Ix. Antigliadin antibodies. IgG sensitive, IgA specific. IgA endomysial a/b. Tiss ue Transglutaminase a/b.jujunal biopsy.
Gluten free diet.Rice potato and maize shoulde be used instead wheat, joe, rye.
Vitamin suppliments.
Clinical response in 6 weeks. In Refactory Spure Give Corticosteroids.

Diverticular Disease: (Old age, Pain in Left Iliac fossa, ass e constipation/Diarrhea )
Remember Acute Diverticulitis = Sever left iliac fossa pain + Fever and constipation + Tenderness on left side of abdomen.
NPO, Bed rest, IV Fluids.

Irratible Bowel Syndrome:

Post Exposure Vaccination in Nonimmunized People within 72 h, and Immune serum globulins in pregnant women within 6 days of exposure.

Dengue Fever: (Sudden fever e chills, Sever aching, breakbone, sore thorat, headache, dizziness, Maculo-Papula RASH, first appears on
dorsum of hands feets then spreads to legs, trunk,and neck. Face spraed. Bleeding tendency. O/E Myalgia, Periorbital edema, hypotension,
Tachycardia.)

Cat.2:Relapse, Treatment Failure, Dafault Treatment. (2 months ATT free)
Cat.3:Sputum -ive with limited parenchymal involvement
Cat.4:Extrapulmonary TB, Chronic Cases

Drugs:
Isoniazid (5mg/KG, Max 300mg) 50mg/5ml.
Rifampicin (10mg/KG, Max 600mg) 100mg/5ml.
Ethambutol (15-25mg/KG, Max 2.5g) 220mg/5ml.
Pyrazinamide (15-30mg/Kg, Max 2g) 250mg/5ml.
Streptomycin (15mg/Kg, Max 1g)

Category 1:
Initial Intensive Phase (2Months) 4 Drugs: Isoniazid plus Rifampicin plus Ethambutol plus Pyrazinamide.
Check Sputum after 2 Month:
If Sputum is Negitive Continue 4 drugs for additional 1 month and 2 drugs for 5 months.
If Sputum is Positive then Continue Isoniazid plus Rifampicin for 6 Months
Continuous Phase (6Months) 2 Drugs: Isoniazid plus Rifampicin,

Category 2:
Initial Intensive Phase (2Months) 4 Drugs: Isoniazid plus Rifampicin plus Ethambutol plus Pyrazinamide plus
Streptomycin.
One More Month: Isoniazid plus Rifampicin plus Ethambutol plus Pyrazinamide.
Check Sputum after 3 Month:
If Smear is Positive: Give 5 drugs for one month more
If Smear is Negitive: Continue Isoniazid plus Rifampicin plus Ethambutol for 5 Months.
Check Sputum after 4 Month:
Contine Isoniazid plus Rifampicin plus Ethambutol for 4 Months whether the Smear is positive or Negitive. Send C/S
Sputum.
Check Sputum after 8 Month:
Labelled it a chronic case and Refer to Chest Specialist.

Pulmonary TB in Childrens:
Most cases of Pulm. TB in Children are sputum Negitive and Falls in category 3.

Category 3:
Isoniazid plus Rifampicin plus Pyrazinamide for first 2 Months.
Isoniazid plus Rifampicin continue for next four months.
Without Pyrazinamide, Isoniazid plus Rifampicin MUST be given for nine months.

ICO Positive sputum Smear, add Ethembutol or Streptomycin for first two month.

Half of Bicarbonate Deficit will be given in first 8 h and next half in 24 hours.

Intranasal corticoids:

Anticholinergic Drugs:
Ipratropium bromide inhaler are 1st DOC.
Theophilline:

medical Tx is not effective.

Antitussive:
Codeine and Dextramethorphan can be given in all trimesters of pregnancy.

Remember:Penicillins, Cephalosporins,Erythromycin, Nitrofurantain are OTC drugs in
Pregnancy.

B- Lactam Antibiotic.
1st DOC at usual doses.

Macrolids:
Erythromycin 1st DOC.

Lincomycin and clindamycin:
