by appropriate persons.

5 Testing of this Emergency Plan

5.1 This Plan will be tested at suitable intervals and revised / reviewed thereafter as necessary.

APPENDIX 1 - DEFINITIONS

In this Plan the following definitions apply. A number are detailed more fully in the COMAH
Regulations and other Statutory Instruments.

COMAH

The Control of Major Accident Hazard Regulations 1999 (Health and Safety) S.I. 1999 Number 743.

Dangerous Substances

Any substance named in Schedule 1 of COMAH Regulations 1999.

Police Incident Officer

The Senior Police Officer attending at the Incident Control Room, acting on behalf of the Chief
Constable, co-ordinating the various services and organisations involved.

Public Information Zone

The area defined under the Control of Major Accident Hazard Regulations 1999, as being the area
where people are liable to be affected by a major accident.

HSE

Health and Safety Executive.

SEPA

Scottish Environmental Protection Agency.

Industrial Activity

An operation carried out in an industrial installation referred to in the COMAH Regulations, involving,
or likely to involve, one or more dangerous substances and includes on-site storage and on-site
transport which is associated with that operation unless the operation is incapable of producing a
Major Accident Hazard.

Lifesaving Phase

Operations that are concerned with the protection of life. Consideration must also be given to the
protection of the environment and property.

Local Authority

Orkney Islands Council.

Major Accident

Means an occurrence (including a major emission, fire or explosion) resulting from uncontrolled
developments in the course of an industrial activity, leading to a serious danger to persons, whether
immediate or delayed, inside or outside, the installation, or to the environment, and involving one or
more dangerous substances.

Incident

Is used to describe the combination of circumstances that follow an accident.

Site

Means the whole of the area of land (182 hectares) under the control of the Terminal Operator.

Terminal Operator

Talisman Energy (UK) Limited.

Emergency Planning Officer

The OIC official whose duties include those of emergency planning.

Recovery Phase

This phase begins at the same time as the Emergency Phase and takes precedence after the Police
Overall Incident Commander is satisfied that all the Procurator Fiscals requirements have been
fulfilled. The Chief Executive of the local authority takes over responsibility for the recovery phase

APPENDIX 3 – ROLE OF THE NORTHERN CONSTABULARY

1 Introduction

1.1 The police co-ordinate the activities of all those responding at and around the scene (except
insofar as the Fire Service has such responsibility within the inner cordon). Unless a disaster
has been caused by severe weather or other natural phenomena, the area concerned must
be treated as the scene of a crime and preserved accordingly. The Police oversee any
criminal investigation. They also facilitate inquiries carried out by the responsible accident
investigation body, such as the Health and Safety Executive or the Air or Marine Accident
Investigation Branch. The police process casualty information, and have responsibility for
identifying and arranging for the removal of the dead. In this task they act on behalf of the
Procurator Fiscal who has the legal responsibility for investigating the cause and
circumstances of deaths arising from a disaster.

2 Police Role in any Major Incident/Accident

• The saving of life in conjunction with the other emergency services

• Co-ordination of the emergency services and other responding organisations during the
emergency phase of the incident

• To call out or place on standby essential services

• The protection and preservation of the scene, including access to and egress from the
site

• Evacuation procedures, undertaken in consultation with the other emergency services


and the Local Authority

• The investigation of the incident in conjunction with other investigating bodies where
applicable

• The collation and dissemination of casualty information

• Co-ordination of the response to the media in the emergency phase

• Identification of the dead on behalf of the Procurator Fiscal who is the principal
investigator when fatalities are involved

• Assist Orkney Islands Council with the restoration of normality at the earliest opportunity

APPENDIX 4 – ROLE OF THE HIGHLAND & ISLANDS FIRE & RESCUE SERVICE

1 Introduction

1.1 The first concerns of the fire service are to rescue people trapped in a fire, or in wreckage or
debris. They prevent further escalation of the disaster by extinguishing fires or undertaking
measures to prevent them spreading. They deal with released chemicals or other
contaminants in order to render the site safe. They assist the ambulance service with casualty
handling and the police with recovery of bodies. The fire service is responsible for the health
and safety of personnel of all agencies working within the inner cordon. They liaise with the
police about who should be allowed access to ensure that they are properly equipped,
adequately trained and briefed.

2 Fire and Rescue Role in any Major Incident/Accident

§ to immediately alert the Police and Ambulance Services


§ mobilise and deploy the Services' resources to respond to the incident
§ carry out rescue operations, fire fighting and the provision of emergency water supplies
§ establish communications with other emergency services and organisations involved
§ provide a Liaison Officer at the Major Emergency Control Room
§ liaise with the Site Management and keep the Emergency Co-ordinating Officer informed
of the situation
§ advise on any decontamination procedures that may be required
§ assist in evacuating where Breathing Apparatus is necessary
§ render such other assistance as may be required, including the provision of special
equipment available from Service resources
§ removal of surplus water
§ maintain a chronological record of the incident (in actual time)

APPENDIX 5 – ROLE OF THE SCOTTISH AMBULANCE SERVICE

1 Introduction

1.1 The major incident role of the Service is principally concerned with the immediate medical
needs of those directly involved in the incident, those associated with the incident and the
triage, treatment and subsequent transportation of casualties to sites of definitive medical
care.

§ Arrangements have been made for the Inverness Ambulance Control to be notified by
Northern Constabulary Police Headquarters in Inverness

§ The first ambulance crew on the scene will liaise closely with the Police and Fire
Incident Officers and with the Medical Incident Officer (MIO) at the locus. They will, in
consultation with the MIO, set up a casualty clearing station, an ambulance loading
point, an ambulance parking point and/or an equipment/stretcher-bearer point, as
circumstances dictate.

1.2 Further details on the actions required of Ambulance Service personnel are contained in the
NHS Orkney Major Emergency Procedure.

1 Introduction

1.1 NHS Orkney is responsible for ensuring that satisfactory arrangements are in place for
handling the public health and health care aspects of the response to a major accident.

2 Hospital Role in any Major Accident/Incident

2.1 The Medical Director co-ordinates the response of the NHS Orkney and other supporting
bodies.

2.2 Details of the role of NHS Orkney in an emergency/incident are contained in the NHS Orkney
Major Emergency Procedure.

APPENDIX 7 – ROLE OF ORKNEY ISLANDS COUNCIL

1 Introduction

1.1 As a local authority, the Orkney Islands Council has a statutory obligation to provide and
maintain a variety of area wide public services. In addition, the COMAH Regulations place a
duty on the Council to prepare and keep up-to-date an adequate “Off-Site” Emergency Plan to
deal with the off-site effects of a major accident in respect of every industrial installation
notified by the Health and Safety Executive.

1.2 In the immediate aftermath of a disaster the principal concerns of the local authority include:
support for the emergency services; support and care for the local and wider community; use
of resources to mitigate the effects of the emergency; and co-ordination of the response by
organisations other than the emergency services.

1.3 As time goes on, and the emphasis switches to recovery, the local authority takes a leading
role in rehabilitating the community and restoring the environment.

2 Orkney Islands Council's Role in any Major Incident/Accident

2.1 To mobilise, deploy and use the services and resources of the Council to the level considered
necessary in accordance with the Orkney Islands Council Emergency Plan and to respond to
calls for assistance initiated by the Emergency Co-ordinating Officer.

2.2 Chief Executive: The Chief Executive of Orkney Islands Council shall be responsible, during
the recovery phase of the emergency operation, for co-ordinating the efforts of the various
bodies and organisations whose assistance may be required.

2.3 Emergency Planning Officer: The Emergency Planning Officer or delegated person of Orkney
Islands Council will be responsible for alerting officials of the Council. They will advise them
of the major accident situation and any requests for assistance. Acting on advice from the
Emergency Controller, the EPO may be required to activate the Orkney Islands Council
Emergency Plan in whole or in part to control and co-ordinate the activities of the local
authority and those of supporting organisations, whenever the scale and complexity of an
accident warrants this.

3 OIC Departments/Services Involved

3.1 The resources of the Council may be involved either totally or partially.

4 Voluntary Organisations

4.1 Voluntary Organisations can assist in providing first aid, humanitarian assistance,
environmental protection and restoration activities. They will always be under the control of a
statutory authority, usually the local authority.

APPENDIX 9 – ADVICE TO THE PUBLIC

In the event of a Major Emergency at the Flotta Oil Terminal it is the responsibility of the Orkney
Islands Council to inform the residents of the Island of Flotta that an incident has occurred.

Orkney Islands Council has requested that the task of informing the local residents on Flotta of a
major emergency be carried out on their behalf by Talisman Energy (UK) Limited, the operators of the
Flotta Oil Terminal. This was agreed at a meeting of the Flotta Community Council held on Monday,
18 October 2004. Minute 1 refers.

In the event of a major emergency the terminal manager or his delegate will ask the terminal’s
security department to inform the island community of the incident and the requested actions to be
taken by them.

The terminal’s security department will inform the local community when requested by the terminal
manager or his delegate, by an automated phone messaging system. As a back up to this system the
terminal has public address systems fitted on two vehicles.
It is the responsibility of the Orkney Islands Council, with the assistance of Talisman, to ensure that
the phone messaging system is kept up to date.

Talisman Energy (UK) Limited will distribute an emergency information card to all households on the
Island of Flotta on behalf of themselves and Orkney Islands Council.

The Emergency Action Card will list the requested actions to be taken in an emergency and replicate
the instructions to be received on the automated phone messaging system.

1 “There is an emergency at the Flotta Oil Terminal” (repeated three times).

Required action: Go inside right away. Close outside doors. Shut windows. Close the
curtains on the terminal side. Stay in a room away from the terminal. Await further phone
instructions. Only make outgoing phone calls if absolutely necessary.

2 “Please evacuate to the Flotta Oil Terminal’s residential building” (repeated three times).

Required action: Evacuate via the West Hill road to the terminal’s residential building.
Should the evacuation happen prior to the arrival of the Police a nominated person will list all
the evacuated residents and the completed form will be handed to the terminal’s security
department who will copy it, the island residents retaining the original document. This is to
allow the terminal manager or delegate to know how many residents have evacuated to the
terminal. On arrival of the Police the nominated resident will hand over the original document
to the Police who will assume the task (if no suitable OIC representative is present on the
island) in case of evacuation of the island residents.

3 “The terminal emergency is over” (repeated three times).

Required Action: Return to normal routine.

The Police will take the decision as to whether or not an evacuation of the resident population will
take place in consultation with Talisman Energy (UK) Limited and the Orkney Islands Council.
