Klasifikasi diskolorasi menurut (Grossman dkk., 1995)
a) Diskolorasi Ekstrinsik Diskolorasi ekstrinsik merupakan perubahan warna yang terjadi di luar
permukaan gigi yang biasanya diakibatkan faktor lokal, seperti noda atau stain tembakau. Diskolorasi
jenis ini lebih mudah dilakukan perawatan pemutihan gigi (Walton dan Torabinejad,1997)
b) Diskolorasi intrinsik Diskolorasi intrinsik merupakan noda yang terdapat di email dan dentin yang
disebabkan oleh penumpukan atau penggabungan bahan di dalam struktur-struktur ini seperti stain
tetracycline (Grossman dkk., 1995)

1. Diskolorisasi kuning
Diskolorisasi kuning secara klinis terlihat sebagai plak yang mengalami pewarnaan kekuning-kuningan.
Diskolorisasi ini dihubungkan dengan keberadaan plak. Dapat terjadi di semua usia, dan lebih banyak
dijumpai pada individu yang mengabaikan kebersihan mulutnya. Penyebabnya biasanya berasal dari
pigman
2. Diskolorisasi coklat
Diskolorisasi coklat merupakan partikel tipis, translusent, biasanya bebas kuman yang mengalami
pigmentasi. Diskolorisasi gigi terjadi pada orang yang kurang baik menyikat giginya atau menggunakan
9 pasta gigi yang pola pembersihanya kurang baik. Sering dijumpai pada permukaan bukal gigi molar
rahang atas dan permukaan lingual incisivus rahang bawah.

3. Diskolorisasi tembakau.
Tembakau menyebabkan deposit yang berwarna coklat tua atau hitam dan melekat erat serta
menyebabkan perubahan warna pada gigi. Proses diskolorisasi pada tembakau ini disebabkan oleh
karena pembakaran dan adanya penetrasi air tembakau kedalam pit dan fisure enamel dan dentin.
4. Diskolorisasi hitam
Diskolorisasi ini biasanya terjadi berupa suatu garis hitam yang tipis pada permukaan oral dan
vestibular gigi dekat gingival margin dan berupa bercakan yang difus pada permukaan proksimal.
Diskolorisasi hitam melekat erat, ada kecenderungan terbentuk kembali setelah dibersihkan. Lebih
sering terjadi pada wanita dan bisa terjadi pada orang dengan oral higiene yang baik. Penyebabnya
adalah bakteri kromogenik.

5. Diskolorisasi hijau
Diskolorisasi ini berwarna hijau atau kuning kehijau-hijauan yang biasa dijumpai pada anak-anak.
Diskolorisasi hijau dianggap sebagai kutikula email yang mengalami pewarnaan. Diskolorisasi hijau
dapat terjadi pada semua umur, tetapi lebih sering terjadi pada usia anak. Anak laki-laki lebih sering
dibanding anak perempuan. Diskolorisasi ini dapat dikumpulkan dari gigi sulung maupun gigi
permanen. Komposisinya terdiri dari: 1) bakteri kromogenik dan fungi, 2) hemoglobin yang mengalami
dekomposisi dan 3) elemen-elemen anorganik termasuk kalsium, potassium, natrium, silikon,
magnesium, fosfor, dan elemen lainnya dalam jumlah kecil.

6. Diskolorisasi oranye
Diskolorisasi oranye disebabkan oleh bakteri kromogenik Serratia marcescens, Flavobactraiumlutes,
terjadi pada pekerja pabrik terpapar uap yang mengandung asam kromat. Biasa mengenai gigi bagian
anterior pada permukaan lingual.

7. Diskolorisasi metalik
Diskolorisasi metalik disebabkan oleh metal dan garam metal. Metal akan masuk ke dalam substansi
gigi atau menempel pada pelikel dan menyebabkan stain pada permukaan gigi. Proses tersebut dapat
terjadi karena pekerja industri menghirup debu industri melalui mulut sehingga menyebabkan substansi
logam berkontak dengan gigi.

2. Bagaimana mekanisme terjadinya diskolorisasi gigi?


Menurut Craig dan Supeene (1999), mekanisme diskolorisasi belum jelas. Pelikel yang melapisi email
diketahui mempunyai muatan negatif yang memungkinkan adhesi selektif terhadap ion positif dengan
permukaan gigi. Hal tersebut dipercaya menjadi peran pentingdalam deposisi Stain pada permukaan gigi.
Ion-ion dari makanan dan minuman yang mengandung tannin dan juga kromogen seperti tembaga, nikel
dan besi melekat dengan muatan negatif pada pelikel shingga menyebabkan diskolorisasi gigi.

3. Mikroabrasi
Microabrasion is a combination of chemical and mechanical technique where a microscopic layer of the
enamel is eroded and abraded away. It involves the softening and removal of 31 both enamel and the
surface defect and is accomplished with rubber dam isolation, hydrochloric acid (HCL) 18% mixed with
pumice, and a special geared-down handpiece

Teknik mikroabrasi merupakan salah satu teknik pengurangan email pada permukaan gigi yang ditujukan
untuk menanggulangi karies gigi dan menghilangkan fluorosis terbatas pada permukaan email Teknik ini
merupakan alternatif terbaik setelah menggunakan metode bor biasa dalam melakukan perawatan gigi
baik restorasi konvensional maupun perawatan perubahan warna, tidak memerlukan anestesi injeksi, tidak
menimbulkan rasa sakit, nyeri, linu, tidak menghasilkan getaran, gesekan dan panas seperti metode bor,
serta menghilangkan jaringan email sedikit saja. Prosedur perawatannya selektif karena hanya
menghilangkan struktur gigi yang rusak, lebih mudah, biayanya mudah, waktu kerja yang lebih cepat dan
nyaman bagi pasien. Teknik mikroabrasi terdiri dari teknik abrasi udara (kinetic cavity preparation) yang
berkerja dengan mekanisme tekanan dan kecepatan tinggi dari sambaran partikel-partikel abrasif yang
dapat menanggulangi karies pada gigi dan teknik mikroabrasi email.

5. Bahan apa yang tepat digunakan untuk kasus di scenario?
1. Hidrogen peroksida
Merupakan bahan pemutih yang paling sering digunakan, tidak berwarna, viskositasnya rendah dan
merupakan oksidator kuat sehingga dalam pengguanaannya harus hati-hati. Contohnya adalah superoxol
yang merupakan bahan pemutih yang mengandung 30% H2O2.
2. Sodium perborat
Penggunaan campuran superoxol dengan sodium peroksida lebih efektif dalam pemutihan gigi.
3. Karbamid peroksida
Merupakan kristal yang berwarna putih dan tidak toksik. Pada in office bleaching digunakan karbamid
peroksida dengan konsentrasi 30-50% sedangkan pada pemutihan ekstrakorona konsentrasinya 10-16%.
Efektivitas bahan pemutih intrakorona dipengaruhi oleh pH,konsentrasi,suhu, waktu dan penyimpanan.
Pada pH basa, proses oksidasi lebih aktif. Penggunaan bahan dengan konsentrasi tinggi prosesnya lebih
cepat namun ada kemungkinan menyebabkan kaustik pada jaringan lunak. Pengaruh adanya kenaikan
suhu dan pemanasan/energi cahaya akan mempercepat reaksi, selain itu, adanya kontak bahan pemutih
yang lama hasilnya akan lebih baik.
