On 21st Nov 2018, at around 04:30pm, AL Hajiry HSE Team planned to conduct a Work at height Mock
Drill Involving key personnel

Purpose:
The main purpose of this drill was to practice a emergency response scenario at Site, determine the

Lessons learnt & communicate those findings in order to optimize response time and appropriate

Procedure should such an event occur.

Scenario Location:
ON PSJV Contractor office
DRILL SCENARIO :
On PSJV contractor office AL REHWAN worker have fall down from the scaffolding
while installing roof sheet .The nearby person have intimated to the supervisor /
foreman who avails on the area ,supervisor called the emergency response team .
the site paramedic received the call & informed to the AHT HSE manager, AHT HSE
MANAGER Informed to the PSJV HSSE Manager who in turn called as the
PETROFAC SAMASUNG JOINT VENTURE Health safety security and environmental
manager.PSJV HSSE MANAGER informed to the PSJV H.O.C bout this incident who
in turn informed to the site management

Drill execution: …
Continued First Aider rushed to the site and checked the IP (Injured Person) and
found that he had no pulse and was not breathing, therefore he started CPR until
the Site First Aid Center staff arrived at site. The ERT members assisted him to
rescue man basket and brought down from the height of 6 Mtrs. As soon as they
arrived at site with the ambulance, the Nurse started to administer ACLS
(Advanced Cardiac Life Support) using also AED according to ACLS protocol. The IP
was transferred to clinic where he was consulted by the Doctor. At this moment
the IP was seen to have pulse and breathing autonomously, therefore after
consultation he was referred to nearby hospital for further treatment. A meeting
was held with AHT HSE Manager, PSJV Site HSSE Manager and all Key persons of
the Drill in order to discuss and analyze positive and negative feedback observed
during the exercise.
Drill Termination: After the exercise was completed, all the people involved in the drill have been
briefed about the outcome and the Drill terminated

Event simulated: PSJV contractor office AL REHWAN WORKSITE

Timeline of Emergency Drill Execution

04.00 PM IP collapsed and fall at work site (06 M height above ground level).

04.01 PM Colleague intimated Supervisor.

04.02 PM Supervisor intimated site emergency response team

04.04 PM First Aider examined the IP and started CPR

04.06 PM Supervisor contacted the site emergency number

04.09 PM Site Doctor intimated the nurse and ambulance rushed to the site.

04.10PM Site Doctor called AHT HSSE Manager who in turn called the AL HAJIRY HSSE Manager

04.12 PM AHT HSSE Manager informed his Site Manager who in turn called the head of construction.
The IP was rescued using rescue man basket from the height of 06 M slab.

04.14 AM Ambulance arrived at site; Nurse assessed the IP and used AED according to ACLS protocol

04.16 AM IP was shifted to site First Aid Center

04.18 PM Doctor informed site HSSE Manger that the victim was Okay and referred to nearby hospital

04.20 PM Drill’s positive and negative outcome have been assessed by PETROFAC, Representatives

04.22 PM Debriefing for all personnel's ended.

In Duqum refinery next to Al Hajiry SITE OFFICE, inside the PSJV contractor building area, Duqum around
04:30 Pm a person fall from roof while doing roof cladding activity.
