4. Steps of nursing actions performed:
a. Pre-Interaction Stage
1) Verify data.
2) Wash hands
b. Orientation Stage
1) Giving greetings as a therapeutic approach
2) Explain the purpose and procedure of action for the family / patient
3) Asking the client's readiness before the activity is carried out
4) Keep privacy
c. Working Stage
1) Wash hands
2) Maintain patient privacy
3) Ask the patient to put one hand on the chest and one hand on the
abdomen
4) Train the patient to do abdominal breathing (deep breathing through
the nose for 3 counts, keep the mouth closed)
5) Asks the patient to feel the abdomen expand (prevent curvature of the
back)
6) Asks the patient to hold their breath for 3 counts
7) Requests to breathe slowly in 3 counts (by mouth, lips like blowing)
8) Asks the patient to feel the abdominal deflation and contraction of the
muscles
9) Tidy up the patient

1) Evaluating the client's response to the procedure.
2) Farewell to clients
3) Clean up the tools
4) Wash hands
5) Record the date and time of examination.
6) Record client response after the inspection procedure.
