Preliminary

Humans are social beings who live and run all activities as individuals in social groups,
communities, organizations and communities. In everyday life, every human interacts with each
other. Therefore, humans cannot avoid an actioncalled communic. Communication is an
interpersonal interaction that uses a system of linguistic symbols, such as a verbal system (words),
verbal and non verbal. This system can be socialized directly / face to face or through other media
(writing, oral and visual). Realized or no, communication is part of human life itself.ation.

on the other hand, to build a familiar sense of humanity, mutual understanding is needed
among fellow community members. in this case communication plays an important role, especially
for modern humans. Modern humans are human beings whose ways of thinking are based on logic,
rationality, or reasoning in carrying out all their activities.the whole activity will be carried out well
through interpersonal communication. the success or failure of communication is when we know
and learn the elements contained in the communication process (Nugroho, 2009: 12)

communication is a very special and meaningful process in human relations. in the nursing
profession for Abdalati (1989) communication becomes more meaningful because it is the main
method in implementing the nursing process. science experience to help others requires special
abilities and great social care. for that according to jhonson (1989), nurses need special abilities and
social care that include intellectual, technical and interpersonal skills reflected in caring or love / love
behavior in communication with others.nurses who have therapeutic communication skills will
not only be easy to establish a relationship of trust with clients, prevent the occurrence of
legal problems, provide professional satisfaction in nursing services and improve the image
of the nursing profession and the image of the hospital, but the most important is to practice
their knowledge fellow human beings (purba, 2003:1)

The meaning of communication

the notion of ethics (etymology), derived from the Greek language, namely ethos, which
means character of decency or custom (custom). ethics is usually related to moral words which are
Latin terms, namely mos and in the plural form mores which also means the customs or way of life
of a person by doing good deeds and avoiding bad things. communication comes from Greek words,
namely communicare which intends to make something that belongs together where the delivery
conveys something to the listener's massage, the listener also acts by giving an appropriate reply. for
the mufti (2015) that talking, listening watching, reading, writing, praying, evaluating themselves and
so on and also are communication activities. according to haffied cangara that communication is a
form of interaction between humans that influence each other influences each other, intentionally
or not intentionally (1998: 19). however, the form of communication is not limited to
communication that uses verbal language, but also in terms of expression, both in the form of facial
expressions, paintings, art and technology.
Islamic communication
in Islamic perspective, communication is an integral part of human life because all of
our moves are always accompanied by communication. communication which is meant is
Islamic communication, namely communication with the character of al-karimah or beretika
which means communication originating from the Qur'an and hadith (sunnah of the
Prophet). in the Qur'an very easily we find concrete examples of how Allah always
communicates with his servant through revelation. to avoid mistakes in receiving messages
through these verses, Allah also gave freedom to the Messenger of Allah to reduce his
revelation through the Matan Hadith. both the hadith are Qauliyah (sayings), fi'liyah
(deeds), Taqrir (approval) of the apostles, then added with the birth of interpreters so that
through their hands there are so many interpretive books. the application of Islamic
communication is contained in the verses of the Qur'an such as QS An-Nahl: 125, QS Al-
Baqarah: 83, QS Ali Imran: 154, Qs An-Naba ': 2-3, Qs Al-Furqan: 63, Qs Fussilat: 33, Qs
Anissa: 154, Qs Al-Anakabuut: 460 and many others. the verses above provide an
affirmation of the essence (essence) of Islamic communication to its implementation stage.

in addition, we find the Prophet Muhammad in communicating with his family,

friends and followers. the Prophet's communication has accumulated in hundreds of
thousands of hadiths which become explanatory amplifiers of the Qur'an and as a guide to
the life of mankind. in the hadith, the principles of communication ethics are found, how the
Prophet Muhammad taught us to communicate. for example, first, qulil haqqa walaukana
murran (say what is right even if it tastes bitter). both falyakul khairan au liyasmut (say if it's
true if you can't, shut up). the third laa takul qabla tafakkur (don't talk before thinking first).
fourth, the prophet advocates good speech as narrated by Ibn Abi Dunya"mention anything
good about your friend who is not present at the meeting, especially the things that you like
about your friend as your best friend conveys your kindness when you are absent". fifth, then
the Prophet (saas) advised, in fact Allah did not like the people .... that is they who
overturned the facts with their tongues like a cow that chewed grass with its tongue". The
prophet's message meant that in communicating should be as good as the facts we see, we
hear, and we experience (fitriyani, 2016 : 1)

therapeutic communication
communication is a process of sending and receiving messages.Communication is the
of sending and receving masages"(burce dkk. 2003:3). through this process, information and
understanding are continued using symbols. Prosea consists of five elements, namely
communicator, message, media, recipient and feedback. "thegeneral process of
communication contains five elements: the comunicator, the massage, the medium, the
receiver, and feedback".

in nursing communication, to eliminate the possibility of conflicting meanings

between nurses and patients, a communication concept developed known as therapeutic
communication. Therapeutic communication is communication carried out by nurses and
other health workers planned and focused on healing patients. therapeutic relationship
between nurses and patients is communication carried out with the aim of improving the
patient's emotions. Therapeutic communication is a joint interaction between nurses and
patients in communication that aims to solve problems faced by patients. in this case

stuart G.W and sundeen S.J (1995) states that therapeutic communication is an
iterpersonal relationship between nurses and clients, in this case nurses and clients obtain
bersma learning experience in order to improve the client's emotional experience. whereas,
S.sundeen (1990) states that a therapeutic relationship is a cooperative relationship
characterized by exchange of behavior.

nursing as follows; communicator, is a nurse with ideas, intentions, information and

a purpose a communicating. massage, the result of the encoding process is the massage. the
purpose of the communicator is experssed in the form of the massage - either verbal or non
verbal. nurses have numerous purposes for communicating, such as to have others
understand their ideas, to understand the ideas of others, to gain acceptance of themselves
or thair ideas, or the produce actions. medium, the medium is the carrier of the massage-
the means by which the message is sent. organizations provide information to members in a
variety of ways, including face-to-face communication. Decoding/ Receiver, for the process
communication to be completed, the massage must be decoded so it's relevan to the
receiver. decoding, a technical term for the receiver's thought processes, involves
interpretation. receivers interpret (decode) the message in light of their own previous
experiences and frames of reference. This undercores the importance of the communicator
being "receiver oriented". Feedback. One-way communication processes do now alow
receiver-to-communicator feedback, in-creasingthe potential for distortion betweenthe
intended massage and the received message. feelings, thoughts and experiences in
developing a therapeutic intimate relationship.

therapeutic relationships between patients and nurses can be identified through

actions taken by nurses and patients that begin with the nurse's actions, patient responses
and goals, and reciprocal transactions to achieve the relationship goals. Therapeutic
communication occurs with the aim of patients helping patients performed by professional
groups through personal approaches based on emotional feelings and based on mutual trust
between the two parties involved in communication.
according to Mahmud Machfoedz there is a difference between social communication and
therapeutic communication, namely;
1. the nurse knows the patient well and understands himself with the values he
adheres to.
2. communication is applied with an attitude of mutual acceptance, mutual trust, and
mutual respect.
3. nurses are able to understand, appreciate the values adopted by patients

4. nurses are aware of the importance of patients' needs, both physically and mentally

5. bidders can create an atmosphere that can motivate patients to change attitudes
and behavior, so they can solve the problems they face.
6. nurses must be able to master their feelings gradually to know and overcome
feelings of sadness, anger and frustration.

7. Able to determine the appropriate deadline and maintain consistency.

8. understand well the meaning of sympathy as the nature of therapeutic action and
not therapeutic.

9. honesty and openness of communication is a basic therapeutic relationship.

10. able to portray capital, in order to be able to show and convince others about health,
so nurses need to maintain a healthy condition physically, physically, socially,
spiritually and lifestyle.

11. nurses must be able to create an atmosphere that allows patients to develop
without fear.

12. Satisfied feeling nurses can help others humanely

13. pay attention to ethics by trying as hard as possible when making decisions based on
the principle of human welfare (2009: 106)

in order to fulfill these characteristics, in communication with patients, nurses make

themselves therapeutically with a variety of communication techniques as optimal as
possible with the aim of changing patients' behavior towards a positive direction. to be able
to implement communication effectively, nurses must have adequate skills and understand
themselves well. nurses must know themselves consciously, in order to be responsible
models. all the behaviors and messages conveyed, both verbally and non-verbally, must be
therapeutic for the class. analysis of familiar therapeutic relationships needs to be done to
evaluate the development of relationships with, and determine the right techniques and
skills at each stage toovercomeclients based on the right place and time

Every nursing activity always begins with communicationbetween nurses and patients
with the aim of establishing relationshipsinterpersonal, so that the nursing process can run
well and effectively. In therapeutic communication, Taylor, Lilis and LeMone (1993) state
that the relationship carried out aims to provide help toovercome the problems faced by
patients. An effective tool in this case is thepersonal nurse. Related insideproviding nursing
care, nurses must be client-oriented. Therefore, nurses must be able to see problemsthe
client is facing from the client's perspective. To be able todoing this nurses must understand
and have the ability to listen actively and attentively.
Teraupetik Communication Analysis
Before communicating, the nurse must firstdo self analysis. This analysis includes self
awareness, value classification, feeling exploration, ability to be a model, and a sense of
responsibility.
a. Self-awareness
Nurse as an instrument in therapeutic communication must be able to know his
personality well. He must be aware who he is.
