AFFIDAVIT OF SELF ADJUDICATION

I, , of legal age, Filipino, Single and with residence at 91 NHA Avenue, Langhaya, Brgy.
Dela Paz, Antipolo City, after duly sworn in accordance with law, depose and state:
That-

1. I am the only daughter and sole heir of OLIVAREZ,

vda. de Olivarez, evidenced by my Certificate of Live birth, a copy of
which is attached as ANNEX “A” and made integral part of this Affidavit of
Self- Adjudication;

2. My mother OLIVAREZ, vda. de Olivarez, died

intestate on November 21, 2010 as evidenced by her Certificate
of Death issued by San Juan City Metro Manila, and attached as
ANNEX “B” and made integral part of this Affidavit of Self-Adjudication;

3. At the time of death of my mother, she had no debts, liabilities or obligations

to any persons, agency or institution;

4. That the property my mother left is a condominium unit with a floor Area of
TWENTY ONE point SEVENTY TWO SQUARE METERS (21.72
SQM.) located at Pasig City, Metro Manila covered by
CONDOMINIUM CERTIFICATE of TITLE (CCT) No.PT-17200, more
particularly described as follows:

Floor Location: 3rd

Net Living Floor Area: 21.72 Square Meters

Which unit form part of the “CONDOMINIUM” Project. Attached as

ANNEX “C” is a copy of CCT No. and made integral part of this Affidavit of
Self-Adjudication.

5. Pursuant to Rule 74, Section 1 of the Rules of Court, I hereby adjudicate unto
myself the above described real estate property by means of this Affidavit
and hereby file the same in the Registry of Deeds for Pasig City, with the
request that the said adjudication be registered and made effective without judicial
proceeding as prescribed by the aforementioned provisions of the Rules of Court.
