Greetings of peace!

As stated in our Philippine Constitution, Article XIII “National and Local Government agencies
are directed to implement programs and services to ensure the full participation of PWDs in
all aspects of community life thru Republic Act 7277, otherwise known as the Magna Carta for
Disabled Persons, and later or amended thru House Bill No. 1214 which provides other
privileges and incentives to PWDs”.

With this regard, we are happy to inform your good office that an organization was already
established to ensure that priority services, rehabilitation, and integration of PWDs as
mainstream of the society is given utmost significance. The Federation of an Active Women
with Disability of Batangas PSWDO is making its meager steps towards these just and noble
goals with the undersigned as its President. However, in order for our organization to fully
realize its functions, Section 2 of the Corporation Code of the Philippines stipulated the need
for us to register both in Securities and Exchange Commission (SEC) and in the Bureau of
Internal Revenue (BIR) under Section 236 Tax Code Revenue Regulations (RR) 7-2012.

Hence our rigorous effort and hard work to raise fund for the organization, the federation
remains in ominous need for fund for us to accomplish the above-mentioned registration
process. With this, in behalf of more than twenty -two thousand persons with disability whose
hope and aspiration holds in this federation, we are humbly soliciting from your good office for
help and support in funding our registration to BIR and SEC. Your act of generosity and
financial support will be of great service for the implementation of programs that will promote
and alleviate the welfare of PWDs in the provision of its basic services.

Attached herewith is our organization’s by-laws, vision and mission and other pertinent
documents for your reference.

It is hoped that this letter will merit your kind consideration and favorable approval.
