AFFIDAVIT OF RECANTATION
AND DESISTANCE

after having been duly sworn to in accordance with law hereby depose and
state:

That I am the complainant’s witness in Criminal Case No.

_________________ for “CONCUBINAGE” filed against my father,
_________________ and _________________;

That after going over my affidavit and my recollection of the events, I

have realized that all of the statements made therein are not true and are
all a misunderstanding. For this reason, I am recanting my affidavit executed

That I am likewise withdrawing/recanting all my statements made in

open court on ______________, ________________, and on other dates as
found in the records of the case;

This affidavit is being executed by me, of my own free will and volition,
for the purpose of withdrawing/recanting my affidavit dated _________________

and on other dates as above-mentioned in connection with the case entitled
“_____________________________” for Concubinage docketed as Criminal
Case No. _________ pending before the Municipal Trial Court, Branch 1,
Meycauayan, Bulacan or any pending claim/action against the foregoing
persons;

That I am further signifying my intention to desist from further

prosecuting my father, _________________ and _________________ for the crime of
concubinage. Further, I am hereby waiving all my rights and claims in the
said criminal case;
