EQUIPMENT DETAILS:

The purpose of this document is to define additional requirements to the Purchase Order that
relate to inspection and testing to be performed and certification, reports and other
documentation to be furnished by the vendor. If these requirements are not complied with to
the satisfaction of PT. GLOBAL PETRO INTERNATIONAL, their representatives or the
Verifying Body, the goods or services shall be liable for rejection. The requirements apply
equally to vendors and sub-vendors. The documentation in Section 1.0 shall be forwarded
under cover of a Transmittal to:

SECTION 2: INSPECTION, TESTING AND DOCUMENTATION REVIEW BY PT.

GLOBAL PETRO INTERNATIONAL OR IT'S REPRESENTATIVES
The documentation in Section 1 shall be reviewed by PT. GLOBAL PETRO
INTERNATIONAL and when acceptable, shall be approved.
The extent of inspection or test shall be (MARK BOX THAT APPLIES):

G2 Limited inspection of the goods or services which shall include:

(a) a pre-manufacturing meeting or contact to discuss the purchase order

requirements, and

(b) final inspection and witnessing of acceptance tests (as nominated on

the Inspection and Test Plan if one is required).

G3 As per G2 but also staged inspection or testing as nominated on the

Inspection and Test Plan

SECTION 3: VERIFICATION OF INSPECTION, TESTING AND

DOCUMENTATION BY THE VERIFYING BODY
The Verifying Body SHALL/SHALL NOT be required to perform an inspection and
verification function (delete which does not apply).

The level of verification shall be (MARK BOX THAT APPLIES):

A2 Limited survey or inspection of the goods or services which shall include:

1. premanufacturing meeting or contact to discuss the purchase order
requirements, and
a)final inspection and witnessing of acceptance tests

A3 As per A2 but also staged inspection as agreed on an inspection and test

plan

When the services of the Verifying Body are required, PT. GLOBAL PETRO INTERNATIONAL
shall meet the Verifying Body’s costs. It is the responsibility of the vendor to ensure that the
Verifying Body is kept fully informed and notified of manufacturing and inspection schedules,
inspects the goods and services and/or verifies and approves the documentation. The extent
of the inspection and testing to be performed by the Verifying Body shall be as nominated by
that Body (which shall be on the Inspection and Test Plan when one is required).

1) The test reports shall be ACCREDITATION AUTHORITY endorsed (refer Note 3).
2) Material certificates shall be traceable to the material:
C1 - Certificate of conformance or certificate DIN 50049 2.1
C2 - Manufacturer's ACCREDITATION AUTHORITY endorsed certificates (refer
Note 3) or certificate DIN 50049 3.1.b
C3 - As for C2 but an independent body’s witness and endorsement is also
required or certificate DIN 50049 3.1.c

REQUIREMENTS FOR THE
MANUFACTURER'S DATA REPORT (PAGE 1 OF 2)
The Manufacturer’s Data Report shall include the following:
A face sheet showing the following:
 Item description
 Supplier/Fabricator details
 TCP or MAXIMO reference no. as supplied on the purchase order
 Purchase order no.

An index which shows:

 Page numbers of the various parts of the contents of the MDR
 The “serial” number of the MDR

A list of the drawings relevant to the purchase order.

The contents of the MDR shall include the final revision of relevant data as follows:

Inspection release certificate / note from the PT. GLOBAL PETRO

INTERNATIONAL inspector.

The Verifying Body certificate of inspection.

(If the final certificate is not immediately available an interim certificate, if
issued, may be included in the MDR. This should be replaced with the
final certificate when available).

Completed and signed ITP.

Traceability map.

Weld procedures, weld production test reports.

NDT reports (delete those not applicable to this job)
