1. Reference: Memorandum from Barbosa PCP Commander re-DPAR dated September

22, 2017.
2. In connection with the above subject and reference, with due respect, the undersigned was
performed his duty as Night Shift Personnel.
3. That on the above date at about 9:00 O‘clock in the morning the undersigned attending his
rehearsal for the donning and pinning of rank at the NCRPO Parade Ground located at Camp Bagong Diwa,
Bicutan Taguig City. (See attached Memorandum from C, RPRMD/R1 dated September 19, 2017)
4. Due to the rehearsal was start at around 9:20 AM and ended at about 11:10 AM he was
exhausted and deteriorated because he was duty last night and the undersigned is needed a long time travel
to go home from Bicutan up to Sta Maria Bulacan. However, he still is performing his duty at that night.
5. Other just and equitable relief under the premises is likewise prayed for.

1. References:
a. OD report dated September 11, 2017; and
b. Spot Report Re: Stabbing Incident dated September 12, 2017.
2. In connection with the above subject and references, with due respect, that on September 11,
2017 the undersigned was performed his duty as Duty Officer up to September 12, 2017 after his tour of
duty he conducting Station DPAR.
3. That on September 12, 2017 at about 5:50 AM the undersigned received telephone call
from duty investigator PO3 Evero of the Station Base. He informed me that they have reported stabbing
incident transpired at about 4:00 o’clock in the morning along CM Recto Avenue near at Isetan
establishment Sta Cruz Manila.
4. He asked me also that “ Sir gagawan pa ba ng Spot report eh patay na yong tao at kukunin
nang Homicide Section yong patay” I asked him “gumawa ka ng report kahit incident report lang siguro”
all conversation was made between 5:55 to 6:10 in the morning.
5. Due to, my report was already submitted at around 5:15 AM in the morning, the stabbing
incident was not included in my report because the undersigned was late informing regarding this matter.
6. As a proof of the undersigned allegation see affixed signature of PO3 Evero in the left
bottom of his explanation.
7. The undersigned has no intention to defy any lawful order of this Police Station mostly in
my superior officer.

Investigation Section PCINSP RODERICK DEAN CARZA re- Insubordination
(Failed to report to the office of DDA after he was caught unshaved on December 2,
2017 at Gate 2 of MPD HQ).
3. That, the above reference the undersigned has no intention to defy nor violate any legal
order of this office. Hence, that on the above dated I was assigned at DSHU and rendering
my duty as a duty sentinel at that time in a single post and I was ordering to report at the
