JUDICIAL AFFIDAVIT OF

PO3 HAZEL G. GARCIA

I, HAZEL G. GARCIA, of legal age, married and a resident of R.Mapa St.,

Brgy. Aiport, Mandurriao, Iloilo City, state under oath as follows:

Preliminary Statement

I am being questioned by Atty. Scarlette Coopera, at Coopera Law Office, 12

San Agustin, Jalandoni St., Iloilo City. I am answering her questions fully
conscious that I do under oath;

Question 1: What is your office and can you describe your duty in said office?

Answer 1: My office is that of a property custodian and patrol member of

Police Precinct 1 Iloilo City and it is my duty to manage and secure
any evidence of any crime or incident and likewise perform all the
regular functions of a PO3.

Question 1: Can you narrate what transpired on October 13, 2018?

Answer 1: Yes Sir. On October 13, 2018 at around 4:30 o’ clock in the
morning , while I was at my desk tending to my duties in the above
mentioned Police Station, Maria Agustin one of the crew members
of 4th Box Computer Shop, located in Jalandoni St., Iloilo City,
went to the police station and reported about a stabbing incident
positively identifying a certain John Ray Agregado as the one who
stabbed PJ NAZARENO
Question 2: What have you done in response to the report of the above
mentioned stabbing incident?

Answer 2: Upon the order of our chief commanding officer PSI Rogelio
Ortigas, I together with PO1 Jee Fabregas immediately responded
to the scene of the incident.

Question 3: Where is the scene of the incident located?

Answer 3: Its location Sir is found just right outside of the above-mentioned
Internet Station.

Question 4: What did you do upon arrival at the scene of the incident?

Answer 4: When we arrived, we conducted series of interviews to the people

around the area who actually witnessed the incident; and found that
they positively identified John Ray Agregado as the culprit and the
person who stabbed PJ Nazareno.

Question 5: After the inquiries what did you do next?

Answer 5: Under the command of PSI Rogelio Regala we proceeded to

conduct Hot Pursuit Operation.

Question: What became of your Hot Pursuit Operation?

Answer: We initially went to his house, then to the house of his relatives
and friends and any possible place he could hide himself.
However all of our efforts were to no avail. We were
unsuccessful in apprehending him.

Question: What did you do next?

Answer: After the failed hot pursuit operation, we went back to the police
station and have the incident blottered and rendered our spot
report.
