inactive decapeptide angiotensin I is converted to the pressor,
aldosterone-stimulating octapeptide angiotensin II in the
pulmonary circulation.
• The reaction occurs in other tissues as well, but it is
particularly prominent in the lungs.
• Large amounts of the angiotensin-converting enzyme
responsible for this activation are located on the surface of

• Lethal ventricular arrhythmias occurred in several patients

taking either of the early second-generation agents,
terfenadine or astemizole, in combination with ketoconazole,
itraconazole, or macrolide antibiotics such as erythromycin.
• These antimicrobial drugs inhibit the metabolism of many
drugs by CYP3A4 and cause significant increases in blood
concentrations of the antihistamines.
• Nefazodone is an inhibitor of the CYP3A4 isoenzyme, so it can
raise the level and thus exacerbate adverse effects of many
3A4-dependent drugs.

• Tiagabine is an inhibitor of GABA uptake in both neurons and

glia. It preferentially inhibits the transporter isoform 1 (GAT-1)
rather than GAT-2 or GAT-3 and increases extracellular GABA
levels in the forebrain and hippocampus where GAT-1 is
preferentially expressed.
• Vigabatrin is an irreversible inhibitor of GABA
aminotransferase (GABA-T), the enzyme responsible for the
degradation of GABA. It may also inhibit the vesicular GABA
transporter.

• Substituted benzamides include metoclopramide and

trimethobenzamide.
• Their primary mechanism of antiemetic action is believed to
be dopamine-receptor blockade. Trimethobenzamide also has
weak antihistaminic activity.
• The principal adverse effects of these central dopamine
antagonists are extrapyramidal: restlessness, dystonias, and
parkinsonian symptoms.

• Drug metabolism primarily occurs in the liver and the wall of

the small intestine, but other sites include plasma, lung, and
kidney.
• Induction (stimulation) of cytochrome P450 isozymes in the
liver and small intestine can be caused by drugs such as
barbiturates, bosentan, carbamazepine, efavirenz,
nevirapine, phenytoin, primidone, rifampin, rifabutin, and St.
John's wort. Enzyme inducers can also increase the activity of
phase II metabolism such as glucuronidation. .
