SPECIAL POWER OF ATTORNEY

Prudentialife Education Plan Certificate No. ____________, against
Prudentialife Plans, Inc. whose corporate assets are currently in the
process of liquidation before Philippine Courts;
2. To sign and execute the necessary documents, agreements, or other
legal papers pertaining thereto;
3. To represent, execute, pay the fee/s required, sign any and all documents
and papers, as well as claim and receive for and in my behalf said
documents from the Insurance Commission; and
4. To do any and all acts in order to accomplish or ensure the
implementation of the aforementioned acts and deeds.

My attorney-in-fact hereby accepts this appointment subject to its terms and conditions,
and agrees to act and perform in said fiduciary capacity consistent with my best interest as he, in
his best discretion, deems advisable, and I affirm and ratify all acts so undertaken.
GIVING AND GRANTING unto said attorney-in-fact full power and authority to do and
perform all and every act and thing whatsoever requisite and necessary to be done in and about the
premises, hereby ratifying and confirming all that said attorney-in-fact shall lawfully do or cause
