good standing and report on pendency and non-pendency of any administrative and
disbarment case.

The certification/s will be used in order to comply with the requirements for my
application for Commission as Notarial Public in the Province of Negros Oriental.

Along with this letter-request, I have attached the following:

1.) A scanned copy of my Certificate of Membership in the Philippine Bar bearing

Roll No. 70642;

2.) IBP Receipt No. 043174; and

3.) BDO Electronic Receipt as proof of payment of the amount of P200.00 for the
certification/s.

I have also prepared a pre-paid LBC pouch for your convenience in sending the said
certification/s.
