AFFIDAVIT OF COMPLAINT

I, Jonel, of legal age, a resident of Taguig City, after having been duly
sworn to in accordance with law, do hereby depose and say that:

1. I am a representative from Z bank, a domestic corporation

organized under corporation engaged in the business of
banking and entering into domestic and foreign loan contract.

2. I demand, the bank manager, of legal age, Filipino, to pay the

amount of one hundred thousand (100,000) in relation to payment of a loan
of a valuable client arising under the following facts.

3. Sometime on May 4, 2019 , Madonna a regular client of Z bank

paid her supposedly 36th payment of her loan amounting to 10,000,000
pesos to the bank branch manager of Z bank.

4. After the payment was received by the bank branch manager in his
house, the manager committed Absence without leave and it resulted to the
loan becoming due.

5. The assistant of the bank branch manager stated that the vault
belonging to the manager in the place of their office was empty, thus the
money was in the hands of the AWOL manager.

7. Due to the fact that AWOL Manager is nowhere to be found, and 2

weeks has passed but the manager still did not report to work I was sent as
a representative of Z bank to ask advice of a lawyer and in hope to recover
the money amounting to 100,000 pesos.

8. I am executing this Affidavit of Complaint to attest to the truth of

all the foregoing facts and for the purpose of instituting a criminal complaint
against BRANCH BANK MANAGER for Estafa, if he will continue his non-
appearance without notice and/or such other crimes appropriate under the
premises.
