AFFIDAVIT OF DISCREPANCY OF NAME WITH UNDERTAKING

I, MARIA CARLA PATRICIA B. TORRE, of legal age and resident of Iloilo city, under oath declares
that:

1. CARLA B. TORRE as appearing in my college transcript of records in the UP Visayas is the

same person as MARIA CARLA PATRICIA B. TORRE which is my name as indicated in my
certificate of live birth.

2. The discrepancy of name was discovered only when I got my transcript of record after
graduating from their Bachelor of Science in Biology program in the said University.

3. It was planned already in the past to change the entry of my name from CARLA B. TORRE to
MARIA CARLA PATRICIA B. TORRE but was not pursued for lack of time.

4. I am executing this sworn statement, as required, for me to be able to qualify to take the
Licensure Examination for Teacher (LET) with promise to change the entry of my name in
the college transcript of records from CARLA B. TORRE to MARIA CARLA PATRICIA B. TORRE
the soonest time possible.
