SUBJECT : REITERATION ON PROCESS FLOW ON FILING OF FORM 6

1. This is to reiterate the process flow on filing of form 6 for teaching, non-teaching and teaching-related
personnel in this Schools Division Office (SDO).

2. Omnibus Rules on Leave Sec. 51 states that, “All applications for vacation leave of absence for one (1)
full day or more shall be submitted on the prescribed form for action by the proper head of agency five
(5) days in advance, whenever possible of the effective date of such leave”. Moreover, Omnibus Rules
on Leave Sec. 53 states that, “All applications for sick leave of absence for one full day or more shall
be made on the prescribed form and shall be filed immediately upon employee’s return from such leave.
Notice of absence, however, should be sent to the immediate supervisor and/or to the agency head.
Application for sick leave in excess of five (5) successive days shall be accompanied by a proper medical
certificate”.

3. In order to have smooth process flow in this SDO, the following must be strictly observed:
3.1. Submitted form 6, must be duly filled-up. All necessary information must be indicated which
shall be checked thoroughly by the Administrative Assistant In-charge (ADAS II/ III)
3.2. Duly filled-up form 6 from the Districts (Elementary)/ Secondary Schools, must be submitted
by the ADAS-in-charge to the HR/ Personnel Section weekly thru the Records Section.
3.3. Form 6 shall not be submitted in bulk at the end of month, together with the DTRs. As soon
as, forms with attachment (when necessary) are duly filled up, it shall be submitted by the
ADAS-in-charge right away to the Personnel Section thru Records Section for ASDS/SDS
approval prior to employee’s scheduled leave
3.4. All Elementary teachers who will apply for leave must submit their documents to the ADAS-
in-charge in their respective District as soon as completed. Thereafter, the ADAS-in-charge
logs the document for it to be included in the record system in the District before submitting
to SDO
