• Walker
3. The purpose of the nursing action performed:
• Helps maintain balance
• Avoid the risk of injury when walking
• Reducing the negative impact of immobility
4. Steps of nursing actions performed:
a. Pre-Interaction Stage
1) Verify data.
2) Wash hands
b. Orientation Stage
1) Giving greetings as a therapeutic approach
2) Explain the purpose and procedure of action for the family / patient
3) Asking the client's readiness before the activity is carried out
4) Keep privacy
c. Working Stage
1) Measure the length and width of the walker according to the client's
hand grip
2) Explain to the client how to walk using the walker
3) Help the client stand up
4) Make sure the client is in balance before mobilizing
5) Use the strongest foot as a support
6) Ask the client to hold the walker's handle
7) Ask the client to walk forward using the help of a walker, while
maintaining 4 walker points on the floor
8) Swing the walke forward as far as 15 cm parallel to the strongest leg
and follow with the weak leg
9) Make sure the client lifts his legs while walking, not interesting
10) Do it repeatedly gradually
11) Review each client's progress, and make corrections if necessary
12) Observation of vital signs after movement training
d. Termination Stage
1) Evaluating the client's response to the procedure.
2) Farewell to clients
3) Clean up the tools
4) Wash hands
5) Record the date and time of examination.
6) Record client response after the inspection procedure.
