ABSTRAK

Stroke hemoragik adalah suatu kondisi ketika pembuluh darah di dalam

otak pecah sehingga menyebabkan perdarahan di dalam jaringan otak atau di
sekitar permukaan otak. Edema serebral dan peningkatan tekanan intrakranial
merupakan komplikasi stroke yang serius dan merupakan tantangan besar pada
unit perawatan intensif stroke yang berpengaruh terhadap mortalitas dan
morbiditas pada fase akut. Salah satu terapi yang telah digunakan beberapa
dekade untuk menghilangkan edema serebral dan peningkatan tekanan
intrakranial adalah Manitol.
Tujuan penelitian menganalisis perubahan status neurologis dan tanda vital
pasien stroke fase akut yang mendapat terapi manitol di Rumah Sakit Kota
Medan. Desain penelitian ini deskriptif dengan pendekatan cross sectional dengan
jumlah sampel 34 responden, diambil secara purposive sampling. Analisa data
yang digunakan analisa univariat. Hasil penelitian menunjukkan terjadi perubahan
status neurologis dan tanda vital secara perlahan setelah pemberian manitol pada
pasien stroke hemoragik fase akut (<72 jam).
Kesimpulan terapi manitol (osmoterapi) merupakan standar emas (golden
standard) pengelolaan edema serebral dan peningkatan tekanan intrakranial (ICP)
yang telah digunakan sejak dahulu. Setelah pemberian terapi manitol terjadi
serangkaian perubahan-perubahan pada otak sebagai organ yang mengatur fungsi
tubuh dan akan mempengaruhi keadaan umum pasien. Perawat sebagai ujung
tombak pemberi pelayanan kesehatan yang berada 24 jam disamping pasien harus
mampu memahami perubahan-perubahan tersebut.

ABSTRACT

Hemorrhagic stroke is a condition when blood vessels in the brain rupture

causing bleeding in the brain tissue or around the surface of the brain. Cerebral
edema and increased intracranial pressure are serious complications of stroke and
are a major challenge in stroke intensive care units that affect mortality and
morbidity in the acute phase. One therapy that has been used for decades to
eliminate cerebral edema and increase intracranial pressure is Manitol.
The aim of the study was to analyze changes in neurological status and
vital signs of acute phase stroke patients receiving mannitol therapy at Medan
City Hospital. This research design is descriptive with a cross sectional approach
with a sample of 34 respondents, taken by purposive sampling. Data analysis used
univariate analysis. The results showed a change in neurological status and vital
signs slowly after administration of mannitol in acute phase hemorrhagic stroke
patients (<72 hours).
Conclusion Mannitol therapy (osmotherapy) is a golden standard for the
management of cerebral edema and increased intracranial pressure (ICP) which
has been used for a long time. After mannitol therapy, a series of changes occur in
the brain as an organ that regulates body function and will affect the general
condition of the patient. Nurses as the spearhead of health care providers who are
24 hours apart from patients must be able to understand these changes.
