You currently owe to me a debt in the amount of ₱25,000 (the ‘Debt’) related to Purchase of
Smartphone. This debt was originally due on May 24th 2018 and, after repeated requests for
payment, remains outstanding.

Please refer to the attached Acknowledgment Receipt for your reference.

I hearby demand that payment of the full amount of the Debt be paid to me within 30 days from
the date of this letter to the address listed above.

Please note that, should I initiate legal proceedings to secure repayment of the Debt, this letter
will be tendered in court as evidence that you have not attempted to resolve this issue. You may
be liable for the costs of litigation, including attorney fees, damages, and/or punitive damages.
