HISTORY
Each area has their own GOALS AND OBJECTIVES:

OUTPATIENT CONSULTATION
To diagnose and threat and give appropriate medical services.

Social Hygiene Clinic/Laboratory

To minimize and be STI/AIDS/HIV free city to eliminate stigma and discriminate
into the society.
To raise awareness of STI/AIDS/HIV within the society.
To reduce and totally eradicate STI/AIDS/HIV related stigma and discrimination.
To disseminate information thru society that they are free to come in the social
hygiene clinic for more information.

Immunization/Maternal Care Services

To immunize children 0 to 11 months old from 7 immunization diseases. To
provide comprehensive maternal care program for pregnant and lactating
mothers.

Availing 4 Molecules (Package of Essential Non-Com Drugs)

To identify target clients and conduct risk assessment using PhilPEN protocol
perform appropriate management based on patient risks. Create a supportive
environment that promotes health lifestyle through policy and program advocacy
plan and work towards sustainability of PhilPEN implementation

Dental
To advance health promotion primary care and quality oral health care.
To value patients and respect clients’ rights to unbiased informal, confidentiality
and quality dental care.
To maintain teamwork among staff through seminars to promote unity and good
working environment.
 Birthing Facility
To provide exceptional client centers pregnancy and birthing care.
To promote safe, effective satisfying, evidence base maternity care. To prevent
maternal and infant mortality rate.
To provide information, education and support in a competent and dignified
manner.
To be recognized and support breastfeeding for all infants and established a baby
friendly facility.
To strengthen commitments to patient rights to unbiased information,
confidentiality and quality care

MISSION
The City Health Office of Urdaneta through its resources will make available and
socially acceptable health care to all levels of the society especially to the poor.
We shall continually maintain in the delivery of a quality patient care through an
effective networking with the LGU and DOH.

VISION
Better quality of life for the people of Urdaneta. Living in a Healthy environment
with healthy lifestyle. Quality care service from the public health system of the
LGU.
