SUBJECT: PREVENTIVE SUSPENSION

Dear Ms. Booc:

The management of the Panabo Polymedic Hospital, Inc. (the “Hospital”) has received a
report of your alleged taking/misappropriation of the SSS Loan Payments of the hospital’s
employees without the permission from the hospital’s management. Said
taking/misappropriation resulted to grave loss and damage to the hospital.

The Grievance/Ethics Committee of the Hospital, after serving you a Show Cause
Order/Notice to Investigate, your Explanation thereto, and investigation of the incident, found
you responsible for the said taking/misappropriation and recommended your preventive
suspension.

Thus, we are giving you this formal notice that you have been preventively suspended
by the Panabo Polymedic Hospital, Inc. effective immediately upon your receipt of this letter.
You are ordered to immediately cease and desist from further entering any transaction in behalf
of the Hospital. You are also prohibited from entering the Hospital’s offices and premises except
for your transactions with the undersigned in discharge of your accountabilities as stated
herein.

You will be scheduled for further administrative hearings in due time. Please note the
grave nature of the allegation against you which is punishable by termination. This is without
prejudice to other civil, administrative or criminal remedies which the Hospital may take against
you to protect its interest.

Your immediate action on this matter is enjoined.
