Scenario: Ian Miller, a visitor to your workplace tripped over a waste bin that was placed near an exit point
from the outpatient clinic to keep the door jammed open. Injuries sustained include:

Mr. Ian, a visitor, tripped over a trash bin which was used to keep a slammed door open in level 1, general

Staffs notified the incidence, took him to the emergency department in a wheelchair. He was examined and
confirmed to have injury to their right knee causing pain and minor superficial laceration to his left elbow. Vital

Scenario
You have just been appointed as health and safety representative (HSR) in your workplace. It is apparent
after reviewing the current legislation and regulations that the organisation is not compliant. After further
analysis of the situation you have determined that the following issues need to be addressed urgently:
1. Workers are unsure of the organisations current WHS policies and procedures and of the health and
safety legislation, regulations and codes of practice that relate to them.
2. They have not had any training or consultation about WHS for at least two years.
3. Workers are unsure of who to go to, or what to do in the event of an issue in the workplace, if they
raise an issue, nothing seems to happen.
4. There are is nothing in place to inform workers of hazards that have been identified in the workplace,
how risk assessment is conducted, the controls used or how the outcomes of risk assessment are
communicated and reported on.
5. There is no support provided to work teams to encourage them to demonstrate safe housekeeping
practices or get involved WHS procedures.
7. A workplace hazard assessment has not been conducted for six months. There were three hazards
that were identified in the last assessment that have not been actioned
8. The management team is unsure of what health and safety records must be kept.
