Dear Sachi Hamai,

This request for information constitutes the requestor’s legal right to

access information.

I would like to know whether or not the following named officials: 1) Schwartz, Keith Louis (Superior Court Judge), 2) Logan,
Dean C (County Recorder), 3) Lauren Weis Birnstein (Superior Court Judge); are covered under any Liability Policy, including
matters of anti-trust violations, ethical compliance, errors and omissions coverage causing property damage or bodily injury, as
well as any or all exclusions.

Please provide a copy of the relevant records, including actual policy terms, policy number, coverage dates, whether the policy
operates on a claims-made, or per occurrence basis, and include any schedules, and the names of all suretys and underwriters,
specific to the relevant policies.

Please provide the types of insurance policies (if any) that aforementioned persons would be required to carry per their public,
corporate, or fiduciary offices of government, and if there be any required coverage that isn’t active, please provide the record of
the date such policy was canceled, or last active.

If any of the above persons are included in a policy of Self Insurance of an Agency or Corporation, please provide certified proof
of insurer solvency, and ability to cover a liability of a minimum of $1,000,000.00. Certified proof would be an asset sheet
certified by an independent accounting professional, or a statement of collateral.
