General
1. The device should be used only by skilled and experienced professionals.

2. When installing the device, make sure of the following points.


1) Install in a dry place where the device can not be splashed with water.
2) Install in a place where the device can not be affected with atmospheric pressure, temperature, humidity,
ventilation, sunlight, dust, salt, the air containing sulfur, etc., and others.
3) Always make sure the device is placed stably. Avoid inclined plane, vibration, and impact (also during
transfer).
4) Do not install the device where any chemical agents are kept or any gases are produced.
5) Note the frequency, voltage, and allowable current (or power consumption) of the power source.
6) Ground the device properly.

3. Prior to use the device, make sure of the following points.


1) Check the switch contacts, polarity, dial setting, meter, etc. and make sure the device functions accurately.
2) Check that the device is securely grounded.
3) Check that all the cords are connected correctly and firmly.
4) Be cautious of using other devices concomitantly as accurate diagnosis may be impeded or hazard may be
induced.
5) Recheck the external circuit that is connected directly to the patient.

4. While using the device, make sure of the following points.


1) Always watch out for any abnormality in the device system and the patient.
2) When any abnormality is found with the device and the patient, take appropriate measure such as pressing
the [Stop] switch after making sure that there is no hazard to patient by halting the device operation.
3) Prevent the patient from touching the device.

5. After each use of the device, make sure of the following points.
1) Turn off the device after putting the operational switch and dial, etc. back to the initial state using the specified
procedures.
2) When removing the cord, do not apply excessive force. Always hold the plug to remove the cord.
3) When storing the device, make sure of the following points.
[1] Store the device in a dry place where it can not be splashed with water.
[2] Store the device in a place where it can not be affected with atmospheric pressure, temperature, humidity,
ventilation, sunlight, dust, the air containing salt, sulfur, etc., and others.
[3] Always make sure the device is placed stably. Avoid inclined plane, vibration, and impact (also during
transfer).
[4] Do not store the device where any chemical agents are kept or any gases are produced.
4) Clean the accessories, cords, and cables after each use and keep them organized in one place.
5) Always clean the device after each use so that it can be readily used next time.

6. When the device malfunctions, do not try to repair it. Put a proper notice on the device and ask the service
personnel for repair.

7. Do not modify the device.

8. Maintenance and inspection


1) Periodically check the device and its parts.
2) When using the device first time after it has not been used for a long time, check that the device functions
normally and safely prior to use.

SAFETY WARNINGS/CAUTIONS
For safe operation, be sure to comply with the following warnings and cautions.
The following labels appear at places in this manual to indicate the magnitude of risk and
damage that may occur in the event that these warnings and precautions are not observed.
This label preceding precautions indicates that

Warnings
1. Confirm that the flange of the syringe barrel is set in the slit firmly and the flange of the syringe
plunger is held securely by the syringe clamp.
(If the above warnings are not observed, not only the various alarm functions but also the flow rate
accuracy can not be guaranteed.)
2. When starting infusion, confirm infusing state and the site of venipuncture. Do not rely on the alarm
function only. Regularly confirm that the remaining amount in the syringe and the set flow rate are
consistent.
3. When the infusion line is occluded, take appropriate corrective actions such as clamping the
infusion line to prevent over infusion
4. Use this pump at a place sufficiently distant from high-frequency wave generating devices such as
medical electric knife, a mobile telephone, radio apparatus to avoid malfunction caused by radio-
wave interference. Connect the pump to a power outlet different from these devices and ground the
pump appropriately.
5. If the pump is dropped onto the floor, or given an impact, immediately stop using it.
(The pump needs service even if it appears normal externally and no abnormal operations are
observed as the inside may be damaged. Contact our sales representative in charge.

Contraindication / Prohibition
1. This pump is specially designed for disposable syringes. Therefore, avoid using any syringe once used.
2. Use only the specified syringes for the pump.
(If a syringe other than the specified is used, flow rate accuracy and alarm functions can not be
guaranteed. Moreover, a medical accident may result.)
3. Do not use the pump in the controlled area of radiological equipment/MRI and in the hyperbaric
oxygen therapy room. Do not use the pump placed outside the hyperbaric oxygen therapy room but
with the infusion line put inside.
(This pump has not been designed with the use in these environments taken into account. Using this
pump under these environments may cause the pump to malfunction, break, or induce explosion)
4. Do not use this pump in a flammable environment.
(The pump may induce fire or explosion.)
5. Do not connect this pump to a circuit in which an excessive negative pressure may arise.
(The syringe plunger may come off the slider of the pump causing the solution to be infused rapidly.)
6. Do not use this pump concomitantly with gravity infusion.
(The pump may not operate according to the specifications: Air bubbles may be formed at the
connection point of the infusion lines downstream to the pump or an alarm state downstream to the
connection site can not be detected.)

SAFETY WARNINGS/CAUTIONS
Precautions for use in relation to the operational procedures
1. Be sure to prime the line before starting the infusion.
(This expels the air out of the tube, eliminates the gap between the flange of the syringe barrel and
the slit, and starts infusion as soon as the [Start] switch is pressed.)
2. Do not remove the syringe without closing the infusion line.
3. Be sure to connect an external device that operates normally when using the external
communication function of the pump. The pump should not be infusing while it is connected to
an external device.
4. Do not use this product and other infusion systems at the same time.
(If other infusion systems are used concomitantly, the pump may not operate according to the
specifications.)
5. When using the pump for microinfusion, be careful of the kink in the infusion line.
(The time to occlusion detection takes longer as the flow rate is set lower. The solution may not be
infused for a long time if kink occurs.)
6. The solution should have reached the room temperature prior to infusion.
(Infusing cold solution may raise the friction resistance of the syringe plunger causing the occlusion
alarm to occur frequently.)
7. When using a prefilled syringe, adjust the setting to the prefilled syringe used.
(The pump may not operate according to the specifications if a prefilled syringe is used without the
setting changed.)
8. If any abnormality is found, contact our sales representative in charge.

Precautions for use


1. Prior to using the pump at the first time after purchase, connect it to AC mains and charge it at least 15 hours.
2. Check that the buzzer sounds and all the lamps blink when the [Power] switch is turned on.
3. If “ ”~“ ” appears on the [Flow rate/Total Infusion Volume] display and the alarm
sounds while self-check is being performed at the power-on or during infusion, the system may be
malfunctioning. Stop using the pump immediately.
4. Install the pump in a place where there is no possibility of the solution splashed onto the AC mains
connector, which may cause short-circuiting.
5. The difference in height between the pump and the patient should be minimized.
6. The internal battery self-discharges even if not used. Charge the internal battery at least 15 hours
when using the pump at first time after it has not been used for a long time.
(It is recommended that the internal battery should be discharged and charged once in a month.)
7. Before using this pump concurrently with an electric cautery, note the following precautions.
1) The extent of high-frequency noise varies for each model of electric cautery. Bigger noises come
from older type of electric cautery (Vacuum tube and gap system). Such model of electric
cautery should be avoided.
2) Provide the sufficient distance between the cord (cautery holder, cautery cord and cord of
counter electrode) or main unit of electric cautery and this pump.
3) Plug the electric cautery and this pump to different power outlets.
8. Turn off the power of this pump and store the slider after each use.
9. Before cleaning the pump, note the following precautions.
1) Be sure to turn off the power, and unplug the power cord.
2) Do not immerse and clean the pump in the water or other solutions.
(Ingress of solution into the pump may cause short-circuit of the electric circuit.)
3) Do not autoclave or EOG sterilize the pump.
4) Do not use alcohol, thinner or any other organic solvent.

Product Overview

Nipro Smartfusion is a medical syringe pump designed for precise continuous infusion of
vasodilatation and vasopressor drugs, analgesics, anti-cancer drugs, anti-coagulants, nutrition for
premature and new born babies, various hormone drugs, and the like with microcomputer control.
The pump operates on AC 100V – 240V mains or internal battery and incorporates a sensor that
detects the size of the syringe loaded. The infusion speed is adjusted and purge is performed as the
following: the rotation speed of the stepping motor is pulse-controlled, then, the rotating motion is
converted into the linear motion to move the slider that pushes the syringe plunger. Abnormality
detection sensors such as syringe loading in-completion, occlusion, and nearly-empty detection
sensors are incorporated as safety devices.

Features

• Small and compact. Easy for carrying and installing multi pumps. Can be stored in a small space.
• Compatible with 10, 20, 30, and 50(60) mL disposable syringes.
• Constantly watches for the syringe plunger disengagement.
• The occlusion detection pressure can be switched to three levels (L, M, and H).
• Flow rate can be set to a desired value in the wide range (0.1-300.0 mL/h).
• Can be operated on either of the three power sources: AC mains, internal battery or Pump Uniter.
• Can be continuously operated on internal battery for approximately 2 hours.
(New internal battery, ambient temperature of 25°C, after charged 15 hours with the power off, flow
rate of 5.0 mL/h)
• Four levels of lamp indication show the amount and remaining charge of the internal battery.
• Disposable syringes from Nipro, Terumo, JMS, and TOP and other brands can be used by changing
the default setting.
• Large pilot lamp with good visibility is incorporated.
• The set flow rate is shown in large figures. The figures below the decimal points are shown smaller.
This prevents the flow rate setting error. The figures below the decimal points are shown smaller and
the color is different.
• The upper part of the operation panel is allocated to the display and the lower part to the operational
section for ease of use.
• The syringe clamp is easy to use: the user just need to pull it up.
• The history data can be exported to PC using the external communication function.
• The alarm sound volume can be switched to three levels.
• The timing for nearly-empty alarm can be switched to three levels.

Operational Procedures
1 Preparation

Before using this pump, read P. 2, “General” and


P. 4-5, “SAFETY WARNINGS/CAUTIONS.”

* When using this device for the first time,


connect it to the AC power source and charge
the internal battery at least 15 hours. Mounting Screw

securely to the infusion stand. To attach the
pump to the clamp, insert the mounting screw

Caution
Confirm the stability of the infusion stand.

1. Connect the furnished AC power cord to the AC


power inlet on the left side of the pump. (Fig. 2)
2. Plug the power cord into power outlet with a
proper grounding terminal.

2 Turning the power on

1 Make sure that the syringe is not loaded. Then,


press the [Power] switch to turn the power on.

2. Discontinue the use immediately if error

[Flow Rate/Total Infusion Volume] display and

Warnings
· Confirm that the brand of the syringe to be used
corresponds to the setting in the pump. If they do not
match, alarm functions can not be guaranteed as
flow rate error exceeding the tolerance may arise.
· This pump is for disposable syringes only. Do not
reuse a syringe that has been used once.
· Stop operation immediately if any abnormality is detected.

4. The set occlusion detection level appears on the


[Flow Rate/Total Infusion Volume] display as
“ ” for about 2 seconds.

3 Opening the “Slider guarding bar”

1. Open the “Slider guarding bar” and press in the


“Slider guarding bar lock.” (Fig. 6)

Caution
If the “Guarding bar lock” is not pushed all the
way in and locked correctly, the flow rate
accuracy can not be guaranteed since the slider Slider guarding

is unprotected. Moreover, there is a possibility of
damage by the impact.

4 Loading the syringe

1. Connect the syringe filled with solution and the


infusion line aseptically.

Caution
Lure lock type infusion line and syringe are
recommended.

2. While holding the slider lever, move the slider to

If the flange is not properly inserted into the slit,
flow rate accuracy and alarm functions can not
be guaranteed.

4. Push down the “syringe clamp” silently to secure
the syringe.

* Figures indicating the syringe size appear on


the [Flow Rate/Total Infusion Volume] display
(about 2 seconds) and [mL] lamp illuminates.
Confirm that the indications match the syringe
to be used.

5. Hold the slider lever and move the slider forward

6. Release the slider lever to let the slider hook

If the flange of syringe plunger is not securely


held with the slider hook, “siphoning (infusion by
gravity)” or “reverse flow” could result. Flow rate
accuracy can not be guaranteed. Fig. 10

7. While holding down the [Function] switch, the


loaded syringe size set in the pump appears on
the [Flow Rate/Total Infusion Volume] display.

5 Setting the flow rate

1. Confirm that the [Flow Rate] and [mL/h] lamps


are on and flow rate is on the [Flow Rate/Total
Infusion Volume] display. (Fig. 12) [Flow Rate] lamp [Flow Rate Setting]

2. Confirm that the [Flow Rate Setting] mode lamp
is on, which indicates that the flow rate can be
entered. (Fig. 12)
3. Press the [Flow Rate Setting] switches and set
the flow rate per an hour (mL/h). (Fig. 12)

6 Priming

Warning
Perform this procedure before inserting I.V.
catheter into the patent.

Caution
The priming volume is added to the Total
Infusion Volume

1. Confirm that the [Syringe load In-completion] alarm


lamp is off and the syringe is loaded properly.
Confirm that no alarm is generated.

* Ifswitch
any alarm is generated, press the [Mute]
to cancel the alarm. Then, eliminate
the cause of the alarm.

2. Pressing the [Purge] switch starts the priming [Purge] switch
and the total infusion volume during the priming
is shown on the [Flow Rate/Total Infusion Fig. 13

the [Pilot] lamp blinks rapidly in green, and the

Warning
Prime the line before starting infusion. Unless Slit
the line is primed, the air stays in the tube; the
gap exists between the flange of the syringe
barrel and the slit, which cause the delay of
infusion when the [Start] switch is pressed. (Fig. 14) Fig. 14

7 Clearing the Total Infusion Volume

To clear the volume added to the total infusion


volume during priming, perform the following [Total Infusion Volume] lamp

procedures. If you do not need to clear this volume,


proceed to P. 14, “8.Venipuncture.”

1. Press the [Shift] switch and confirm that total


infusion volume appears on the [Flow Rate/Total
Infusion Volume] display. (Fig. 15)

2. While the total infusion volume is on the display


(about 15 seconds), keep holding down the
[Clear] switch (about 1 second) to clear the total
infusion volume. (Fig. 16)

Total Infusion Volume] display.

9 Starting the infusion

1. Confirm that the flow rate is set, the syringe is


loaded securely, and the alarm is not generated. [Infusing] mode lamp

13 Turning off the power

1. Keep pressing down the [Power] switch (about 4


seconds) to turn off the power. The buzzer
sounds 3 times every 1 second while the switch
is kept pressed until the power is turned off.

• This pump has not been designed to detect any abnormality in infusion such as “over-infusion” or “under
infusion.” During infusion, do not rely solely on the alarm functions and regularly check that the remaining
amount in the syringe and the set flow rate are consistent.
• Confirm that the flange of the syringe barrel is set properly in the slit and that the flange of the syringe
plunger is secured firmly by the slider hook. If the flanges are dislocated, gravity infusion or reverse flow may
occur. Also, it is recommended to minimize the difference in height between the pump and the patient
connection site.
• When the infusion line is occluded, the internal line pressure is increased. Eliminate the cause of occlusion
only after appropriate measures have been taken to prevent over-infusion such as clamping the infusion line.

If the alarm sounds during infusion

If the [Pilot] lamp illuminates in red and the


[Syringe Load In-completion] alarm lamp turns [Pilot] lamp
on with the alarm sound, the syringe is not
loaded properly.
The infusion has been stopped automatically for
safety. Press the [Mute] switch to cancel the alarm
sound. Load the syringe properly referring to P. 11,
“4. Loading the syringe.” (Fig. 27)

If the [Nearly Empty] alarm lamp illuminates and


the alarm sounds intermittently, the remaining

If the [Pilot] lamp illuminates in red and the


[Occlusion] alarm lamp turns on with the alarm [Pilot] lamp
sound, the infusion line is occluded and the line
pressure has exceeded the occlusion detection
pressure.

automatically for safety. Press the [Mute] switch to
cancel the alarm sound. Since the infusion line
pressure is increased, close the line to prevent over-
infusion at the release of occlusion. Hold the slider

Replacing the syringe to continue infusion
1. Press the [Stop] switch and close the infusion line. (Fig. 31)
2. After confirming that the brand of the replacing syringe
corresponds to the setting in the pump, load the syringe.
3. After switching the infusion line using a three way
stopcock or equivalent to prevent the solution from
being delivered to the patient, perform priming
referring to P. 13, “6. Priming.”

primed, the air stays in the tube; the gap exists between

4. Clear the total infusion volume if necessary. (Refer

5. Open the infusion line and press the [Start] switch
to commence infusion.
Operating on the internal battery
When the supply of AC power is interrupted or the AC
voltage is decreased during infusion, the pump will be
automatically switched to internal battery operation.
The internal battery powers the pump for about 2
hours (in case of a new internal battery, ambient
temperature of 25°C, after charge of at least 15 hours
with the power turned off, and flow rate of 5.0 mL/h)

Cleaning

If the fluid splashes on the pump during or after operation, wipe the fluid immediately with a cloth. Also,
clean the pump with a wet cloth on a regular basis.

• Power off and remove the AC power code before cleaning.

• Do not sterilize or autoclave the pump.
• Do not use thinner or alcohol for cleaning.

Locate the following devices and materials before check-up (excluding the check-up before each use).

2. Pump can be operated on internal battery.
3. [AC Connect] lamp illuminates when the AC power cord is connected to the pump without turning on the
