JUDICIAL AFFIDAVIT

I, SUSANNA PEREZ, a Doctor and Licensed Psychologist, of legal


age, married, Filipino, and a resident of 226 A. Luna Poblacion,
Mandaluyong City, under oath depose and state:

PRELIMINARY STATEMENT

The person interrogating me is ATTY. MA. FLORENCE R. FUERTE


with office address at 647 San Diego Street Sampaloc, Manila. The
interrogation is being held at the same office address.

I am answering his questions fully conscious that I do so under oath


and may face criminal liability for false testimony and perjury.
OFFICE TESTIMONY: The testimony of DR. SUSANNA PEREZ is being
offered as part of her testimonial evidence as a competent and qualified
expert witness to prove that:

1. The Respondent is found to be suffering from personality disorders


which are known as NARCISSISTIC AND HISTRONIC
PERSONALITY DISORDERS, as seen and manifested on the
following signs and symptoms:

a. Respondent is a materialistic person who failed to exhibit


affection and sympathy to Petitioner.

b. Respondent loves to be the recipient of male’s attention.

c. Respondent is an interpersonally exploitative woman.

d. Respondent is a demanding individual with a striking sense of


entitlement.

e. Respondent did not care about the feelings and needs of others.

f. Respondent lacked depth to her marital commitments and even


got involved with another man.

QUESTIONS AND ANSWERS

Q1: Ms. Witness, after this examination, you will be required to bring
with you a written judicial affidavit (which will embody the
questions and answers I have asked and you have answered,
respectively) to a notary public before whom and in his presence,
you will subscribe and swear the judicial affidavit. Will you do
this?

Q2: Are you fully conscious that in answering the questions being
asked of you now, it will later on be made under oath once you
appear before a notary public where the judicial affidavit will be
subscribed and sworn to before him?
