PENDAHULUAN
Akan tetapi, konsensus ini gagal memberikan kriteria klinis dan histologis
yang dapat diandalkan. OLL tidak selalu lebih terlokalisasi dibandingkan
OLP. Kami juga tidak mempunyai bukti kuat bahwa OLCL adalah dalam
hubungan topografis dengan isi amalgam dan hubungan temporal OLDR
dengan konsumsi obat tertentu adalah bervariasi dan juga daftar obat yang
diduga berhubungan. Selain itu, beberapa entitas, dengan menggunakan
karakteristik klinis dan/atau histologis LTR/IFD di mulut, telah dikeluarkan
dari klasifikasi tersebut. Penulis lain juga telah mengajukan klasifikasi
alternatif (Cortes-Ramirez, Gainza-Cirauqi, Echebarria-Goikouria, dan
Aguirre-Urizar, 2009; Ismail dan Kumar, 2007; Van der Meij dan Van der
Waal, 2003), tetapi masih ada persetujuan umum yang kurang terhadap OLL
(Corrozzo dan Thorpe, 2009; Aguirre, 2008; Van der Waal, 2009).

TINJAUAN PUSTAKA
2.1.1. Definisi
Oral Lichenoid Reactions atau Lesions (OLR/OLL) adalah
perkembangan klinis dan histologis Oral Lichen Planus (OLP) yang sering
tidak dapat dibedakan dalam manifestasinya. Patokan pembeda antara
kedua kelompok tersebut adalah hubungan OLR/OLL dengan faktor
pencetus yang diketahui, yang ketika teridentifikasi dan dieliminasi, sering
menyebabkan regresi pada lesi (Kamath, Setlur, dan Yerlagudda, 2015)
2.1.2. Etiologi
2. Obat-obatan
Literatur menyebutkan bahwa banyak obat-obatan sistemik
yang kemudian menimbulkan komplikasi di rongga mulut.
Komplikasi tersebut misalnya hiposalvasi (xersotomia), Apthous-
like ulcers, hyperplasia fibrovaskular, infeksi, dan reaksi Likenoid
atau yang lebih dikenal dengan istilah Lichenoid Drug Eruptions
(LDEs). LDEs pertama kali dilaporkan pada tahun 1940 yakni pada
kasus rheumatoid arthritis yang mendapatkan tatalaksana dengan
emas. Pada thun 1945, ada juga laporan LDEs yang terjadi pada
orang yang mendapatkan obat anti-malaria (Dudhia, Dudhia, Patel,
dan Jani, 2015).
Semakin lama prevalensi LDEs semakin meningkat dan obat-
obatan yang memicu pun semakin bervariasi. Obat-obatan tersebut
antara lain :

Angiotensin Converting Enzyme-inhibitors
Diuretik (khususnya golongan Hydrochlorothiazide)
B-blockers
Obat oral untuk diabetes (terutama golongan Sulfonylurea)
Anti-konvulsan (misalnya Carbamazepine)
Anti-retroviral
Obat immunomodulator (misalnya gold salts dan
Penicillamine)
Imanitib mesylate STI571
NSAID (Yuan dan Woo, 2015).
2.1.3. Epidemiologi

unilateral (Oliveira, Souto, dan Goes, 2015).

2.1.10. Penatalaksanaan
Pada saat ini masih tidak ada obat untuk OLP. Perawatan saat ini
hanya bersifat paliatif dan penatalaksanaan secara umumnya hanya
empiris. Penatalaksanaan OLP hanya untuk kasus yang mempunyai gejala.
Kortikosteroid Topikal dan obat topikal yang lain dapat digunakan untuk
pasien yang gejalanya ringan. Setelah gejala penyakit tersebut telah
berkurang, obat yang digunakan harus dikurangi. Pasien dengan gejala
OLP yang menyebar dengan luas, diskuamatif gingivitis, atau beberapa
mukokutaneus mungkin memerlukan terapi imunomodulator yang
sistemik (Schlosser, 2010).
2.1.11. Pencegahan
OLR berhubungan dengan bahan restoratif gigi, seperti amalgam,
akan tetapi, terdapat sedikit bukti yang mengatakan bahwa penggantian isi
amalgam dapat mengurangi OLR (Murray, Nunn, dan Steele, 2003).
2.1.12. Prognosis
dan prognosis. Tetapi, ciri klinis dan patologis mereka bertumpang tindih
dan menyebabkan dilema diagnostik bagi dokter. Membedakan jenis OLR
yang satu dengan yang lain adalah wajib karena beberapa OLR (seperti
GVHD), reaksi likenoid akibat amalgam, DLE mempunyai kecenderungan
tinggi untuk keganasan (Hiremath, Kale, dan Charantimath, 2011).
Laju transformasi maligna OLP tertulis 0,5-2% dalam berbagai
studi. Meskipun tidak ada studi besar dalam prediksi transformasi maligna
OLR, ada studi yang menuliskan bahwa laju transformasi maligna pada
OLR adalah 2,1%, sedangkan pada OLP adalah 0,5% (Van der Meij, Mast,
dan Van der Waal, 2007).

HASIL DAN PEMBAHASAN
3.1. Hasil
Hasil ulasan ini didapatkan dari pembahasan yang dilakukan oleh
para ahli. Kami mempunyai studi epidemiologis yang kurang pada OLP,
terutama karena kesulitan dalam membedakan OLP dari sejumlah lesi
yang mirip OLP. Klasifikasi yang sudah ada tampak tidak mampu
mewakili semua kemungkinan OLTR dan terutama yang mengenai
kurangnya pertimbangan terhadap LE. Satu-satunya OLTR yang mungkin
dapat mudah dibedakan dari OLP tanpa pemeriksaan yang lebih jauh
adalah cGVHD (Meroni dan Schur, 2010).
Tabel 3.1. menunjukkan daftar penyakit utama yang menunjukkan
bahkan karakteristik histologis dan/atau klinis oral LTR (OLTR)/IFD yang
kecil. Beberapa kelainan ini jarang, sedangkan yang lain sering sulit dinilai
atau hanya mewakili istilah yang tidak cocok. Akan tetapi, semuanya
dapat sulit dibedakan dari OLP klasik.
3.2. Pembahasan
3.2.1. Oral Lichen Planus (OLP)

Salah satu target utama antigen dalam MMP adalah suatu molekul
hemidesmosomal transmembran dengan 180 kDa yang disebut bullous
pemphigoid antigen 180 (BP180). Menariknya, IgG anti-BP180 yang
bersirkulasi dalam kadar rendah sering dilaporkan dengan menggunakan
immunoblotting pada ekstrak seluler kultur keratinosit dalam hingga 17%
pasien OLP dan dianggap merupakan fenomena penyebaran epitop.

Akan tetapi, model hewan telah gagal dalam membuktikan bawha isi
amalgam dapat menyebabkan OLCL (Dunsche, et al., 2003). Suatu studi
yang lebih baru yang menggunakan model hewan yang sama (Brown
Norway Rats) tetapi dengan modalitas paparan yang berbeda,
menunjukkan bahwa raksa nontoksik dapat menyebabkan lesi mukosa
mulut lupus-like (Seno, et al., 2013).
Tidak ada uji spesifik terhadap OLDR. Adanya lesi unilateral dapat
membantu (Lamey, McCartan, MacDonald, dan MacKiel, 1995), tetapi
bukan merupakan tanda yang konstan. Secara histologist, infiltrat subepitel
yang mengandung jumlah eosinofil yang relevan dan/atau sel plasma dan
yang lebih tersebar dan lebih dalam daripada OLP atau mempunyai
penampilan vaskuler dapat ditemukan (Van den Haute, Antonie, dan
Lachapelle, 1989). Utamanya, tidak satu pun dari karakteristik ini telah
dilaporkan dan, sebaliknya, penemuan histologis tersebut dapat ditemukan
pada DLE (McCartan dan McCreary, 1997).
DLE adalah bentuk yang paling sering dari LE kutaneus kronis dan
dapat dibagi menjadi dua kelompok: lokal dan generalisata. Penyakit yang
tersebar luas ini lebih sering dihubungkan dengan abnormalitas
pemeriksaan lab (Oke dan Wahren-Herlenius, 2013). DLE telah
dihubungkan dengan area gen HLA dan lokus non-MHC (Tabel 3.1.),
tetapi bukti lebih lemah daripada pada SLE (Jarvinen, et al., 2010;
Jarvinen, et. al., 2010; Najou, et al., 2011).

dan sekitar 40% lesi OLP yang dilaporkan (Zaraa,et al., 2013) . Hanya tiga
kasus yang menunjukkan lesi oral eksklusif telah dipublikasi (Solomon, et
al., 2007; Mignogna, et al., 2010), tetapi tanda klinis dan imunologis yang
serupa menuju ke diagnosis MMP pada sedikitnya satu kasus lain yang
dipublikasi (Lee, Blazek, Beltraminelli, dan Borradori, 2011).

selain itu, spongiosis eosinofilik juga dapat merupakan suatu ciri

KESIMPULAN
Pada saat ini masih tidak ada obat untuk OLP. Perawatan saat ini hanya
bersifat paliatif dan penatalaksanaan secara umumnya hanya empiris. Peawatan
tersebut bertujuan untuk menjaga kebersihan gigi dan mulut. Penatalaksanaan
OLP hanya untuk kasus yang mempunyai gejala. Kortikosteroid Topikal dan obat
topikal yang lain, misalnya Cyclosporine atau Retinoid, dapat digunakan. Pada
kasus yang berat, injeksi Kortikosteroid pada lesi atau Kortikosteroid secara
sistemik dapat diberikan.
