Notes

● The “Start Date” of the Tx Plus must match the date that the client and/or legal guardian signed the
Treatment Plan.
● The “Target Date” and the “End Date” of the Tx Plus should be “one year, minus one day” from the “Start
Date” Only enter the “End Date” in the top blue box.
● Select the BLANK “Program” from the drop down menu. ​Note: Credible may auto-assign the program,
so please manually select the BLANK “Program” when completing the Tx Plus from the service.
● For clients receiving more than one mental health service (IIH, OPT, TFC, DT, Med Mgmt, etc), remember
to include ​all ​providers in the “Who is Responsible?,” ​all ​services in the “Frequency & Service,” and a
description of each service in the HOW (Support Interventions).

Template: Enhanced-Benefit Services (IIH) Tx Plus

Characteristics: ​Diagnosis
Description: ​Symptoms, behaviors, and concerns based on CCA; “What does this diagnosis look like for this
specific client?

Long Range Outcome: ​Summary/Keywords of long range goal, i.e. Reduce/manage symptoms of diagnosis
Description: ​Long term goals; in the words of the client/family (found in CCA under Goals)
Where am I now?: ​Brief summary of what is going on in their life and where they are at in terms of the goal
they want to reach.

WHAT (Short Range Goal): ​Brief phase, capturing the target of the goal
Description:​ SMART, Short term goal - Individual; include client quotes, related to client diagnosis
Who is Responsible?: ​Client and family, Providers/Agencies
Frequency & Service: ​Service and Frequency, for each service provided

WHAT (Short Range Goal): ​Brief phase, capturing the target of the goal
Description: ​SMART, Short term goal - Family; include client/family quotes, related to client diagnosis

Description: ​EBTs, for each service provided, i.e. Motivational Interviewing (MI); Cognitive Behavioral
Therapy (CBT); Structural Family Therapy (SFT) and any other practices that the Team is trained on.

Don’t forget to add in the description of WHAT/objective..what the client will do “as evidenced by: client
report, therapeutic observations and psychiatric report, teacher report” and any other behavior added

Example: Enhanced-Benefit Services (IIH) Tx Plus

Characteristics: ​ODD, ADHD


Description: ​Defiance toward authority figures at home and school, temper tantrums, verbal and physical
aggression including arguing, yelling, hitting, throwing things, difficulty following directions,
difficulty paying attention.

Long Range Outcome: ​Reduce/manage symptoms of ODD, ADHD


Description: ​Client: “I don’t want to get in trouble anymore”
Mother: “I just want him to do what he’s supposed to do, and be happy.”
Where am I now?: ​Client was referred for services by School Counselor, due to concern about aggressive
behaviors in the home and school. Client previously received School-Based Outpatient
Therapy Services, but behaviors have continued to escalate during the past several
months. Client has been recommended for IIH Services at this time.

WHAT (Short Range Goal): ​Reduce aggression


Description: “Quote from client or guardian.” ​Client will identify and practice at least 5 coping strategies to
reduce aggressive behaviors at school and home per client, parent, and teacher report.
Who is Responsible?: ​Client and family, Access Family Services IIH Team & Psychiatric Provider, Kathy
Hayes, PA-C
Frequency & Service: ​Intensive In-Home Services, up to 32 sessions within 60 days; Psychiatric Services for
medication management and case oversight, once a month or as needed.

WHAT (Short Range Goal): ​Reduce family conflict


Description: “Quote from client or guardian.”​Client and family will learn strategies to communicate
without yelling, and practice those strategies during conflicts consistently per client and parent report.
Who is Responsible?: ​Client and family, Access Family Services IIH Team, Kathy Hayes, PA-C
Frequency & Service: ​Intensive In-Home Services, up to 32 sessions for the first 60 days of treatment;
Psychiatric Services, up to Once a Month or as needed.

HOW (Support Interventions): ​IIH Services, Psychiatric Services


Description: ​IIH: Provide case management including monthly CFT Meetings, coordination with other
service
providers, and referrals to community resources as needed; EBTs including MI - open ended
questions, reflective statements, assessing/enhancing motivation to make changes within the
family; CBT - identifying/expressing thoughts and feelings, coping strategies to manage
emotions, practicing alternative thoughts and behaviors; SFT - ecomaps, genograms, identifying
core interactional patterns within the family, enactments to improve communication and family
relationships, parenting strategies to support behavioral changes within the family. Crisis
stabilization as needed.
Psychiatric: Provide case oversight, medication management evaluations, and treatment
recommendations for client and family.

Mother: “I just want him to do what he’s supposed to do, and be happy.”
Where am I now?: ​Client was referred for services by School Counselor, due to concerns about difficulty
completing assignments and recent decrease in academic performance. Client has been
recommended for School-Based Outpatient Therapy Services at this time.

Objective: ​Increase following directions


Description: ​Client will identify and practice at least 5 strategies to improve focus, following directions within
more consistently per client, parent, and teacher report.
Who is Responsible?: ​Client and family, Access Family Services Outpatient Therapist and Psychiatric
Provider; School - Owen Middle
Frequency & Service: ​Outpatient Therapy Services, up to 1-2 sessions a week; Psychiatric Services up to
Once per Month or as indicated by Provider; Academic Supports as Needed

Intervention: ​Outpatient Therapy; Psychiatric Services; Academic Supports


Description: ​Outpatient: EBTs including MI - open ended questions, reflective statements,
assessing/enhancing motivation to make changes within the family; CBT - identifying/expressing
thoughts and feelings, coping strategies to ma;
‘nage emotions, practicing alternative thoughts and
Behaviors. Psychiatry: Evaluate and provide medication management. Academic Supports:
School will follow IEP plan and provide prompts/redirection as well as positive reinforcements.
School and therapist will coordinate as needed.
