ANALYSIS OF NURSING ACTIONS (DOPS)

3. The purpose of the nursing action performed:
• Training the activities of all joints of the body so that the joints are not
stiff and there is no injury or accident when the body is moved
• Increase or maintain muscle flexibility and strength
• Prevent stiffness in the joints
• Prevent. Deformities, stiffness and contractures
• Stimulates blood circulation
• Maintain heart and respiratory function
4. Steps of nursing actions performed:
a. Pre-Interaction Phase
1) Verify data.
2) Wash hands
b. Orientation Stage
1) Giving greetings as a therapeutic approach
2) Explain the purpose and procedure of action for the family / patient
3) Asking the client's readiness before the activity is carried out
4) Keep privacy
b. Working Phase
1) Wash hands.
2) Take care of clothes that cause movement barriers
3) Lift the blanket as needed
4) Encourage the client to lie in a comfortable position
5) Do the exercise as follows:
a) Shoulder joint training
• The patient is in a supine position
• One hand of the nurse supports and holds the elbow, the other
hand holding the wrist.
• Align the patient's elbow, moving the patient's arm away from
his body towards the nurse (Abduction).
• Then move the patient's arm close to his body (Adduction).
• Move the forearm down until it touches the bed, palms facing
down (internal rotation).
• Lower and return to its original position with your elbows
straight.
• Move your forearm back until it touches the bed, palms facing
up (external rotation).
• Lower and return to its original position with your elbows
straight.
• Avoid excessive stretching of the shoulder.
• Repeat 10 times or according to tolerance
b) Elbow joint exercises
• The patient is in a supine position
• The nurse holds the patient's wrist with one hand, the other
hand holds the upper arm
• Position the patient's hand supinasi, then do a bend (flexion)
and straighten (extension) the elbow.
• Instruct the patient to stay relaxed
• Make sure the given movement is in the right midline
• Note the range of motion of the joints formed, whether they
are within a normal or limited range.
• Repeat 10 times
c) Arm exercises
• The patient is in a supine position
• The nurse holds the patient's elbow area with one hand, the
other hand holds the patient's hand outward (supine /
supination) and inwardly (face down / pronated).
• Instruct the patient to stay relaxed
• Repeat 10 times
d) Wrist joint exercises
• The patient is in a supine position
• The nurse holds the patient's forearm with one hand, the other
hand holds the patient's wrist, and bending the patient's wrist
up and down
• Instruct the patient to stay relaxed
• Repeat 10 times
e) Exercise joint fingers
• The patient is in a supine position
• The nurse holds the patient's wrist with one hand, the other
hand helps the patient make a fist / bend the fingers and then
straightens the patient's fingers.
• The nurse holds the patient's palm and four fingers with one
hand, the other hand rotates the thumb.
• The nurse's hand helps widen the patient's fingers then close
again.
• Instruct the patient to stay relaxed
• Repeat 10 times
f) Groin joint exercises
• The patient is in a supine position
• Place one nurse's hand under the patient's knee and one hand
on the heel.
• Keep the patient's foot position straight, lift your legs about 8
cm from the bed, move your feet away from the patient's body
• Move your feet closer to the patient's body
• Back to its original position
• Then place one nurse's hand on the ankle and the other hand
on the knee.
• Turn your feet away from the nurse.
• Turn your feet towards the nurse
• Back to its original position
• Avoid excessive lifting of the feet.
• Repeat 10 times or according to tolerance
g) Knee joint exercises
• The patient is in a supine position
• One hand care under the patient's knee and hold the patient's
heel with the other hand
• Lift the legs, bend at the knees and groin.
• Continue bending the knees towards the chest as far as
possible
• To bring your legs down and straighten your knees by lifting
your legs up
• Instruct the patient to stay relaxed
• Make sure the given movement is in the right midline
• Note the range of motion of the joints formed, whether they
are within a normal or limited range.
• Repeat 10 times
h) Ankle joint training
• The patient is in a supine position
• The nurse holds the upper half of the patient's foot with one
finger and holds the ankle with the other hand.
• Turn the foot inward so that the sole of the foot is facing the
other leg (infersion)
• Return to the original position
• Turn the foot out so that the sole of the foot is away from the
other foot (efficiency)
• Return to the original position
• Then place one nurse's hand on the patient's foot and one hand
on the ankle. Keep your feet straight and relaxed.
• Bend your ankles, point your toes towards the patient's chest
(flexion dorso).
• Return to the original position
• Bend the ankle away from the patient's chest (plantar flexion)
• Return to the original position
• Instruct the patient to stay relaxed
• Repeat 10 times
i) Toe joint exercises
• The patient is in a supine position
• The nurse holds the patient's ankle with one hand, the other
hand helps the patient make a motion to bend the toes and
then straightens the patient's toes.
• The nurse's hand helps widen the patient's toes and then close
again.
• Instruct the patient to stay relaxed
• Repeat 10 times
6) Assess the effects / effects of exercise on clients, especially
hemodynamic clients
7) Arrange clients in a comfortable position
8) Fix blankets and linen
c. Termination Stage
1) Evaluating the client's response to the procedure.
2) Goodbye to clients
3) Clean up the tools
4) Wash hands
5) Record the date and time of examination.
6) Record client response after the inspection procedure.
