Introduction
EN: This document serves to provide a comparative perspective of how each language treats the
subject of nutrition, its audience, discusses its surrounding fields, and inner narrative. As a future
dietitian and bilingual speaker, reviewing other nutrition related literature in a different language
broadens my own insight into other research methods implemented in a contrasting cultural
context and deepens my understanding of the global health field.
FR: Ce document fournit une perspective comparative de la manière dont chaque langue traite le
sujet de la nutrition, de son public et discute de ses domaines. En tant que future diététiste
bilingue, examiner d’autres publications sur la nutrition dans une langue différente me permet
d’approfondir mes connaissances sur d’autres méthodes de recherche mises en œuvre dans un
contexte culturel contrasté et d’approfondir ma compréhension du domaine de la santé mondiale.
Vitamine D
Outre son rôle dans la minéralisation osseuse, le métabolisme phosphocalcique et le maintien de
l’homéostasie calcique, la vitamine D est également impliquée dans la production hormonale, la
modulation de la réponse immunitaire ou le contrôle de la prolifération cellulaire et de la
différenciation. Les études expérimentales ont montré que la vitamine D active a, in vitro, des
effets antitumoraux sur certaines cellules exprimant le récepteur à la vitamine D, en réduisant la
prolifération, en favorisant la différenciation des cellules tumorales et l’apoptose ou en inhibant
l’angiogenèse péritumorale et le développement de certaines métastases chez l’animal [55]. Il
existe ainsi des arguments très forts par des études in vitro d’un effet préventif et de freination du
développement des cancers en général et du sein en particulier. Le risque serait inversement
corrélé au taux circulant de 25OH vit D quelle qu’en soit l’origine, endogène, exogène ou par
supplémentation. Des travaux récents rapportent une diminution du risque de CS associée aux
taux sériques de 25OH vit D, significative dans deux études cas-témoins sur trois [56–58].
Goodwin et al. ont également montré que les patientes carencées en vitamine D ont une survie
spécifique diminuée [59]. Le bénéfice attendu de la correction des carences en vitamine D est
remarquable, ne serait-ce que par le biais de la prévention des complications de l’ostéoporose ou
des douleurs ostéoarticulaires sous AA, même s’il n’existe aucun effet démontré à ce jour sur le
risque de rechute. Rappelons que les biphosphonates ne sont efficaces qu’après correction d’une
carence en vitamine D. À ce titre, l’étude de Prieto recommande de doser en routine le taux de
vitamine D à toutes les patientes atteintes de CS qui reçoivent une AA [60]. Rappelons enfin que
la France est l’un des pays d’Europe où le taux de 25OH vit D est le plus bas [61].

Vitamin D
Apart from its role in bone mineralization, phosphocalcic metabolism and calcium homeostasis,
vitamin D is also linked to hormonal production, immune response, cell proliferation and
differentiation. Experimental research has indicated that, in vitro,1 active vitamin D has
antitumor effects on certain vitamin D receptor cells by reducing proliferation, by promoting the
differentiation of tumor cells and apoptosis, or by inhibiting peritumoral angiogenesis and the
development of certain metastases in animals. There exist as well substantial arguments raised by
some in vitro studies showing a preventative effect, as well as suppression of cancer in general,
and breast cancer in particular. The risk is inversely correlated with the circulation rate of 25OH
vitamin D, whether its origin is endogenous, exogenous or by supplementation. Indicated in two
out of three case control studies,2 recent research supports a diminished risk of breast cancer3
associated with 25OH vitamin D serum levels. Goodwin et al. likewise established that vitamin
D deficient patients have a diminished chance of survival. Expected benefits of correcting
vitamin D deficiency are remarkable, if only as a means of preventing complications associated
with osteoporosis or osteoarticular pain with aromatase inhibitors (AI),4 even though there is no
evidence, to date, on the risk of relapse. Recall that bisphosphonates are only effective after a
vitamin D deficiency has been corrected.5 For this reason, the Prieto study recommends routine
doses of vitamin D to all breast cancer patients who receive aromatase. Lastly, let us remember
that France is one of the European countries with the lowest rate of 25OH vitamin D.
