EXECUTIVE SUMMARY
A Field Report Attachment Submitted For the Award of Basic Certificate in Pharmaceutical
Science

As this report entails the description of the major activities, tasks and skills that I learnt during
my field practical Training which I conducted for about 5 weeks from for the purpose of making
into practice what I have been learning at the institute and acquire new experience from ST.
PHILIPO HEALTH CENTER

Frankly, this report compose historical background of the organization, table of contents, list of
abbreviation, acknowledgment, executive summary, introduction and objective, organization
structure, actual work include description of department and duties undertake, and skills acquired
problem and challenge encountered by the organization, possible solution, conclusion and
recommendation. Expected to practice the theories and concept taught in the class. To see
whether they are mentioned organization. It was a very useful experience to get opportunity to
link theories and practice during my practical training

INTRODUCTION

St Philip health center is located at Tabora municipal at lwanzari district nearby new era secondary school
and kizigo primary school.
St Philip health center is a center which provide enough services to their patients/customer, also there is
enough workers of all sex, there is equally to all workers also there is good buildings and enough security.
Also there is different department like outpatient department and inpatient department.

MEDICAL STORE DEPARTMENT (MSD)


Medical Store Department is the governmental department under ministry of health and social
welfare responsible for storage, distribution and procurement of drugs and medical supplies and
other related products to all governmental and faith based facilities through the country. It
established on 1993 for the aim of maintenance of essential drugs and other medical supplies to
the society.
The purpose of MSD is to make sure that the quality and affordable drugs and other medical
supplies is available in faith based facilities and governmental hospitals frequently

Medical Store Department has divided into two six department which are:

-Sales Department

-Dispatch Department

-Data

-ILS parking

-Receptions Department

-Warehouse department.

From above department I performed to conduct my field practices in four department which are
ware house department, ILS packing, sales department and dispatch department. We divided into
six groups each contains six members so in both section I was being with my fellow collegus,
we started with ware house then ILS packing, then sales and the last was disp atch department

WARE HOUSE
This is the department deals with receiving order from sales section then processing sales orders
readily for packing and picking customer consignment, processing picked consignment into
system and hand over to dispatch section. Things I lent in this section are

o Receive sales orders from sales section.


o Processing sales orders readily for picking.
o Picking customer consignment.
o Processing picked consignment into system and handling over to dispatch section.
SALES DEPARTMENT
This is the section is section deals all sales and payment of dungs and other medical
supplies purchased in MSD .Things I learnt in this section are
o Filling of sales invoice.
o Calculating of costs of normal drugs
o How to issue vertical drugs.
o Sales and its category which are ILS and vertical and normal sales.

Something to note in this section is, the drug should be issued even if they have not being paid
be for.

ILS PACKING DEPARTMENT.


Is the section deals with packing of drugs for internal local supply like health centers and
dispensary In this section I leant th following.

o To identify the required medical supplies for packing.


o Pick and packing of medicine and other medical supplies to their respective boxes.
o To avoid damage of fragile the Havier material should be kept first in the box to avoid
breakage.
o Sealing of boxes and readily dispatched and

DISPATCH WARE HOUSE DEPARTMENT


This is department where all material are dispatched to customers for distribution to
the user unit i.e. hospitals.
MSD perform their function through the following way;

The MSD divides the health facilities in three category A, B and C, they make this in order to
simplify the distribution of drugs and other medical facilities .Each group receive drug fourthly
per year. Each group available, then the order is picked and packed in the boxes. In the last box
the forms like sale invoice, verification and claim are packed.

ST. PHILIPO HEALTH CENTER

Also there is different department. I was able to conduct my field practice in different department
which are; Eye Department, Main store, Supply store, Inpatient Department, Infusion
Department, CTC Store and finally OPD Department.

So, in whole department I was with my two colleges we started with; Eye department, Main
store, Supply store, CTC store, in patient department and lastly OPD.
EYE DEPARTMENT
This is the department concerned with the supply of drug for treatment of eyes.

Activities done are:


Dispensing of drug for eye treatment e.g. ciprofloxacin eye drop and chloramphenicol eye
drop.

Ordering of drugs from main store.

Recording of daily dispensed drugs.

Challenges:

 Doctors hand write


 The need of high concentration on calculating the drops for patient use.

Difficult to dispense due to use of many abbreviation like BE, LE, Occult and LE
MAIN STORE
o Documentation of issued and received drugs in bin cards and store ledger.
o How vertical programs drugs is issued and received and also requested from MSD or
other supplier

Challenges:

 Wrong documentation due to receiving and issuing of large amount of drugs in different
categories.
 It take time to arrange the drugs in the shelf due to large amount of drug.
 Issuing of unnecessary drugs to dispensing unit caused by receiving of many different
requisition forms.

CTC store

This is the store that used to keep ARVs drugs for AIDs patients and also anti-tuberculosis drugs.

Activities:

 Arrangement of drugs according to FEFO and its categories.


 Issuing and receiving ARVs drugs.
 Separation of expired and unexpired drugs.
 Documentation of issued and received medicine.
The main challenge in this section is to count the tins and box of drugs in order to document
actual amount of drugs.

INPATIENTS DEPARTMENT
This is the department that supply drugs to inpatients that admitted to hospital wards via ward
nurses

Activities:

 Ordering of drugs from main store.


 Dose calculation and total amount of drugs for in patient by considering patients file.
 Documentation of issued and received drugs in store ledger and bin cards
 Arrangement of medicine according to dosage forms and its pharmacological effects.
 Calculating of costs of drugs for discharged patients.

Challenges:

 Doctors hand writings.


 Calculation of drugs for admitted patients.
 High numbers of patients files

INFUSION SECTION
This is the section deals with preparation of electrolytic intravenous fluid like normal
saline solution and dextrose solution and drugs for irrigation like normal saline 0.9%.

Activities I practiced in this section are

o Visual in inspection of packed iv drug by using viewing lamp.


o Rinsing of bottles for packaging of drugs
o Filtration of dextrose by using sintered glass
o Filling the bottle with drugs.
o Filtration of water by reverse osmosis (RO)
o Sterilization of infusion by using autoclave machine
o Preparation of monitor.
o Rinsing of sintered glass by using chemical. Example of chemical used in rinsing of
sintered glass are 100ml of permanganate, conc sulfulic acid 94 to 98%

The picture above is an autoclave used for sterilization of drugs under 121 0c for 15mins under
pressure of 1.2bar.

Challenges

o Too difficult to measure accurate amount drug by using beam balance.


o Lack of enough equipment for preparation.
o Distance between sterilization and preparation unit.

NB: something to note in this department is always to maintain cleanness in order to avoid
contamination of microorganism and pyrogens to the prepare

OUT PATIENT DEPARTMENT (OPD)


This is the department concerned with dispensing of drugs to patient who are note
admitted to KCMC ward. Most drug in this department are taken under care of heath personnel.
ACTIVITIES IN THIS DEPARTMENT.

o Dispensing of outpatient drug according to prescription, condition of patient, dose, age


and duration.
o Recording of dispensed drugs in daily dispensing register.
o Direct talking with patient about his or her problems.
o Arrangement of drugs in shelves according to pharmacological effect and dosage forms.
o Pre packing of drugs for better management of time.
o Documentation of drugs received issued and received from in long term records
example ledger book.
o Calculating of costs of drugs.

CHALLENGES

 It take a time to calculate amount of drugs and dose required to patient.


 Number of patient is become more due lack of experience in dispensing.
 Patient complying on medicine.
 It take a lot of time to understand the location of medicines
 Difficult to read the prescription due to doctors hand writings.

NATIONAL HEALTH INSURANCE FUND (NHIF)


This department is concerned with issuing of drugs to the patient who have NHIF
identification card. In this department drugs are arranged in small shelves and also there is place
for keeping pre packed medicine.

Activities in this section.

o Dispensing of drugs to the patient with appropriate direction on how to use.


o Documentation of drug dispensed in daily dispensed register.
o Arrangement of drugs in shelve according to dosage forms and their therapeutic actions.
o Calculating of cost of drugs dispensed by using NHIF forms.
o Pre packing of drugs
Calculation of drugs to be dispensed.

The challenge is many of drugs are out of stock and also there are many forms that require to be
filled.

OPD DEPARTMENT
This department is dealing with dispensing of drugs to outpatient only. Drug dispensed in this
section is only essentials.

Activities

o Dispensing of prescribed drugs to the patient with appropriate direction on how to


use those drugs.
o Recording of drugs dispensed in daily dispensing register.
o Ordering of drugs from main store by using requisition forms with considering
consumption rate of drugs.
o Documentation of issued and received drugs to ledgers

The challenges was the poor working environment due to lack of enough working places. Also
the other is most of drugs are not present so patient are directed to go to buy in street pharmacy
example amoxicillin capsule, metronidazole tables and cotrimoxazole were not present.
IN PATIENT DEPARTMENT
In this department deals with dispensing of drugs and other medical supplies to the patient who
admitted in wards. Also in this department drugs for psychosis are dispensed and preparation of
jik for hospital use.

ACTIVITIES

o To receive files and dispensing of inpatient drugs and psychotic drug e.g.
Chlorpromazine, haloperidol, carbamazepine and phenobabitone. Also other materials
issued are Gloves, giving set, spirit and bectal.
o Recording of drug dispensed in drug dispensing register.
o Ordering of drugs from main store by considering consumption rate by using
requisition forms.
o Preparation of jik for hospital use.
o Issuing of to ward admitted patient via nurses wards.

Challenges

o Most drugs in hospital are out of stock.


o Doctors hand writings

CTC DEPARTMENT
Is the department deals with dispensing Anti-Retroviral to patient affected by HIV/AIDS And
also giving them appropriate instruction on how to use those drugs.

I learnt the following from this department.

 The five group of ant retroviral drugs which are


1. Nucleoside reverse transcriptase inhibitor. Example Zidovudin (AZT), Lamivudin
(3TC), Emitricitabin (FTC) and Abacavir (ABC).
2. Non- nucleoside reverse trascriptase inhibitor (NNRTI). E.g. Nevirapine (NVP)
and Efavirenz (EFV)
3. Protease Inhibitors (PIs) e,g, Ropinavir and ritonavir
4. Integrase inhibitors (TIs)
5. Fusion Inhibitors (FI)
 Recording of dispensed drugs in RNR on both manual and computerized.
 How to start drugs to patient and to shift patient from first line to next line of treatment.

In our country we use only first three group because of low economy.

ARVs drugs must be used in combination.

Cotrimoxazole is the drug of choice for prevention of opportunistic infection and busting of body
immunity.

The main challenge in this department is patient complying about the late to give them drugs.

Note
In this department the important thing to capture is to be very careful on drug that you
dispense because now days the drug that was in single now there in combination, so that to
avoid over dose or under dose to patient.

RECOMMENDATION
Generally the government should give high priority to health sector so as to save the peoples in
order to facilitate the development in our country.
