PETITION

COMES NOW, the Petitioner, on his own behalf, unto this

Honorable Court, most respectfully alleges that:

1. Petitioner was born January 17, 1990, is twenty-seven

(28) years old, Filipino, single, and a resident of 789
Eagle St., Villa Rosal Subdivision, Magugpo South, Tagum
City, Davao del Norte, with office address at 2nd Floor,
Yap Bldg., 1st Avenue, Magugpo South, Tagum City,
Davao del Norte, and with cellphone number 0977 832
6864;

2. He has satisfactorily completed the required course in law
at St. Thomas More School of Law and Business on April
2017;

3. He has passed the 2017 Bar Examinations and was

admitted to the Philippine Bar on the year 2018;

4. He is a member of the Philippine Bar in good standing

with clearances from the Office of the Bar Confidant
(OBC) of the Supreme Court and the Integrated Bar of
the Philippines (IBP);

5. He has paid the required IBP annual membership dues as

evidenced by Official Receipt No. 043163, dated May 29,
2018;

6. He has paid the necessary Professional Tax as evidenced

by Official Receipt No. 2406449;

7. The undersigned has not been charged nor convicted in

the first instance of any crime involving moral turpitude;

8. He desires to be commissioned as Notary Public for and in

the Province of Davao del Norte, and possesses all the
qualifications and none of the disqualifications for said
office;

9. In compliance to the requirements of the Honorable Court

for Notarial Commission, attached hereto are the
following annexes:

a. Annex “A” - Petitioner’s Certificate of Membership

in the Philippine Bar under Roll No. 70584;

b. Annex “B” - Certification from the OBC that

Petitioner has no pending administrative case against
him before said office;

 c. Annex “C” - Certification of Good Standing by two
executive officers of IBP Davao del Norte -
Compostela Valley Chapter;

d. Annex “D” - IBP Official Receipt No. 43163;

e. Annex “E” - Professional Tax Receipt No. 2406449;

f. Annex “F” - Court Clearance issued by the Office of

the Clerk of Court of the Regional Trial Court at
Tagum City;
g. Annex “G” - Three (3) impressions of Petitioner’s
notarial seal and three (3) specimens of his
signature;

h. Annex “H” - Three (3) passport-size color

photographs with light background of Petitioner with
his signature at the bottom part thereof;

i. Annex “I” - Petitioner’s NBI Clearance.

PRAYER

WHEREFORE, premises considered, it is most

respectfully prayed of this Honorable Court that:

1. The Petition for Commission as Notary Public filed by

Petitioner be acted favorably; and
2. He be commissioned as a Notary Public for and in the
Province of Davao del Norte.

Other reliefs as may be just and equitable under the

circumstances are likewise prayed for.

del Norte, Philippines, after having been duly sworn in accordance with
law, depose and state that:

1. I am the Petitioner of this case and have caused the preparation

and filing of the foregoing Petition;

2. I have not commenced any other action or proceedings involving

the same parties or issues in any court, tribunals or quasi-
judicial agency and to the best of my knowledge, no such other
action or claim is pending thereon;

3. Should I learn that a similar action or proceeding has been filed

or is pending in the aforementioned courts, tribunal or agency, I
promise to notify this Honorable Court of the present status
thereof within five (5) days from my notice of the same;

4. I have read and understood the contents of this Petition and that
the averments therein contained are true and correct of my own
knowledge and/or based on authentic documents.
