would like to offer our Comprehensive Pest Abatement Program to Northern Cement
Coporation located at the above address.

SCOPE OF WORK:

Sanitation Spraying:

With the proliferation of diseases brought about by germ-carrying pests such as

cockroaches, mosquitoes and flies, it is very important to keep your establishment
free from these health hazards. An effective way to control this is by sanitation
spraying. We are to sanitize all places of infestation by spraying them with powerful
residual activity insecticides to prevent them from breeding further.

Cracks and Crevices Gel Treatment:

This treatment is specially formulated for German cockroach, which is commonly

found where food is present. This is the most advanced and versatile roach control
system available because they contain a powerful delayed-action insecticide that kills
roaches in their nest. We are to place gel baits on cracks and crevices to effectively
control the infestation.

Misting using Ultra Low Volume (ULV) Treatment:

Mosquitoes are the most efficient vectors of infectious diseases such as cholera,
typhoid, malaria, and dengue fever. Using ULV method of treatment, we will control
the broad spectrum of flying and crawling insects with the droplets dispersed evenly to
contact and kill insects in flight and at rest.

Rodent Control:

Having rats around has all the disadvantages. Rats are carriers of general plagues,
jaundice, typhus, and rabies. Many fires have been attributed to mice chewing on
electrical insulators. We are to remove any possible nesting sites and layout baits and
traps whenever necessary.

In addition to our offer, we will conduct, simultaneously with our servicing, an

inspection of the premises to detect early infestation of termites.

FREQUENCY : ONCE A MONTH

COST PER MONTH : Php 15,000.00/month

MODE OF PAYMENT : 15 days after presentation of Statement of Account
